What is the Curative Baseline Visit?

The Baseline Visit has two parts: onboarding with a Care Navigator and a one-on-one clinical check-in over Zoom. Everyone aged 18 and older on the plan must finish it within 120 days of the effective date to keep $0 copays and $0 deductibles in network.

What Does Curative Pay For at Each Level of Care?

Curative’s published pages name inpatient treatment, virtual IOP, outpatient substance use services, counseling, psychiatry and medication management. Detox, residential and partial hospitalization are not named by level on those pages, so a member heading for one of them should call Member Services on 855-428-7284 and ask how that level is covered and authorized before a date is set.

Medical Detox

Withdrawal management is not named on Curative’s substance use pages, which is a gap worth closing by phone rather than by assumption. Ask whether medical detox is handled as an inpatient admission under the general medical benefit and what the admitting clinician has to submit. Alcohol and benzodiazepine withdrawal can produce seizures, and acute withdrawal is an emergency department matter regardless of network questions.

Residential and Inpatient

Curative names inpatient treatment through Recovery Unplugged in Austin and in Fort Lauderdale, at $0 for members aged 18 or older who have completed the Baseline Visit, with family members on the plan also eligible. An elective admission needs authorization three business days in advance; a non-elective admission needs notification within one business day.

PHP, IOP and Outpatient

The virtual intensive outpatient program through Recovery Unplugged is available online in Texas and Florida. Outpatient substance use services, individual, couples and family therapy, psychiatry and medication management are all listed at $0 in network, with no session limit stated on Curative’s mental health pages. Partial hospitalization is not named, so treat it as a question for the Care Navigator.

Does Curative Need Prior Authorization for Rehab?

Sometimes. Curative’s provider manual states that nine in ten medical services need no prior authorization, but elective admissions and listed outpatient services require authorization three business days ahead, and non-elective inpatient admissions require notification within one business day. Curative runs concurrent review and discharge planning on inpatient admissions and observation stays.

Clinical Care Management sits inside Curative rather than with an outside vendor, and the prior authorization line is 855-414-1089, with a fax of 877-942-4448. The manual refers to internal guidelines drawn from published medical evidence; ASAM, InterQual, MCG and LOCUS are not named, so a clinician arguing for a level of care should ask which criteria are being applied. No behavioral health authorization list is published.

Do I Need a Referral to See a Therapist or Prescriber?

No. Curative states that no referral is needed to see a therapist or psychiatrist and directs members straight to Provider Search. The same door covers virtual and in-person appointments, and the Rula partnership includes 24/7 crisis support. Curative publishes no separate behavioral health phone line, so Member Services on 855-428-7284 is the number to use.

Out of network cuts both ways. EPO and EPO Value plans pay in network only. On PPO and PPO Max there are out-of-network benefits, but charges above the allowable amount fall to the member on top of the deductible and coinsurance. Cash Card transactions also stay subject to coverage, clinical guideline and prior authorization review.

How Much Does Rehab Cost With Curative Health Plan?

In network, $0, once the Baseline Visit is done. Without it, a $5,000 individual or $10,000 family in-network deductible applies, plus the copays set by the plan. In one named Texas group, the Calallen ISD 2025 plans, the non-completion schedule was a $25 office visit copay, $50 specialist copay, 20% medical coinsurance and 25% pharmacy coinsurance on top of that deductible.

Out-of-network figures vary by plan. The PPO carries a $10,000 individual and $20,000 family out-of-network deductible with a $15,000 or $30,000 out-of-pocket maximum. PPO Max carries a $0 out-of-network deductible with a $7,500 or $15,000 out-of-pocket maximum. In the Calallen ISD 2025 set, PPO+ ran a $5,000 and $10,000 out-of-network deductible at 20% coinsurance, the PPO $10,000 and $20,000 at 50%, and the EPO had no out-of-network coverage at all.

Those are the sums a program would bill without coverage, and they are the reason the Baseline Visit is worth an hour. The Curative Cash Card, formerly the Zero Card, is a separate route: it is used with providers labelled Cash Card only in Provider Search and at any urgent care, counts as payment in full, and lists behavioral health and select inpatient and outpatient hospital services among eligible services. It is open to members aged 18 and over for the first 120 days, then all year once the Baseline is done.

Does Curative Cover Suboxone, Methadone and Other Addiction Medication?

Curative’s substance use page names medication-assisted treatment as an option and sets preferred medications at $0 in network, but it does not name individual drugs. The formulary runs preferred drugs in Tier 1 and non-preferred drugs in Tiers 2 and 3, with non-preferred prescriptions at $50 or $250 at in-network pharmacies once the Baseline is complete.

Curative’s published opioid prior authorization policy covers the fentanyl patch, hydrocodone and acetaminophen solution and oxycodone solution for pain. Buprenorphine, methadone and naltrexone do not appear in it, so no authorization requirement is published for them. Formulary placement for those three was not readable on the public drug search, which makes a call to the pharmacy team the sensible step before a discharge prescription is written.

Where the Prescription Is Filled

Pharmacy benefits are managed in house by the Curative PBM team, and Curative Pharmacy is an in-house pharmacy for members with delivery to home or work in most states, reachable on 833-428-7284. Standard plans use a network of more than 30,000 pharmacies; PPO Max widens that to more than 60,000 including CVS, Walgreens and Walmart. One 2025 Texas group ran mail order plus select retail chains instead.

Methadone for opioid use disorder is dispensed through licensed opioid treatment programs rather than a retail counter, so it is authorized as a program service where it is covered at all. Ask the admissions team which route applies before the first dose is due.

Which Rehab Centers Are In-Network With Curative Health Plan?

A program is in network with Curative when it holds a contract that reaches the member’s specific product, either directly with Curative or through the network the plan rents. From January 1, 2026 that is the Cigna Healthcare PPO Network or the Curative Wrap Solution, which includes the HealthSmart Preferred Network. Network status is settled in Curative’s provider search and by the plan named on the member card.

Programs that accept Curative are gathered on the Curative Health Plan page, with residential rehab and other levels of care listed separately. Each listing shows the payments the program says it accepts, its licensing and accreditation, and how far it sits from the nearest airport, so a shortlist can be drawn up before the phone call that settles network status and authorization.

Curative also takes direct provider contracts in the District of Columbia, Florida, Georgia, Indiana, Maryland, Ohio, Texas and Virginia, and its provider manual allows a market on a wrap network to be switched to a Curative direct network with 30 days’ notice. Members can nominate an out-of-network provider to accept the Cash Card, which is worth raising with an admissions team that is not currently contracted.

How Do I Find a Rehab That Takes Curative Health Plan?

Start with Curative’s provider search, where in-network entries are the $0 ones, or call Member Services on 855-428-7284, which runs 24/7, and ask for a Care Navigator. The same number handles questions about Recovery Unplugged. EPO Value members use the Curative Pass tool in the Member Portal instead, where Cash Card only providers and any urgent care location are $0.

Have three answers ready before the admissions call: which plan type the card shows, whether the Baseline Visit is done, and whether the program sits in the Cigna Healthcare PPO Network or a Curative direct contract. Those three settle the cost long before anyone discusses admission dates.

Is Curative the Same as Cigna or First Health?

No. Curative Insurance Company is an independent insurer, first licensed by the Texas Department of Insurance and owned by Curative Inc., which put $100 million of capital into it. It rents the Cigna Healthcare PPO Network as its wrap network from January 1, 2026, replacing First Health. Curative is not a Blue Cross Blue Shield licensee and holds an AM Best A- rating.

Before you call a program: complete the Baseline Visit if the 120 days have not run out, check the plan type on the card, and ask the admissions team whether it bills Curative directly or through the wrap network. Claims go to payer ID CURTV with a 95-day filing window, which is why an out-of-network admission is worth settling in advance rather than after discharge.
