Opioid, Fentanyl and Heroin Rehab Centers in Minnesota

Opioid treatment in Minnesota runs through two main routes: opioid treatment programs that dispense methadone, and clinics or prescribers who offer buprenorphine or naltrexone in an office or by telehealth. The listings on this page include withdrawal management, residential care, intensive outpatient and outpatient programs that report treating opioid, fentanyl and heroin use.

Minneapolis, Duluth and Rochester appear most often as program locations. The opioid programs listed split about 42 percent urban, 34 percent town and 24 percent rural. The sections below explain how medication treatment starts, what Medical Assistance pays for, and where to get naloxone.

Common questions about Opioid Rehab

Answered from what is listed here.

Which Minnesota cities have opioid rehabs?

Minnesota opioid rehab listings cluster in Minneapolis first, then Duluth, Rochester, Saint Paul and Grand Rapids. Minneapolis, Duluth and Rochester are also the big cities closest to the listings, which makes those three practical starting points for anyone comparing programs across the state or planning travel for treatment.

Rehab Centers in Minneapolishttps://rehabseekers.com/locations/us/minnesota/minneapolis/
Do opioid rehabs in Minnesota take Minnesota Medical Assistance?

Nearly all of the opioid rehabs in Minnesota record accepting Minnesota Medical Assistance, the state's Medicaid program. Coverage runs through a health plan such as Blue Plus, HealthPartners, Hennepin Health, Medica, PrimeWest Health or UCare. The plan name on a member's card matters.

UCare's MinnesotaCare and Minnesota Senior Care Plus coverage continues in 2026 under the UCare name while Medica acquires UCare contracts. Members can ask admissions staff how that change affects their coverage.

Rehab Centers in Minnesota That Accept Minnesota Medical Assistancehttps://rehabseekers.com/locations/us/minnesota/minnesota-medical-assistance/
Are there opioid rehabs near a river in Minnesota?

Within 25 miles of a river sit nearly all of the opioid rehabs in Minnesota, with a median of about 6.9 miles from listed facilities to the nearest one. The most-named rivers are the Minnesota River, the Credit River and the Rum River. Which river is closest depends on a program's location.

Rehab Centers Near Rivershttps://rehabseekers.com/attractions/rivers/

How do I start methadone or buprenorphine treatment in Minnesota?

Methadone treatment in Minnesota starts with an intake at an opioid treatment program, because federal rules under 42 CFR Part 8 allow methadone for opioid use disorder only through a certified program. The Minnesota Department of Human Services licenses these programs under Minn. Stat. chapter 245G. Programs are listed under methadone clinics in Minnesota.

Buprenorphine can be started in more settings, including opioid treatment programs. Since the federal X-waiver ended in 2023, any clinician whose DEA registration covers Schedule III medications can prescribe it in an office, clinic or emergency department. Minnesota programs that report office-based prescribing appear under Suboxone clinics.

Starting buprenorphine by telehealth follows federal prescribing rules. No separate Minnesota rule on beginning it by video was found. Minnesota Health Care Programs pay for substance use disorder services delivered by live two-way video. Audio-only visits are covered until July 1, 2027 for scheduled appointments. Programs offering remote care are listed under telehealth rehab in Minnesota.

MedicationWhere it is providedWhen it starts
MethadoneCertified opioid treatment program onlyAt intake, after a medical assessment
BuprenorphineOpioid treatment programs, office-based prescribers, some telehealthUsually once early withdrawal has begun, timed by the prescriber
Extended-release naltrexonePrescriber’s office or clinic, as a monthly injectionOnly after withdrawal is complete, typically 7 to 10 days without opioids

What kinds of opioid treatment programs are available in Minnesota?

Minnesota listings for opioid use disorder cover several kinds of care:

Withdrawal management by itself does not treat opioid use disorder.

Fentanyl in the street drug supply leads many people to choose medically managed withdrawal or a supervised medication start instead of stopping alone. Fentanyl can change how withdrawal unfolds, so buprenorphine starts are often planned with a clinician.

Do residential programs accept people taking methadone or buprenorphine?

Residential programs in Minnesota differ on medication, so ask before arranging a stay. Ask the admissions team three things:

  • whether the program coordinates methadone dosing with an outside opioid treatment program
  • whether buprenorphine can be continued or started on site
  • what medication plan is in place at discharge

Nearly every listed opioid program, 99 percent, sits within 25 miles of a river, most often the Minnesota River. Minneapolis-Saint Paul International is the nearest major airport for 52 percent of programs. The median distance to the nearest airport is 13.6 miles.

Does Minnesota Medicaid pay for opioid treatment medications?

Medical Assistance, Minnesota’s Medicaid program, covers substance use disorder treatment. Enrollees should ask their health plan how it covers methadone, buprenorphine and naltrexone. Health plans include:

  • Blue Plus
  • HealthPartners
  • Hennepin Health
  • IMCare
  • Medica
  • PrimeWest Health
  • South Country Health Alliance
  • UCare

More on coverage is on the Medicaid rehab page.

Medicare Part B covers services at opioid treatment programs enrolled in Medicare, including medication and counseling. See Medicare rehab in Minnesota.

The Behavioral Health Administration at the Department of Human Services administers federal block grant money for substance use disorder care, and the state’s Opiate Epidemic Response Fund supports additional grants. Options are gathered under state-funded rehab. Listings also show private insurance, sliding scale and self-pay where programs report them.

Pregnancy and opioid treatment

Pregnant people receive admission preference at programs funded by the federal block grant, under 45 CFR 96.131. Pregnant people who inject drugs come first, then other pregnant people with a substance use disorder. If no program can admit them, the state must arrange interim services, including a prenatal care referral, within 48 hours. See rehab for pregnant women in Minnesota.

Where can I get naloxone in Minnesota?

Naloxone is available at Minnesota pharmacies. Pharmacists can prescribe and dispense it under a Board of Pharmacy protocol, and nasal spray versions are sold over the counter. Community organizations can order free kits through the Department of Human Services State Naloxone Portal. Under Minnesota law, anyone may carry naloxone and give it in good faith.

Overdose risk is higher after any stretch of not using, such as after withdrawal management, a residential stay, jail or a hospital admission. Tolerance drops quickly, while the drug supply may be stronger than before.

In an overdose, call 911. Minnesota’s Good Samaritan law, Minn. Stat. 604A.05, often called Steve’s Law, protects two people from charges for possession, sharing or use of a controlled substance: the person who seeks medical help in good faith, and the person overdosing. The protection applies when the caller gives their name and contact information, stays with the person and cooperates with responders.

How should I read the Minnesota opioid listings?

Each Minnesota listing shows what the program reports about medications, levels of care, payment, accreditation, languages and populations served. Details come from government treatment registers, state licensing records, accreditor listings and information the providers themselves supply.

Under Minn. Stat. 144.343, a minor of any age may consent to alcohol and drug services. For current openings, Fast-Tracker is a free online statewide search of mental health and substance use treatment availability. Other treatment types are on the Minnesota treatment overview, and options in other states are on the national opioid treatment page.
