Treatment Centers in Nebraska That Accept Nebraska Medicaid (Heritage Health)

Nebraska Medicaid, run by the Nebraska Department of Health and Human Services, pays for drug and alcohol rehab through Heritage Health, its managed care program. Three statewide plans, Nebraska Total Care, Molina Healthcare of Nebraska and UnitedHealthcare Community Plan, cover outpatient care, withdrawal management, residential treatment and medications for addiction treatment.

Each listing on this page shows the program’s phone number, the levels of care it offers and any accreditation from The Joint Commission or CARF. Listings come from government treatment registers and Nebraska licensing records. Browse the programs below, or filter by city, level of care or medication to narrow them down.

Why Trust RehabSeekers.com?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Does Nebraska Medicaid (Heritage Health) Cover Rehab in Nebraska?

Yes. Nebraska Medicaid covers drug and alcohol rehab through Heritage Health, which since January 1, 2017 has combined physical health, behavioral health and pharmacy in one managed care plan per member. Substance use treatment is therefore part of the same plan card, from rehab programs in Omaha to the Panhandle, with no separate state behavioral health carve-out.

Heritage Health runs statewide through Nebraska Total Care, UnitedHealthcare Community Plan of Nebraska and Molina Healthcare of Nebraska, chosen in September 2022 for contracts starting January 1, 2024, when dental also joined. School-based and long-term care services stay in fee-for-service, and Nebraska is one of the states running the Medicaid program under expansion since October 1, 2020.

Does Nebraska Medicaid Cover Inpatient Rehab in Nebraska?

Yes. Nebraska Medicaid covers residential rehab programs and inpatient addiction treatment through the Heritage Health plans. Nebraska’s Section 1115 substance use demonstration lets Medicaid pay for short-term stays for adults aged 21 to 64 in treatment facilities of more than 16 beds, which federal rules otherwise exclude as institutions for mental diseases.

How long will Nebraska Medicaid pay for residential rehab?

Nebraska Medicaid pays for as long as the plan finds the stay medically necessary under its review. The Department’s December 2023 program summary describes the covered stays as short-term and does not set a fixed number of days. Under the waiver, each Heritage Health plan must run utilization management and follow contract rules on ASAM-based placement criteria.

Does Nebraska Medicaid need prior authorization for inpatient rehab?

Yes, for the higher levels of care. Nebraska Total Care requires authorization for inpatient, residential and partial hospital stays, and Molina’s 2026 guide lists residential treatment, partial hospitalization, day treatment and inpatient chemical dependency care, with intensive outpatient needing review after 16 units. The treating provider files the request, not the member.

Does Nebraska Medicaid Cover Detox in Nebraska?

Yes. Nebraska Medicaid covers drug and alcohol detox centers at several intensities. Nebraska Total Care’s benefit list names non-hospital residential detoxification, and its provider manual adds ambulatory and social detoxification. The 1115 demonstration added medically monitored inpatient withdrawal management, ASAM level 3.7-WM, for members who need round-the-clock nursing during withdrawal.

Does Nebraska Medicaid Cover Outpatient Rehab in Nebraska?

Yes. Nebraska Medicaid covers outpatient substance use counseling, substance use assessment, intensive outpatient rehab and partial hospitalization through every Heritage Health plan. Nebraska Total Care members need no doctor’s referral to see an in-network behavioral health provider, and its handbook lists telehealth among covered ways to receive care.

Peer support for adults 21 and older appears in Nebraska Total Care’s 2026 provider manual, and Community Support Services have required prior authorization since October 9, 2020. Halfway house care is a listed rehabilitation benefit, which gives members stepping down from residential treatment a supervised setting before returning home.

Does Nebraska Medicaid Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Nebraska Medicaid’s state plan amendment NE 25-0007, in force from October 1, 2025 and authorized by CMS on February 26, 2026, covers naltrexone, buprenorphine and methadone in every form the FDA has authorized for opioid use disorder, with counseling. The Nebraska Medicaid Preferred Drug List, clinical criteria and quantity limits still apply to each product.

Does Nebraska Medicaid cover methadone?

Yes. Nebraska Medicaid covers methadone clinic treatment through Opioid Treatment Programs, which must hold federal certification under 42 CFR Part 8. A 2021 provider bulletin said the state expected the Heritage Health plans to pay most of these services, so a member’s own plan is the one that pays the clinic.

Does Nebraska Medicaid cover Suboxone?

Yes. Nebraska Medicaid covers Suboxone and buprenorphine treatment as part of the medication benefit, and the state plan pairs it with individual, group and family therapy. A non-preferred product on the state drug list needs prior authorization, so prescribers usually start with the preferred buprenorphine form.

Does Nebraska Medicaid cover naltrexone and alcohol use medications?

Yes. Nebraska Medicaid covers naltrexone and Vivitrol treatment for opioid use disorder under the same state plan section. Medications for alcohol use disorder follow the state Preferred Drug List, and Nebraska Total Care adds a plan formulary alongside it, so the plan’s drug look-up shows each product’s status.

Nebraska Medicaid Rehab Centers by the Rivers, Lakes and National Forests

Nebraska Medicaid pays the same way along the Missouri River, beside the Sandhills lakes and in the Pine Ridge, because all three Heritage Health plans serve every county. The setting changes the drive, not the benefit. Listings here note each program’s distance from the nearest river, lake or forest.

Nebraska Medicaid rehab near the Missouri and Platte rivers

Nebraska’s two great rivers frame most of its cities. Rehab centers near rivers run from Omaha and South Sioux City on the Missouri to Columbus, Grand Island and Kearney along the Platte, and treatment centers in Nebraska City sit on the Missouri in Otoe County. Any of the three Heritage Health plans covers them.

Nebraska Medicaid rehab near Lake McConaughy and the western lakes

Lake McConaughy, the state’s largest reservoir, sits north of Ogallala in Keith County, in the Southwest behavioral health region. Rehab centers near lakes there and treatment programs in Ogallala take the same Heritage Health card as an Omaha clinic; a residential stay still needs the plan’s authorization before admission.

Nebraska Medicaid rehab near the Pine Ridge and national forests

Northwest Nebraska’s Pine Ridge, around Chadron in Dawes County, holds part of the Nebraska National Forest, and the Bessey unit lies near Halsey in the central Sandhills. Programs near national forests fall in the Panhandle region, where rehab programs in Alliance are among the nearer options.

How Do I Find a Rehab That Accepts Nebraska Medicaid in Nebraska?

Browse the listings on this page. Every program shown accepts Nebraska Medicaid, and filters narrow them by city, level of care and medication. Each listing gives the program’s own phone number, so a call reaches the admissions team directly, and statewide Medicaid rehab listings sit on a separate page.

Admission for residential, inpatient and partial hospital care runs through the member’s Heritage Health plan, because the plan authorizes those levels. The program usually files the request; the plan’s member services line can explain benefits or a pending decision. Eligibility questions go to iServe Nebraska at 1-855-632-7633, not to the plans.

Nebraska Medicaid rehab in Omaha, Bellevue and Papillion

Omaha sits in the Omaha Metro behavioral health region, and its Heritage Health members choose among the same three plans as the rest of the state. Nebraska Total Care keeps its office in Omaha, and Medicaid members there can compare programs in South Omaha with treatment centers in Papillion before calling a plan.

Nebraska Medicaid rehab in Lincoln and Lancaster County

Lincoln members reach residential and outpatient care under the same Heritage Health rules as Omaha, and the Southeast behavioral health region funds services for people who are not yet on Medicaid. Lincoln drug and alcohol rehabs and rehab programs in Seward appear in the listings below.

Nebraska Medicaid rehab in Grand Island, Kearney and Hastings

Grand Island, Kearney and Hastings are the Central region’s largest cities, strung along Interstate 80 and the Platte. Members who need medically monitored withdrawal management or a residential bed often compare treatment programs in Grand Island with rehab centers in Kearney, and the plan authorizes the stay wherever it is.

Nebraska Medicaid rehab in Norfolk and Columbus

Norfolk anchors the Northern behavioral health region in Madison County, and Columbus serves Platte County to the south. Heritage Health members there keep the same plan if they travel to Omaha or Lincoln for a residential stay, and listings for substance use treatment in Norfolk show which programs are local.

Nebraska Medicaid rehab in North Platte and Scottsbluff

North Platte, in the Southwest region, and Scottsbluff and Gering, in the Panhandle region, are the main western hubs. Nebraska Total Care’s provider manual sets a two-hour standard for emergent behavioral health care in rural and frontier areas, against one hour elsewhere. Listings include rehab programs in North Platte and treatment centers in Scottsbluff.

Can I use Nebraska Medicaid for rehab in a different county?

Yes. Heritage Health plans cover all 93 counties, so a member in Kearney can enter a residential program in Omaha with the same card and the same plan. Out-of-state care is different: Nebraska Total Care requires prior authorization for any out-of-network provider except emergency and urgent care, and unauthorized care may be billed to the member.

How much does rehab cost with Nebraska Medicaid?

Usually little or nothing. Nebraska Total Care’s 2026 provider manual says it is waiving copays for medical, generic pharmacy, and mental health and substance use services, and its handbook lists no copays for covered medications. Adults in the expansion group qualify up to 138 percent of the federal poverty level.

Nebraska Medicaid is not the only way to pay for treatment here. The Division of Behavioral Health funds care through six regions for people without insurance, and new expansion applicants have faced work requirements since May 1, 2026. State funded rehab programs and sliding scale treatment options are listed alongside Medicaid programs below.
