Intensive Outpatient Programs (IOP) in Maryland

An intensive outpatient program, or IOP, lets a person with a substance use disorder live at home while attending structured treatment several days a week. Maryland’s options cluster in the Baltimore metro and the Washington suburbs of Montgomery and Prince George’s counties. Western Maryland and the Eastern Shore have fewer choices.

Listings on RehabSeekers.com draw on government treatment registers, state licensing records, accreditor listings and information the providers send us. Program format, insurance and the groups served appear where a provider has supplied them. Call intake staff to confirm a schedule before you plan around it.

Common questions about IOP

Answered from what is listed here.

Which Maryland cities have IOP programs?

Maryland listings for intensive outpatient treatment cluster in Westminster, Frederick, Hagerstown, Lake Arbor and Towson. Programs in those communities run scheduled group and individual sessions on set days each week while people keep living at home, so travel time matters when comparing options.

Baltimore, Waldorf and Washington are larger cities nearby, and people living in or around them can also look at listings in those areas when a shorter daily trip makes more sense.

Rehab Centers in Westminsterhttps://rehabseekers.com/locations/us/maryland/westminster-md/
Is intensive outpatient treatment covered by Maryland Medicaid?

Maryland Medicaid is recorded as accepted by nearly all of the intensive outpatient programs in Maryland. Specialty behavioral health care, including addiction treatment, is administered by Carelon Behavioral Health through the Maryland Public Behavioral Health System, outside HealthChoice plans such as Priority Partners. Intake staff can confirm coverage.

Rehab Centers in Maryland That Accept Maryland Medicaidhttps://rehabseekers.com/locations/us/maryland/maryland-medicaid/
Are there IOP programs near a river in Maryland?

Rivers are within 25 miles of nearly all of the intensive outpatient programs in Maryland, with a median of about 10.9 miles from each program to the nearest one.

The South Branch Patapsco River, the Middle Patuxent River and the Hawlings River are the most-named rivers near these programs. Because intensive outpatient treatment is attended from home, a nearby river is mostly a neighborhood detail rather than part of daily sessions.

What is an intensive outpatient program for substance use in Maryland?

An intensive outpatient program for substance use disorder in Maryland is a set schedule of group and individual counseling, held several times a week, for people who do not need round-the-clock care. The American Society of Addiction Medicine (ASAM) classifies this level as 2.1 in The ASAM Criteria, its national placement standard.

Sessions usually include counseling, skills for handling cravings and stress, and family work, and some providers also offer medication for opioid or alcohol use disorder. For adults, the standard sets a floor of 9 treatment hours a week, while length depends on assessment and reassessment rather than a calendar, ranging from a few weeks to several months.

Some Maryland intensive outpatient programs treat mental health conditions only. A mental-health-only program is a different service, so ask whether treatment for substance misuse is part of the schedule. People who need care for both should look at providers that list dual diagnosis services.

Maryland Medicaid, also called Medical Assistance, routes substance use disorder treatment through the Public Behavioral Health System, which Carelon Behavioral Health administers separately from the HealthChoice plans. Ask any program whether it is enrolled with that system, and whether it offers evening or virtual sessions, which Maryland Medicaid covers by video and by audio-only phone.

How many hours a week does IOP take, and how long does it last?

Under the ASAM Criteria, IOP means at least 9 hours of treatment a week for adults and at least 6 for adolescents. The same standard places standard outpatient care in Maryland below those hours and partial hospitalization at 20 or more hours a week. Maryland licenses programs under its own regulations, and this page does not state a separate state hour minimum. Ask the program which ASAM level it is licensed and billed for.

Level of careASAM levelWeekly hours (adults)Setting
Outpatient1Fewer than 9Clinic or office visits
Intensive outpatient2.19 or moreClinic, day or evening, home each night
Partial hospitalization2.520 or moreDay program, home each night

Length depends on the assessment, not a calendar. The treatment plan sets a starting estimate, and staff revisit it as goals are met. Some people step down to standard outpatient after a few weeks, and others stay for several months.

Is IOP the right level of care, or is PHP or residential needed?

IOP fits someone who is medically stable, has a safe place to live and can get to sessions, but needs more structure than a weekly appointment. A clinical assessment decides where to start. A person at risk of serious withdrawal from alcohol, benzodiazepines or opioids may need withdrawal management first, through detox in Maryland, before stepping into IOP. When home is unsafe or daily use continues, the assessment may point to a partial hospitalization program in Maryland or residential treatment in Maryland.

Maryland law lets a minor consent to drug and alcohol treatment the same way an adult can. A minor cannot refuse certified intensive outpatient treatment that a parent has consented to (Health-General 20-102). Programs that work with adolescents are grouped under teen rehab in Maryland.

Does Maryland Medicaid or private insurance cover IOP?

Maryland Medicaid, also called Medical Assistance, handles substance use treatment through the Public Behavioral Health System. Carelon Behavioral Health administers that system separately from the HealthChoice managed care plans. Before starting, members should ask an IOP whether it is enrolled with that system. Our Medicaid coverage page has more general notes. Maryland expanded Medicaid in 2014, so many adults with lower incomes may qualify.

Maryland residents without coverage can look at state-funded rehab in Maryland and sliding-scale programs. The Behavioral Health Administration oversees federal block grant money for substance use services. Medicare and private plans cover some outpatient addiction care, but benefits vary by plan. Older adults can start with Medicare rehab in Maryland and ask the admissions team to run their benefits. Veterans can also use VA care, which has medical centers in Baltimore and Perry Point, or browse veterans rehab in Maryland.

Can I attend IOP in the evenings or online in Maryland?

Maryland IOP listings include day, evening and virtual formats where providers have reported them. Maryland Medicaid continues to cover telehealth by video and allows audio-only visits under a June 2025 provider transmittal. Virtual programs are collected under telehealth rehab in Maryland. Each listing also shows accepted insurers, languages and the groups served, such as women or veterans. Programs that offer counseling in Spanish appear under Spanish-speaking rehab in Maryland.

Most Maryland IOP listings are in urban areas (58 percent) or towns (34 percent). Westminster, Frederick, Hagerstown and Towson come up most often. Ninety-six percent sit within 25 miles of a river, most often the South Branch Patapsco River. For about six in ten, Baltimore/Washington International Thurgood Marshall Airport is the closest airport. The median distance to the nearest airport is 11.8 miles.

Checking a license and getting help now

Maryland’s Behavioral Health Administration, part of the Maryland Department of Health, licenses community behavioral health programs under COMAR 10.63. Ask a program for its license and look it up with the administration before enrolling. Court-ordered DUI Education Programs, which run at least six weekly two-hour sessions under COMAR 10.63.05, are a separate service from IOP. Maryland has no separate state addiction helpline. The former 211 option now routes to 988, which you can call or text at any hour. Other service types are listed on the Maryland treatment page.
