Outpatient Rehab Centers in Kentucky

Outpatient rehab in Kentucky, as used on this page, means treatment for drug and alcohol use that you attend while living at home. It does not mean physical therapy after an injury or surgery. Programs range from one counseling session a week to several hours of group work on most weekdays, and many also offer medication for opioid or alcohol use disorder.

Listings here are built from government treatment registers, state licensing records, accreditor listings and details the providers send us. Kentucky’s outpatient listings lean rural: 45 percent are in rural areas, 36 percent in urban areas and 19 percent in small towns.

Common questions about Outpatient Rehab

Answered from what is listed here.

Which Kentucky cities have outpatient rehabs?

Mount Vernon, Ashland, Louisville, Elizabethtown and Owensboro, in that order, are where outpatient rehab listings in Kentucky cluster. Lexington, Louisville and Meads are the larger cities nearby, so people living around them often compare programs across town lines before choosing a schedule of counseling and group sessions that fits work or school.

Outpatient Rehab Centers in Louisvillehttps://rehabseekers.com/locations/us/kentucky/louisville-ky/outpatient-rehab-louisville/
Do outpatient rehabs in Kentucky accept Medicaid?

Nearly all of the outpatient rehabs in Kentucky record accepting Kentucky Medicaid. Coverage runs through the managed care plan a member is enrolled in, such as Aetna Better Health of Kentucky, Humana Healthy Horizons in Kentucky or WellCare of Kentucky, so checking the plan with intake staff first is worthwhile.

Rehab Centers in Kentucky That Accept Kentucky Medicaidhttps://rehabseekers.com/locations/us/kentucky/kentucky-medicaid/
Are there outpatient rehabs near a river in Kentucky?

The most-named rivers for outpatient rehabs in Kentucky are the Dix River, the Salt River and Blue River Branch. Nearly all of the outpatient rehabs in Kentucky sit within 25 miles of a river, at a median of about 13.2 miles from each facility.

Outpatient care means returning home after each session, so travel time to the program usually matters more than a nearby river. Distances are measured from each listed program's address, not from where a client lives.

Rehab Centers Near Rivershttps://rehabseekers.com/attractions/rivers/

What does outpatient treatment for substance use look like in Kentucky?

Outpatient treatment in Kentucky means scheduled counseling, group sessions and medication visits at a clinic you travel to, and you go home afterward. Most programs start with an assessment, then set a treatment plan and change the schedule as needs change, with standard outpatient care usually meaning fewer weekly hours than an intensive program.

For opioid use disorder, medication options include buprenorphine, which office-based clinics often prescribe (see Suboxone clinics in Kentucky). Methadone is given only through opioid treatment programs, which the Department for Behavioral Health, Developmental and Intellectual Disabilities oversees as the State Opioid Treatment Authority. Naltrexone is another option, and for alcohol use disorder, providers may offer naltrexone, acamprosate or disulfiram.

Kentucky’s outpatient listings sit close to water: nearly all are within 25 miles of a river, at a median of 13.2 miles, and the Dix River is the one most of them sit near. Blue Grass Airport in Lexington is the most common nearest airport, ahead of Louisville Muhammad Ali International, and the median distance to the nearest airport is 29.6 miles.

Kentucky Medicaid covers telehealth, including audio-only visits, and some programs hold sessions online (see telehealth rehab in Kentucky). Medicaid, which the state expanded in 2014, pays for outpatient care through five managed care plans, and Medicare and private plans also cover this level. Uninsured residents can ask about state-funded and sliding-scale options.

Programs for teens, pregnant women and veterans

Kentucky law lets a minor consent to substance use disorder diagnosis and treatment by a physician without a parent’s consent, under KRS 214.185. The provider may still tell a parent if that helps the minor. For programs that serve young people, see teen rehab in Kentucky. Pregnant and postpartum women can get case management through KY-Moms MATR, which runs in all 14 of the state’s behavioral health regions. Veterans can ask the VA health care systems in Lexington and Louisville about outpatient care. Each listing shows the languages and populations the provider reports serving.

How many hours a week is outpatient care compared with IOP and PHP?

Weekly hours are the main difference between standard outpatient care, intensive outpatient (IOP) and partial hospitalization (PHP). The figures below follow the long-standing ASAM level definitions, which many providers and payers use. Each program sets its own schedule, so ask intake staff how many hours a week you would attend.

LevelASAM levelTypical weekly hours
Standard outpatient1Fewer than 9 for adults, fewer than 6 for adolescents
Intensive outpatient2.19 or more for adults, 6 or more for adolescents
Partial hospitalization2.520 or more

Programs at the more structured levels have their own pages: intensive outpatient programs in Kentucky and partial hospitalization in Kentucky. For a national overview, see outpatient programs.

Does Kentucky Medicaid or private insurance pay for outpatient treatment?

Kentucky Medicaid pays for outpatient substance use treatment, and the state expanded Medicaid eligibility in 2014. The Department for Medicaid Services runs the program through five managed care plans:

  • Aetna Better Health of Kentucky
  • Humana Healthy Horizons in Kentucky
  • Passport Health Plan by Molina Healthcare
  • UnitedHealthcare Community Plan
  • WellCare of Kentucky

Behavioral health care is part of each plan. Anthem stopped serving as a Kentucky Medicaid plan on January 1, 2025, so members who were with Anthem should confirm their current plan before calling a provider. The 2026 Medicaid behavioral health fee schedule pays providers $135.72 per day for intensive outpatient services. That figure is what the provider receives, not a price you pay. For more on coverage, see rehab that accepts Medicaid.

Medicare and private plans also cover outpatient care. Federal parity law requires most private plans to cover substance use treatment on terms no stricter than they use for medical care. If you have no coverage, the Adult Substance Use Treatment and Recovery Services Branch oversees state and federal funds for community providers. Options are listed under state-funded rehab in Kentucky and sliding-scale rehab in Kentucky. Each listing shows the insurers and payment types the provider reports accepting. Coverage can change, so confirm it with the admissions team.

How can I find out whether an outpatient program in Kentucky is licensed?

Nonhospital alcohol and other drug treatment programs in Kentucky need a license from the Office of Inspector General in the Cabinet for Health and Family Services, issued under 908 KAR 1:370. Ask the program for its license and the levels of care that license covers, then contact the Office of Inspector General if you want to look up its status. Accreditation from The Joint Commission or CARF is a separate, voluntary credential that you can also look for. RehabSeekers.com shows what providers and public records report. We do not rate, rank or vouch for any listing.

When is outpatient care not enough?

Outpatient care may not be enough when stopping a substance could cause dangerous withdrawal, or when living at home makes it hard to stay safe. Withdrawal from alcohol or benzodiazepines can lead to seizures. If you drink or use heavily every day, ask for an assessment before you stop. Medically managed withdrawal is listed under detox in Kentucky, and 24-hour care is listed under residential rehab in Kentucky. Many people step down to outpatient care afterward.
