Treatment Centers in Minnesota That Accept UCare

UCare still covers drug and alcohol rehab in 2026 for members who kept a UCare Medical Assistance, MinnesotaCare, Minnesota Senior Care Plus, UCare Connect or Individual and Family plan. Medica took over those plans on January 1, 2026, they take no new members, and UCare individual plans will not be sold on MNsure for 2027.

Program entries here include a phone number, the treatment offered, which might be withdrawal management, a residential stay, group counseling or medication, along with accreditation status from CARF or The Joint Commission. Federal and state registers and Minnesota licensing are the sources. Search by town, then ask the program if it still bills UCare this year.

Does UCare Cover Rehab in Minnesota?

Yes, for current UCare members through December 2026. UCare’s MinnesotaCare handbook lists substance use screening and assessment, outpatient treatment, inpatient hospital care, residential non-hospital treatment, outpatient medication-assisted treatment, withdrawal management, treatment coordination and peer recovery support as covered. Programs that bill UCare appear among the Minnesota drug and alcohol programs listed here.

UCare was a Minnesota nonprofit founded in 1984 by the University of Minnesota. After heavy losses it agreed in November 2025 to pass its 2026 Medicaid and individual plans to Medica, and a Ramsey County court placed UCare Minnesota into rehabilitation on December 17, 2025. The 2026 plans are issued by UCare Community Health Plan, whose contracts Medica assumed.

Does UCare Connect still cover rehab?

Yes, for adults with disabilities who kept UCare Connect, the non-integrated Special Needs BasicCare plan, which continues for 2026. The integrated version, UCare Connect + Medicare, ended December 31, 2025; the state moved those members to the non-integrated plan from January 1, 2026 and gave dual-eligible members a special period to choose Medicare coverage.

Does UCare Cover Inpatient Rehab in Minnesota?

Yes. UCare’s handbook covers inpatient hospital substance use treatment and residential treatment programs outside a hospital, and room and board that a substance use assessment finds necessary is paid by the Minnesota Department of Human Services rather than UCare. Inpatient rehab programs bill UCare for the treatment portion.

How long will UCare pay for residential rehab?

UCare sets no published day count for Medical Assistance members; the stay follows the ASAM level from the member’s comprehensive assessment. UCare’s provider page lists concurrent oversight for residential substance use treatment under Minnesota’s Section 1115 waiver, which lets the state pay for residential care in larger facilities, so longer stays go back to UCare.

Does UCare need prior authorization for inpatient rehab?

UCare asks for authorization or notification for inpatient and residential substance use care, according to its 2025 provider grid for UCare-origin plans now run by Medica. The MH and SUD Intake Team takes those requests at 1-855-260-9710. For Medical Assistance members, placement itself starts with a comprehensive assessment under Direct Access, without county approval.

Does UCare Cover Detox in Minnesota?

Yes. UCare’s handbook lists withdrawal management as covered, and covers inpatient detoxification only when a hospital stay is medically necessary. Residential withdrawal management programs licensed by the state under chapter 245F are the usual route, at ASAM levels 3.2 and 3.7.

Does UCare Cover Outpatient Rehab in Minnesota?

Yes. UCare covers outpatient substance use treatment, treatment coordination and peer recovery and aftercare support, and its older MinnesotaCare handbook marked no substance use service as needing authorization. UCare adopted the Department of Human Services’ revised outpatient billing codes and rates for services on or after August 1, 2026, for every UCare plan.

UCare members can reach mental health and substance use support, including appointment and scheduling help, through the UCare Mental Health and Substance Use Disorder Triage Line at 1-833-276-1185, which can also give service approvals. UCare also requires no referral to see specialists in its network, so a member can go straight to a network counselor.

Does UCare Cover Suboxone, Methadone and Other Addiction Medications?

Yes. UCare’s handbook lists outpatient medication-assisted treatment as a covered substance use service, and Minnesota requires every provider in its Section 1115 substance use demonstration to offer or arrange medication-assisted treatment. Medical Assistance members had no prescription copays in 2025 under state rules.

Does UCare cover Suboxone?

Yes, through outpatient medication-assisted treatment and the pharmacy benefit. UCare Medical Assistance plans follow the state’s drug rules, and the Minnesota Health Care Programs manual lists medications for opioid use disorder as covered. For remaining UCare members, care at Suboxone clinics accepting Medicaid runs on UCare’s 2026 plans until they end.

Does UCare cover methadone?

Yes, through licensed Opioid Treatment Programs, which the Minnesota Health Care Programs manual lists as a covered Medical Assistance service. UCare Medical Assistance members can use methadone treatment programs in the plan’s network, and the state manual UCare’s Medicaid plans follow excludes guest dosing.

UCare Rehab Centers Near Brainerd’s Lakes, the Upper Mississippi and the Superior National Forest

UCare’s map shrank before its 2026 handoff. On September 1, 2025 it left Medical Assistance and MinnesotaCare in eleven counties, several in lake and forest country, and members there had to choose another plan. In its remaining counties, UCare keeps 2026 members but takes no new enrollments or re-enrollments, and the state does not assign new members to it.

UCare and rehab in the Brainerd lakes

Crow Wing County, home to Brainerd, Baxter and dozens of lakes, was one of the eleven counties UCare left on September 1, 2025. Medical Assistance members there now hold another plan’s card, and rehab centers near lakes around Baxter addiction programs deal with that plan for authorization, not UCare.

UCare and rehab along the upper Mississippi

Sherburne, Wright, Benton and Stearns counties line the Mississippi from Saint Cloud down to Elk River and Monticello, and UCare left all four for Medical Assistance in September 2025. Programs near rivers and Elk River treatment centers are listed, but care there now runs through whichever plan those members chose instead.

UCare and rehab near the Superior National Forest

St. Louis County, which holds much of the Superior National Forest north of Duluth, also lost UCare for Medical Assistance and MinnesotaCare on September 1, 2025. For care at rehabs near national forests there, the member’s new plan decides authorization, while the Department of Human Services helps with plan questions at 1-800-657-3672.

How Do I Find a Rehab That Accepts UCare in Minnesota?

Browse the listings on this page. Minnesota programs here come with a contact number, care levels and accreditation, and the list sorts by town or service. Programs under Medicaid rehab coverage include many that bill Minnesota Medical Assistance plans; ask whether the program bills UCare for 2026 before admission, and whether it is in UCare’s network.

UCare admission for a Medical Assistance member starts with a comprehensive assessment at the chosen program, which then contacts UCare for any authorization or notification. Members call the number on their UCare card; the plan lines are below. The table shows what each UCare product looks like in 2026.

UCare rehab in Minneapolis and Hennepin County

Hennepin County was not on the list of eleven counties UCare left in September 2025, so UCare members there who kept their 2026 plan can still use it for treatment, with Medica running it in the background. Those members can choose Minneapolis rehab centers or Phillips neighborhood programs in UCare’s network.

UCare and rehab in Saint Paul

Ramsey County was one of the eleven counties UCare left for Medical Assistance and MinnesotaCare on September 1, 2025, after the Department of Human Services mailed notices in early July. Ramsey members could choose plans including HealthPartners and Medica, and they now use their new plan at Saint Paul treatment programs.

UCare and rehab in Saint Cloud

Stearns and Benton counties, which share the Saint Cloud area, both lost UCare for Medical Assistance and MinnesotaCare on September 1, 2025. Medica had added Medical Assistance in Stearns that January and in Benton the same September day, so Medica was among the plans open to those members at Saint Cloud rehab programs.

UCare and rehab in Duluth and the northwest

St. Louis County, around Duluth, along with Pennington and Roseau counties in the northwest, left UCare’s Medical Assistance map in September 2025. Medical Assistance members using Duluth addiction programs or Thief River Falls treatment now have a different plan, and UCare’s remaining members statewide keep coverage only through December 2026.

Can I use UCare for rehab in a different county?

Yes, within Minnesota, if the program is in UCare’s network. Direct Access lets a Medical Assistance member choose any enrolled provider for the comprehensive assessment and treatment. Out-of-network care generally needs plan authorization, and UCare’s network for 2026 is the same network its members had before Medica took over.

How much does rehab cost with UCare?

Usually nothing for UCare Medical Assistance members, who had no copays and no deductible in 2025. MinnesotaCare members pay a monthly premium and, under UCare’s 2023 handbook, copays for some services such as non-preventive visits, and the Department of Human Services, not UCare, pays residential room and board.

UCare is closing down for good: UCare Minnesota plans to finish winding down by the end of 2026, and its support for the Medicaid contracts Medica acquired ended on October 1, 2026. UCare members should watch for notices about their 2027 coverage and can call Department of Human Services Health Care Consumer Support at 651-431-3722 with questions.
