Treatment Centers in Minnesota That Accept Humana

Humana’s coverage of drug and alcohol rehab in Minnesota comes through Medicare: Medicare Supplement policies insured by Humana Insurance Company, stand-alone Medicare drug plans and, if Humana sells one in the member’s ZIP code, Medicare Advantage. Humana has been leaving employer medical coverage since 2023, has no Minnesota Medical Assistance plan, and does not administer TRICARE in Minnesota.

Each listing shows the program’s phone number, its treatment types (withdrawal management, a residential unit, outpatient counseling or medication) and whether it is CARF- or Joint Commission-accredited. The information comes from government registers and Minnesota licensing records. Choose a town or service, then call and ask if the program bills Medicare and Humana.

Does Humana Cover Rehab in Minnesota?

Yes, for Minnesotans with Humana Medicare coverage. Every Medicare plan Humana sells must pay at least what Original Medicare pays for substance use care, meaning Part A hospital treatment and Part B therapy, day programs and opioid treatment. Look for rehabs that accept Humana among the rehab programs in Minnesota on this page.

Humana’s Minnesota business has narrowed to Medicare products, plus dental and vision policies. In February 2023 Humana said it was exiting employer group medical plans, including self-funded and federal employee plans. It kept Medicare Advantage, Medicare Supplement and drug plans, and its 10-K says it offered at least one Medicare plan type in all 50 states.

Does Humana Cover Inpatient Rehab in Minnesota?

Yes, when medically necessary. Medicare Part A, the base for every Humana Medicare plan, pays for stays in psychiatric or general hospitals, so Medicare inpatient rehab programs are covered under it. A 2026 Texas Humana Gold Plus HMO document shows member costs for psychiatric inpatient days, day treatment, intensive outpatient and outpatient substance use visits.

How long will Humana pay for residential rehab?

Humana follows Medicare’s limits, and Medicare’s only stated cap is 190 lifetime days in a freestanding psychiatric hospital. Within that limit, each stay lasts as long as it remains medically necessary for the member. Minnesota residential programs are licensed under chapter 245G and certify their ASAM levels with the Department of Human Services.

Does Humana need prior authorization for inpatient rehab?

Humana’s rule depends on the product. A Humana Medicare Advantage plan’s own documents set any authorization rule for inpatient, residential or partial hospitalization care, and the program requests it through the number on the member card. On a Humana Medicare Supplement policy, Medicare decides what is covered first, and the policy pays its share of the Medicare-covered stay afterward.

Does Humana Cover Detox in Minnesota?

Yes, as part of a covered inpatient stay. Humana counts medically necessary inpatient withdrawal care under the Medicare inpatient benefit, so Medicare detox programs in hospitals qualify. Itasca Medical Care’s provider manual notes that Medicare has no withdrawal management benefit of the kind Minnesota licenses under chapter 245F, so dual members rely on Medical Assistance for it.

Does Humana Cover Outpatient Rehab in Minnesota?

Yes. Part B pays for therapy, counseling, psychiatric evaluations, medication management, family sessions and partial hospitalization, and Humana’s Medicare materials name no session cap for outpatient therapy. Medicare intensive outpatient programs appear on the 2026 Texas Gold Plus document with their own copay, alongside partial hospitalization and outpatient substance use visits.

Humana Medicare plans also pay for phone and video counseling, including Medicare telehealth programs, and Humana says some Medicare Advantage plans charge nothing for mental health telehealth while others charge a copay. On Humana’s USAA Honor Giveback PPO, video or phone visits for mental health and substance use count only in network, so a Minnesota member should ask before booking.

Does Humana Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Opioid treatment programs are paid under Part B for methadone, buprenorphine or naltrexone with counseling, and Part D drug plans can cover the prescriptions themselves. Humana markets three stand-alone Medicare drug plans, Value Rx, Premier Rx and Basic Rx, by ZIP code, which pair with Medicare medication treatment programs.

Does Humana cover methadone?

Yes, through Medicare-enrolled opioid treatment programs in Minnesota and elsewhere. The 2026 Texas Humana Gold Plus document lists an opioid treatment program copay, and Original Medicare charges nothing for them after the Part B deductible. A Minnesota member on a Humana Supplement can use methadone maintenance clinics that bill Medicare.

Does Humana cover Suboxone?

Yes, on Humana Medicare drug lists and through opioid treatment programs, which dispense buprenorphine under Part B. A Humana Part D plan sets the tier and any quantity limit, so the member should search the plan’s drug list before a Medicare Suboxone provider writes the first prescription in Minnesota.

Humana Rehab Centers Near Minnesota Lakes, River Bluffs and the Superior National Forest

Humana Medicare Supplement coverage follows Original Medicare, which pays any Medicare-enrolled program in any county, so the setting a member chooses rarely changes what Humana pays. A Humana Medicare Advantage plan is different: its network and copays are tied to the member’s county, and an HMO pays only network programs outside an emergency.

Humana rehab near Alexandria’s lakes

Humana members in Douglas County live among the Alexandria chain of lakes, including Lake Carlos, Lake Miltona and Lake Le Homme Dieu. Rehabs near lakes and treatment programs in Alexandria that bill Medicare also take a Humana Supplement policy as the second payer after Medicare, and a Medicare Advantage member pays the plan’s network copay instead.

Humana rehab among the Mississippi River bluffs

Humana members in Goodhue, Wabasha and Winona counties live along the Mississippi bluffs and Lake Pepin, from Red Wing to Winona. Programs near rivers and Red Wing treatment centers compare with programs across the river in Wisconsin. There a Medicare-enrolled program also bills Original Medicare first, and on a Humana PPO the member may pay a higher out-of-network share.

Humana rehab near the Superior National Forest

Humana members in Lake and Cook counties live along the Superior National Forest, from Two Harbors to Grand Marais. Rehab centers near national forests along the North Shore and programs in Duluth both bill Original Medicare first, and a Supplement policy follows Medicare to any enrolled program, so the drive between them does not change Humana’s share.

How Do I Find a Rehab That Accepts Humana in Minnesota?

Browse the listings on this page. Every Minnesota entry shows its own phone line, a list of services and any accreditation, and filters sort the entries by city and level of care. Programs under Medicare rehab programs accept Medicare, which most Humana policies in Minnesota build on, so ask about both Medicare and Humana when you call.

Humana admission works differently for each product. For a Medicare Advantage member, the program asks the plan for any required authorization by calling the number on the card, 800-457-4708 for Humana Medicare. For a Supplement member, the program bills Original Medicare and then Humana. The table shows each Humana line in Minnesota and its status for 2026.

Humana rehab in Minneapolis and Saint Paul

Humana Medicare members in the Twin Cities who also qualify for Medical Assistance face a Minnesota-specific choice: the state’s dual program, Minnesota Senior Health Options, is offered by Medical Assistance health plans, and Humana is not one of them. Humana Supplement members can use Minneapolis rehab centers or treatment programs in Saint Paul that bill Medicare.

Humana rehab in Rochester

Humana Medicare Supplement members in Olmsted County hold a policy insured by Humana Insurance Company, which, Humana notes, may carry a different name from the standard lettered Plans A through N. The coverage still pays after Medicare at downtown Rochester treatment programs that accept Medicare.

Humana rehab in Duluth

Humana members in Duluth and the Arrowhead with military coverage should note that Humana Military runs TRICARE only in the East Region, while Minnesota belongs to the West Region, which TriWest Healthcare Alliance runs. Military families and retirees call TriWest at 888-874-9378 about benefits before choosing among addiction treatment programs in Duluth.

Humana rehab in Saint Cloud

Humana members in Stearns County who held a Humana employer plan have moved to other coverage, since Humana’s phase-out of fully insured, self-funded and federal employee medical plans began in 2023. Retirees with Humana Medicare, including group Medicare plans Humana kept, can still use downtown Saint Cloud recovery programs that bill Medicare.

Can I use Humana for rehab in a different state?

Yes, on a Humana Medicare Supplement, which follows Original Medicare to any enrolled provider in the country. Humana Medicare Advantage depends on plan type: HumanaChoice PPO members may see non-network providers willing to treat them, usually paying more, while Humana Gold Plus HMO plans generally need network providers outside an emergency.

How much does rehab cost with Humana?

Humana costs vary by plan and county. Its Medicare Advantage plans usually charge a set copay per visit, a daily copay at the start of a hospital stay, and a cap on yearly in-network spending. Under Original Medicare most outpatient mental health care costs 20 percent once the Part B deductible is met, which a Supplement may pay.

Humana said some of its Medicare Advantage plans will not be renewed for 2027, with notices going out by early October 2026. Coverage in those plans stops when 2026 ends, and Humana has not named the counties.
