Treatment Centers in Colorado That Accept UnitedHealthcare

In Colorado, UnitedHealthcare insures drug and alcohol rehab under employer and level-funded plans, Medicare plans and, under its Rocky Mountain Health Plans brand, marketplace HMOs, Health First Colorado Region 1 and Child Health Plan Plus. Optum Behavioral Health manages the behavioral network for UnitedHealthcare members.

Every Colorado program listed below gives a direct phone line, a description of its care, from medical detox to residential treatment, and any accreditation from The Joint Commission or CARF. The details come from Colorado licensing records and government treatment registers, so a member can sort options before UnitedHealthcare’s authorization step.

Does UnitedHealthcare Cover Rehab in Colorado?

Yes. Substance use treatment is covered for Colorado members of UnitedHealthcare employer, marketplace, Medicare and Medicaid plans, though benefits differ by plan and each member can look up their own by signing in. A member comparing rehab facilities across Colorado needs to know which UnitedHealthcare line the card belongs to.

UnitedHealthcare bought Rocky Mountain Health Plans of Grand Junction in a deal announced in July 2016, and in Colorado it sells marketplace, Medicaid and Child Health Plan Plus coverage under that brand. Its employer plans and the Optum network carry the UnitedHealthcare name, and rehabs that accept UnitedHealthcare may belong to either side.

Does UnitedHealthcare Cover Inpatient Rehab in Colorado?

Yes. UnitedHealthcare covers inpatient and residential treatment in Colorado, including short-term residential with insurance and hospital addiction treatment, once Optum decides it is medically necessary by The ASAM Criteria, Fourth Edition. Colorado is also one of 24 states where Optum publishes its own set of behavioral clinical criteria.

How long will UnitedHealthcare pay for residential rehab?

UnitedHealthcare does not publish a set residential stay length for Colorado; Optum authorizes a period and then reassesses against ASAM placement. On the Rocky Mountain Health Plans marketplace HMOs, a grid effective January 1, 2026 requires authorization for residential Levels 3.1 and 3.5, and Region 1 Medicaid stays at Levels 3.1 through 3.7 need authorization under the April 2026 grid.

Does UnitedHealthcare need prior authorization for inpatient rehab?

Yes. For UnitedHealthcare’s Colorado employer members, Optum, the brand of United Behavioral Health, authorizes behavioral admissions through the behavioral number on the card. One 2025 Choice Plus certificate asks for authorization at least five business days ahead of a planned out-of-network admission and for partial hospitalization and intensive outpatient care. Skipping that step raises the out-of-network share to 50 percent.

Does UnitedHealthcare Cover Detox in Colorado?

Yes. Colorado detox on a UnitedHealthcare plan is covered at whichever withdrawal level the ASAM assessment supports, from residential detox programs to hospital-based detox. The 2026 Colorado marketplace grid lists ASAM 3.7 medically monitored inpatient withdrawal management among services needing authorization, so the program calls 888-478-4760 before admitting a Rocky Mountain HMO member, or 888-282-8801 to report an admission.

Does UnitedHealthcare Cover Outpatient Rehab in Colorado?

Yes. UnitedHealthcare covers outpatient rehab options and IOP covered by insurance in Colorado. Optum’s network manual says routine outpatient visits and medication management generally go ahead without authorization. Colorado marketplace members need only a notification when intensive outpatient starts, with authorization required after 15 sessions; partial hospitalization at Levels 2.5 and 2.7 needs authorization.

UnitedHealthcare’s confidential Substance Use Helpline at 1-855-780-5955 is staffed 24 hours a day by recovery advocates who can arrange a professional evaluation. For a Colorado employer member the helpline is a benefits step: it explains what the plan covers before Optum’s authorization, while Rocky Mountain plan members use their own plan line.

Does UnitedHealthcare Cover Suboxone, Methadone and Other Addiction Medications?

Yes. UnitedHealthcare’s Colorado employer drug lists, run by Optum Rx, include buprenorphine treatment and other addiction medicines. The Rocky Mountain Health Plans PRIME and Child Health Plan Plus plans also use Optum Rx, while Health First Colorado Region 1 members fill prescriptions through the state’s list.

Does UnitedHealthcare cover methadone?

Yes, for Medicare and Medicaid members. UnitedHealthcare’s Medicare plans in Colorado must match Medicare’s benefit, which pays methadone programs with no copayment after the Part B deductible. The Rocky Mountain Health Plans behavioral manual lists monitoring of methadone among covered medication management services, and employer members will find the rule in their own plan document.

Does UnitedHealthcare cover Suboxone?

Yes. UnitedHealthcare’s prescription drug list effective September 1, 2026 puts sublingual buprenorphine in Tier 1 and buprenorphine-naloxone films and tablets in Tier 2, both with quantity limits, so insured Suboxone treatment for a Colorado employer member costs a lower tier copay. Zubsolv also sits on Tier 2.

Does UnitedHealthcare cover alcohol use medications?

Yes. The September 2026 UnitedHealthcare list places all three alcohol use disorder pills, acamprosate, disulfiram and oral naltrexone, in Tier 1, so Campral with insurance, Antabuse covered by insurance and oral naltrexone treatment carry the lowest copay for Colorado members on that list. Naloxone nasal spray appears on the same list for overdose reversal at home.

UnitedHealthcare Rehab Centers Along Colorado Rivers, Near Lakes and in Wilderness Country

UnitedHealthcare’s employer network follows its members anywhere in Colorado, but its Medicaid role through Rocky Mountain Health Plans is centered on the Western Slope and the southern mountains. That makes the river valleys, reservoirs and wilderness counties of Region 1 the places where UnitedHealthcare brands handle the widest mix of coverage.

UnitedHealthcare rehab along the Colorado River

UnitedHealthcare’s PRIME plan covers Garfield County, where the Colorado River runs through Glenwood Canyon, New Castle, Silt and Rifle. Rehab centers near rivers and addiction treatment in Rifle are listed here; a PRIME member’s program asks Rocky Mountain Health Plans for authorization, while an employer member’s program asks Optum, so the card decides which desk gets the call.

UnitedHealthcare rehab near Blue Mesa and Taylor Park

UnitedHealthcare covers Gunnison County, home to Blue Mesa Reservoir and Taylor Park Reservoir, through PRIME and the Region 1 Medicaid organization. Rehab centers near lakes and programs in Gunnison use the Rocky Mountain Health Plans authorization line, 800-421-6204, for Medicaid stays, and the Optum number on the card for employer plans.

UnitedHealthcare rehab in wilderness country

UnitedHealthcare’s PRIME counties include Pitkin, home to the Maroon Bells-Snowmass Wilderness, and the Roaring Fork Valley down to Carbondale. Rehab centers near wilderness areas and Carbondale recovery programs fall under Rocky Mountain Health Plans for Medicaid and the Optum network for employer plans. Aspen, in the same county, is PRIME territory too.

How Do I Find a Rehab That Accepts UnitedHealthcare in Colorado?

Browse the listings on this page. All the Colorado programs shown take UnitedHealthcare in some form. Phone details, care levels and accreditation sit on every listing, and filters narrow the list by town and service. Call the program with your card in hand to learn whether it bills Optum, Rocky Mountain Health Plans or both.

UnitedHealthcare’s Colorado admissions to residential, inpatient or partial hospitalization care go through Optum on employer plans and through the Rocky Mountain Health Plans utilization team on that brand’s plans, usually at the program’s request. The table below gives each plan’s member line. Emergency care never needs authorization.

UnitedHealthcare rehab in Denver and Englewood

UnitedHealthcare reaches Denver-area members through employer and level-funded small business coverage, because Health First Colorado in Denver runs through Colorado Access rather than a UnitedHealthcare brand. Federal employees on GEHA plans also use the UnitedHealthcare Choice Plus network. Downtown Denver treatment programs and rehab centers in Englewood are listed, and Optum handles these members’ behavioral authorizations.

UnitedHealthcare rehab in Grand Junction

Rocky Mountain Health Plans, UnitedHealthcare’s Colorado Medicaid plan, is based in Grand Junction, where it was founded in 1974. Mesa County is both a Region 1 county and a PRIME county, so Medicaid members there have PRIME for physical health as well as Region 1 behavioral care. Downtown Grand Junction programs appear in the listings.

UnitedHealthcare rehab in Pueblo and Canon City

UnitedHealthcare’s Region 1 organization has handled Pueblo County Medicaid since July 1, 2025, replacing Health Colorado, Inc. across the southern counties under the new contracts. Fremont and Pueblo are also in its safety net service area. Downtown Pueblo rehab programs and treatment options in Canon City request authorization at 800-421-6204.

UnitedHealthcare rehab in Greeley and Loveland

UnitedHealthcare’s Colorado community plan site lists Northeast Health Partners, the Region 2 organization for Weld and Larimer counties, alongside the Rocky Mountain Health Plans plans, with its own line at 800-541-6870. Downtown Greeley treatment centers and rehab options in Loveland request Medicaid authorization there, while employer members in both counties go through Optum.

Can I use UnitedHealthcare for rehab in a different state?

Yes, if a Colorado member’s employer plan is Options PPO, Choice Plus or Surest, which reach the nationwide UnitedHealthcare and Optum networks. The Colorado marketplace HMOs do not cover out-of-network care except emergency and urgent care, and PRIME and CHP+ pay non-network care only in emergencies, for family planning or with prior approval.

How much does rehab cost with UnitedHealthcare?

What rehab costs on UnitedHealthcare in Colorado depends on the plan line. Region 1 Medicaid members pay nothing for substance use care, Child Health Plan Plus caps yearly costs at 5 percent of household income, and Surest replaces the deductible with set copays shown before care. Marketplace and other employer plans use deductibles plus copays or coinsurance.

UnitedHealthcare is only part of the payment picture in Colorado. The listings also show payment assistance programs and programs taking other insurers, and each facility page lists the plans it bills, so a member can sort out costs ahead of the authorization call. Members who lose a plan can compare other options here too.
