Methadone Clinics in California

Methadone for opioid use disorder in California is dispensed only by licensed narcotic treatment programs, the state’s name for what federal rules call opioid treatment programs. A regular pharmacy can’t fill methadone for this purpose. Most people visit a clinic daily at first, and take-home doses may become possible later.

Below is a plain guide to starting treatment, take-home rules, paying through Medi-Cal, Medicare or a private plan, and the state office that oversees clinics. For a wider look at care for opioid use in California, including residential and counseling options, see opioid rehab in California.

Common questions about Methadone Clinics

Answered from what is listed here.

Which California cities have methadone clinics?

Fresno leads the California cities where methadone clinic listings cluster, followed by Lancaster, El Cajon, North Hills and Murrieta. Opioid treatment programs also serve people around Los Angeles and San Francisco. Daily dosing is common early in treatment, so a clinic close to home or work can matter a great deal.

Rehab Centers in Fresnohttps://rehabseekers.com/locations/us/california/northern-california/fresno/
Does Medi-Cal cover methadone treatment in California?

Nearly all of the methadone clinics in California record accepting Medi-Cal. Coverage runs through county-contracted Drug Medi-Cal providers and, in counties that opt in, the Drug Medi-Cal Organized Delivery System, rather than through one statewide list of plans.

Members can ask their county alcohol and other drug program which nearby clinics take Medi-Cal for methadone, and intake staff can confirm whether a clinic contracts with the member's county before the first visit.

Rehab Centers in California That Accept Medi-Calhttps://rehabseekers.com/locations/us/california/medi-cal/
Which methadone clinics in California are close to a river?

The Los Angeles River comes up first among rivers named near California methadone clinic listings, ahead of the South Fork Fresno River and the South Fork Santa Clara River. Nearly all of the methadone clinics in California are within 25 miles of a river, a median of about 10.2 miles from the nearest one.

How do methadone clinics work in California?

A methadone clinic in California is a narcotic treatment program licensed by the state and certified under federal rules to dispense methadone, usually alongside counseling. Each program also holds a DEA registration, and many offer buprenorphine or naltrexone as well. Nursing staff give doses on site, often starting early in the morning so people can dose before work. Dose changes happen under medical supervision and are adjusted gradually over days and weeks.

Methadone programs in California are concentrated in cities and larger towns. In our listings, 67% of methadone programs are in urban areas. Fresno and Lancaster are among the places that appear most often. Southern California, the Bay Area and the Sacramento region have the most programs, while the far north, the Sierra and parts of the Central Valley have far fewer.

Travel time matters when dosing is daily. Ninety percent of listed methadone programs are within 30 miles of an airport, at a median of 8.9 miles, and Hollywood Burbank is the airport most often closest. If you travel, ask your clinic to arrange guest dosing at another program. If you move, ask about a formal transfer. Some people choose buprenorphine from an office-based prescriber instead, filled at a pharmacy. Those options appear on the Suboxone clinics in California page.

How do you start methadone treatment, and can you begin the same day?

Starting methadone in California usually means an assessment and a medical exam at a licensed program. Some clinics give a first dose the same day or the next day. Timing depends on each program’s staffing and capacity, so ask the admissions team rather than planning on same-day entry. Expect questions about your opioid use, other medicines and health conditions, along with a drug test. Medi-Cal guidance allows the first Drug Medi-Cal assessment to happen by video, though a clinic may still want an in-person exam before the first dose. Video-based care is covered on the telehealth rehab in California page.

California’s automated SUD Non-Emergency Treatment Referral Line, (800) 879-2772, connects callers to their county alcohol and drug program. Each county also publishes its own access line, and many of those lines are listed as open around the clock.

What to bring to intake

Bringing a photo ID, a Medi-Cal or insurance card, a list of your current medicines, and any records from a past program can speed up intake. The first visit often takes several hours.

Who gets priority for admission?

Pregnant people receive preference in admission under California’s block grant policy. If a program has no space, it must provide a referral or interim services within 48 hours. More options are on the rehab for pregnant women in California page. Under state law, minors 16 and older may consent to opioid use disorder medication at a licensed program where federal law expressly permits it. Younger teens need parental consent.

When can you get take-home doses of methadone in California?

Take-home methadone in California depends on time in treatment, stability, and the clinic physician’s judgment. These decisions follow federal rules in 42 CFR Part 8, which were revised in 2024, along with state requirements. The 2024 update allows take-home doses earlier in treatment than the older rules did. News stories describing earlier limits may be out of date. Each program applies its own policy within state and federal rules, so ask how its schedule works during intake.

Safety becomes more important once doses leave the clinic. Methadone can slow breathing, and the risk rises when it is mixed with alcohol, benzodiazepines, sleep medicines or other sedatives. Tell clinic staff about everything you take. Store take-home bottles in a locked box away from children and visitors. Nasal naloxone is sold over the counter in California, and the state’s Naloxone Distribution Project supplies free naloxone to clinics and community groups. Ask your program for a kit.

Does Medi-Cal, Medicare or private insurance pay for methadone in California?

Medi-Cal pays for narcotic treatment program services through county-contracted Drug Medi-Cal providers. In counties that opted in, coverage runs through the Drug Medi-Cal Organized Delivery System. California opened Medi-Cal to expansion adults on January 1, 2014. More on coverage is on the Medicaid rehab page.

PaymentWhat to know
Medi-CalProvided through county Drug Medi-Cal providers; your county access line can point you to one.
Medicare Part BHas covered opioid treatment program services since 2020; see Medicare rehab in California.
Private insuranceCoverage and copays depend on your plan; ask whether a specific program is in network.
Self-payMany programs charge a weekly fee; ask for the current rate.
Public fundingCounty and grant-funded slots may be available; see state-funded rehab in California.

Who licenses and oversees methadone clinics in California?

The California Department of Health Care Services licenses methadone clinics through its Licensing and Certification Division. Within that division, the Counselor and Medication Assisted Treatment Section oversees narcotic treatment programs. Each program also needs federal certification and a DEA registration before it can dispense methadone. The department also runs the State Opioid Response program, which announced 2025 funding that included satellite medication units at narcotic treatment programs.

RehabSeekers.com listings show what each program reports: certification, medications offered, payment types and insurers, dosing hours, languages, populations served and location. The details come from public treatment registers, state license records, accreditation databases and the providers themselves, and they can change. Call ahead to ask about hours and openings. Related California pages include Spanish-speaking rehab, outpatient rehab and detox, and the California treatment page covers every option in the state.
