Drug and Alcohol Rehabs in California

California recorded 9,028 drug overdose deaths in 2024, a rate of 22.9 per 100,000 residents and around 6 percent below the national figure.

That is a state doing better than most of the country, and it is still nine thousand people.

The improvement was real and it was concentrated. Los Angeles County recorded 2,438 overdose deaths in 2024, down from 3,137 the year before — a 22 percent fall and the largest single-year decline on record there. Fentanyl deaths in the county dropped 37 percent, from 2,001 to 1,263.

But the substance driving California is not the one driving the national conversation. Methamphetamine was involved in 57 percent of the state’s overdose deaths in 2024 — more than any other drug, fentanyl included.

That matters because meth has no equivalent of naloxone and no approved medication comparable to buprenorphine or methadone. A treatment system built around the opioid crisis is not automatically built for the crisis California actually has.

The second thing to understand is that California is not one treatment market. It is dozens, and they differ more from each other than most states differ among themselves.

Where you live determines whether the nearest program is a walk, a bus ride or a ninety-minute drive over a mountain grade; whether it operates in your language; and whether the county you live in has opted into the system that guarantees an assessment.

This page sets out how treatment works statewide and where the differences fall. Every figure comes from a named source, and the county and city pages beneath go further into local detail.

Which Level of Care Do You Need

California has more licensed treatment capacity than any other state, and the composition matters more than the total. What follows is what each level involves and how well provided it actually is here.

Medically Supervised Detox

Detox manages withdrawal under clinical observation, usually over three to seven days for alcohol and five to ten for opioids. Alcohol and benzodiazepine withdrawal can be medically dangerous without supervision, which is the reason this level exists as a distinct step rather than a formality.

Around two in five programs listed in this directory statewide offer detox. It is well provided in the coastal cities and thin in the interior — Alpine and Campo in the San Diego backcountry, and much of Imperial County, sit a long way from the nearest bed.

Inpatient Residential Treatment

Residential treatment means living at a program for 30, 60 or 90 days. It suits people who have completed detox, who have tried outpatient care without it holding, or whose home circumstances make stopping difficult. That last one describes a household rather than a person.

Roughly a third of listed programs offer residential care. It is worth saying plainly that in remote parts of the state — Borrego Springs, Ramona, the desert and mountain communities — residential treatment is often the more practical option rather than the more drastic one, because it replaces dozens of long journeys with a single one.

Outpatient, IOP and PHP Treatment

Partial hospitalization runs most of the day several days a week. Intensive outpatient runs a few hours several times a week. Standard outpatient is lighter again, and most people step down through these rather than stopping after residential care.

Outpatient services are near-universal across listed programs, IOP is available at around two in five and PHP at around one in four. Telehealth is offered by roughly four in five, which in a state this large is not a convenience but the difference between a plan that holds and one that quietly stops.

Dual Diagnosis and Co-Occurring Disorders Treatment

Around half of people with a substance use disorder also have a diagnosable mental health condition, and treating one while leaving the other alone rarely holds.

Roughly two in five listed programs statewide indicate they treat co-occurring conditions, and the distribution is uneven enough to be the single most useful question to ask before admission. Some catchments run well above that figure; others, including several across San Diego County, sit at or near zero. If depression, anxiety, post-traumatic stress or bipolar disorder is part of the picture, ask for integrated treatment by name rather than assuming a referral will find it.

Sober Living Homes and Transitional Living

Recovery residences provide drug-free housing after formal treatment ends, and this directory does not list them separately. California certifies them through the California Consortium of Addiction Programs and Professionals and through regional affiliates of the National Alliance for Recovery Residences.

Standards vary widely between operators, and certification is a fair thing to ask about directly. Where a residence is run by the same organization as a treatment program, it is reasonable to ask whether a referral between the two is clinical or commercial.

Addiction Treatment Cost and Insurance Coverage

California has the widest range of treatment prices in the country, from no-cost county placement to executive programs charging more per month than most households earn in a year. Which end applies to you is usually settled by one phone call.

How Much Does Addiction Rehab Cost in California

Those are private-pay figures and most Californians never encounter them. Beyond Medi-Cal, several hundred SAMHSA-listed substance use facilities in the state operate a sliding fee scale based on income, and a further group offer payment assistance. The awkward position is being uninsured and just above the Medi-Cal threshold, and even then a sliding scale often exists within reach.

Does Insurance Cover Rehab in California

Parity is enforceable rather than aspirational. If a plan approves shorter stays, demands more frequent reviews or maintains a thinner network for substance use treatment than for comparable medical care, that is a documented parity concern and grounds for appeal. The California Department of Managed Health Care handles complaints against most commercial plans in the state, and the Department of Insurance covers the rest.

Two things people often do not know. California extended full-scope Medi-Cal to income-eligible adults regardless of immigration status, and immigration status does not appear on a substance use referral. And Medi-Cal must provide interpretation at no cost to the member, so being told a program cannot work in your language is a reason to ask for a different placement rather than to accept less.

Free and State-Funded Addiction Rehab in California

The most important thing on this page is a system most Californians have never heard of.

The Drug Medi-Cal Organized Delivery System gives a Medi-Cal member in a participating county the right to an assessment against American Society of Addiction Medicine criteria and a referral to whatever level of care that assessment indicates — anywhere in the county, not merely whatever happens to be contracted nearby. As of January 2025, 40 of California’s 58 counties participate.

Every Southern California county runs it. Riverside went live in February 2017, San Bernardino in March 2018, San Diego and Imperial in July 2018. That is not true across much of the north, and it is the sharpest structural difference between Southern California and Northern California.

If you live in a participating county, the county access line is almost always the right first call rather than the last. It costs nothing, it operates around the clock, and because it places countywide it is the route past a thin local market.

Alongside it: the California Naloxone Distribution Project supplies naloxone free to community organizations, and several hundred facilities statewide are funded through Indian Health Service, Tribal and Urban Indian sources, where eligibility rests on tribal membership or descent rather than income and runs alongside Medi-Cal rather than instead of it.

More Help and Recovery Support

Where the differences fall. Southern California holds the larger share of listed capacity and the greater extremes. Los Angeles and San Diego between them carry most of the state’s urban provision, with Downtown San Diego, East Village and Hillcrest among the densest concentrations anywhere in California. Orange County carries a large commercial market around Irvine, Anaheim and Santa Ana. Inland, Riverside County, San Bernardino County and the desert cities of Palm Springs and El Centro are thinner, and the backcountry — Alpine, Campo, Pauma Valley, Borrego Springs — thinner still.

Language. Spanish is provided by a substantial share of SAMHSA-listed facilities statewide, and sign language by several hundred. Beyond those two, provision drops sharply: Tagalog, Farsi, Chinese, Russian, Vietnamese, Arabic, Korean and Hmong are each offered by fewer than a hundred facilities in the whole of California. In districts like City Heights, Koreatown and Logan Heights, that gap between the community and the provision is the practical obstacle rather than cost or distance.

Medication. Buprenorphine is prescribed widely across the state and naltrexone almost as broadly. Methadone is different: California has roughly 129 certified opioid treatment programs in total, and early methadone treatment requires daily attendance at one. From most of Southern California that is workable; from Borrego Springs or the Mountain Empire it is not, and buprenorphine is the option worth raising first.

Larger markets: Los Angeles, San Diego, Long Beach, Santa Ana, Anaheim, Irvine, Riverside, San Bernardino, Chula Vista, Pasadena, Oceanside, Escondido, El Cajon, Bakersfield, Santa Barbara, Ventura. Crystal Meth Anonymous has a significant presence in the coastal cities, which given what the state data shows about methamphetamine is worth knowing. SMART Recovery and Refuge Recovery meet in most metropolitan areas for anyone who prefers a non-twelve-step approach.
