Treatment Centers in Idaho That Accept Molina

Molina Healthcare of Idaho covers drug and alcohol rehab for three groups: adults with both Medicare and Medicaid in Idaho Medicaid Plus, members of its dual special needs Medicare plan, and people who bought a Molina Marketplace plan for 2026. Its new Idaho Medicaid and Medicare dual contract began on January 1, 2026.

Listings below give each program’s phone number, levels of care and accreditation, drawn from government treatment registers and Idaho licensing records. Filter by city or by level of care, then call the program with the Molina card so staff can tell whether the program bills Molina, Medicare or the Idaho Behavioral Health Plan.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Does Molina Cover Rehab in Idaho?

Yes. Molina Healthcare of Idaho lists mental health services among the benefits of its Idaho Medicaid plan, its dual members keep Medicare’s substance use benefits, and its Marketplace plans carry no limits on essential health benefits. Members can review Idaho substance use programs alongside the statewide listings of rehabs that accept Molina.

Molina’s Idaho Medicaid product is Idaho Medicaid Plus, and it is not a general Medicaid plan: most Idaho Medicaid members get substance use care through the Idaho Behavioral Health Plan instead. Molina serves members with both coverages, so programs listed among Medicaid-accepting rehab programs and Medicare rehab programs matter to its members.

Does Molina Cover Inpatient Rehab in Idaho?

Yes, for dual members through Medicare first. Medicare pays for inpatient stays, and Molina’s Medicare behavioral health prior authorization form lists inpatient detox, residential treatment, partial hospitalization and intensive outpatient care as services it reviews. Medicare inpatient rehab and short-term rehab that takes Medicaid are listed statewide.

How long will Molina pay for residential rehab in Idaho?

Molina pays for as long as the stay remains medically necessary under the plan that is paying. For a dual member, Medicare’s rules come first and Idaho Medicaid fills in what Medicare does not cover. Short stays such as 21-day rehab with Medicaid are judged under the same authorization request the program files, with updates sent as the stay continues.

Does Molina need prior authorization for inpatient rehab in Idaho?

Yes. Molina’s Medicare behavioral health prior authorization form, in use since April 1, 2024, covers inpatient psychiatric and detox admissions, residential care, partial hospitalization, intensive outpatient and day treatment, along with non-participating outpatient providers. The admitting program files it. Idaho members can call Molina member services on (844) 809-8445, TTY 711.

Does Molina Cover Detox in Idaho?

Yes. Inpatient detoxification, voluntary or involuntary, is one of the services on Molina’s Medicare behavioral health authorization form, so dual members can use Medicare detox centers once Molina authorizes the admission. Medicaid-covered medical detox is also listed for members whose Medicaid pays the remaining share, such as Medicare deductibles and coinsurance.

Does Molina Cover Outpatient Rehab in Idaho?

Yes. Medicare covers outpatient therapy and counseling for dual members, and Molina’s authorization form lists Medicare PHP options and intensive outpatient care among the levels it manages. Medicaid IOP programs suit members who need several sessions a week while living at home, and Molina’s Idaho page lists Teladoc virtual visits.

Molina’s Idaho Medicaid plan is offered at no cost, and some services stay with Idaho Medicaid itself: developmental disability services, non-emergency medical transportation and dental are provided separately for members of the duals plans. Members with co-occurring conditions can look at Medicare dual diagnosis rehab in the listings.

Does Molina Cover Suboxone, Methadone and Other Addiction Medications in Idaho?

Yes. Dual members in Idaho Medicaid Plus keep Medicare Part D drug coverage, and Part D coverage is included in every special needs plan, including Molina Medicare Options Plus. Medicare also pays for opioid treatment program services directly, so Medicare MAT clinics bill Medicare for medication and counseling together.

Does Molina cover methadone in Idaho?

Yes, through Medicare. Medicare covers methadone, buprenorphine and naltrexone at Medicare-enrolled opioid treatment programs, together with counseling, therapy and drug testing, and Molina’s dual plans must cover at least what Original Medicare covers. Methadone dosing clinics in Idaho bill under that benefit, with the program’s counseling and drug testing billed under the same Medicare benefit.

Does Molina cover Suboxone for Idaho Marketplace and dual members?

Yes. Dual members fill buprenorphine prescriptions, including Suboxone, through their Part D drug list, and Marketplace members use Molina’s own drug list, since Marketplace plans must cover substance use disorder treatment as an essential health benefit. Buprenorphine maintenance with Medicaid is listed for members whose Medicaid also pays.

Molina Rehab Centers by Panhandle Marinas, Near Craters of the Moon and Below the Sawtooths

For Molina’s dual members, the county decides more than the setting does, because Idaho Medicaid Plus is mandatory only in the counties on the state’s list. Setting matters for travel and for which plan rules apply. Listings show each program’s distance from marinas, lava desert and mountains across Molina’s Idaho service area.

Molina rehab by the Panhandle marinas

The marinas of Lake Coeur d’Alene and Lake Pend Oreille sit in Kootenai and Bonner counties, both on the Idaho Medicaid Plus list, so dual members there choose Molina or UnitedHealthcare, and Marketplace members in the Panhandle hold a separate Molina card. Rehabs near marinas and Ponderay treatment programs are listed for that shoreline.

Molina rehab near Craters of the Moon

Craters of the Moon spreads its lava fields across Butte and Blaine counties. Blaine County is on the Idaho Medicaid Plus list and Butte County is not, so dual members in Arco may stay in Idaho Medicaid’s regular program. High desert rehab programs near Carey and Shoshone sit in listed counties.

Molina rehab below the Sawtooths

The Sawtooth Range rises over Blaine and Custer counties. Blaine County dual members enroll in Idaho Medicaid Plus, while Custer County, around Stanley and Challis, is not on the mandatory list, so dual members there may stay in the regular Idaho Medicaid program. Sawtooth-area mountain rehabs can tell callers whether they bill Molina for Medicaid services.

How Do I Find a Rehab That Accepts Molina in Idaho?

Browse the listings on this page. Each listing names the program’s city, levels of care and phone number, so a Molina member can call and ask whether the program bills Molina for Medicaid, Medicare or Marketplace coverage. Filters narrow the listings to detox, residential, intensive outpatient or medication programs.

Admission then runs through the plan that pays first. For dual members that is usually Medicare, with Molina’s authorization for inpatient, residential and partial hospitalization care. Idaho Medicaid lists Molina on 866-403-8293 for the duals plans, and Molina’s Idaho member services line is (844) 809-8445. The table shows Idaho Medicaid Plus status by county.

Molina rehab in Boise and Garden City

Ada County dual members aged 21 and over with Medicare Parts A, B and D and Enhanced Medicaid must join Idaho Medicaid Plus, choosing Molina or UnitedHealthcare. Their Medicare stays the same. They can call Boise rehab and detox programs or treatment options in Garden City with both cards.

Molina rehab in Nampa and Greenleaf

Canyon County follows the same rule, and Molina’s 24-hour Nurse Advice Line, (844) 808-1382, is open to its Idaho members there. Marketplace members in Canyon County hold a different Molina card with Marketplace benefits. Both groups can reach Nampa substance use programs or Greenleaf rehab options.

Molina rehab in Idaho Falls and Ammon

Bonneville County is on the Idaho Medicaid Plus list, and Molina is one of the two plans Idaho chose in its first competitive procurement for dual members, after Blue Cross of Idaho’s contract ended in June 2025. Members can contact Idaho Falls addiction treatment or Ammon treatment programs.

Molina rehab in Pocatello and Chubbuck

Bannock County dual members also enroll in Idaho Medicaid Plus, and those who want one card for Medicare and Medicaid can choose Molina’s voluntary Medicare Medicaid Coordinated Plan instead. A 2020 state guide says coordinated plan changes are allowed quarterly. Pocatello rehab centers and Chubbuck addiction programs can confirm which Molina plan they bill.

Molina rehab in Twin Falls and Kimberly

Twin Falls County was the first county where Idaho Medicaid Plus became mandatory, in November 2018, and it remains on the list. Molina members there can call Twin Falls substance use treatment or Kimberly rehab options, and changes to an Idaho Medicaid Plus plan choice are made each year.

Can I use Molina for rehab outside Idaho?

Usually not for routine care. Idaho Medicaid Plus covers Medicaid benefits for Idaho residents in listed counties, and Molina’s Idaho Marketplace plans are HMO plans, which generally pay for network care. Medicare remains national for dual members, so a Medicare-enrolled program in another state can bill Medicare even when Molina does not cover the rest.

How much does rehab cost with Molina in Idaho?

For dual members, usually little or nothing. Idaho Medicaid Plus carries no premium, Molina describes its Idaho Medicaid plan as offered at no cost, and Idaho Medicaid’s copay list excludes behavioral health. Marketplace members pay the deductible and copays of their 2026 plan, which Molina priced after an average rate rise.

Molina’s Idaho lineup may change after 2026. Molina plans to concentrate its Marketplace business in about six states for 2027 without naming them, and it is leaving standalone Medicare Advantage plans for 2027 while keeping dual plans. Members can call 988 in a crisis, and Molina’s Idaho page lists the 24-hour nurse line.
