Treatment Centers in Oklahoma That Accept Humana

Humana covers drug and alcohol rehab in Oklahoma mainly through Humana Healthy Horizons in Oklahoma, one of three SoonerSelect plans serving SoonerCare members statewide, and through Humana Medicare plans. The Medicaid plan covers mental health and substance use treatment, rehabilitation services, opioid treatment programs and peer recovery support without a primary care referral.

Oklahoma programs on this page are drawn from public treatment registers and the state’s licensing files. Each listing gives the program’s direct number, the services it provides (from withdrawal care and residential beds to counseling and medication). Accreditation from CARF or The Joint Commission appears where a program holds it. City and care-level filters shorten the list.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Does Humana Cover Rehab in Oklahoma?

Yes. For the SoonerCare members it enrolls, Humana Healthy Horizons in Oklahoma pays for diagnosis, treatment and rehabilitation of substance use and mental health conditions, and Humana Medicare plans cover addiction care under Medicare rules. Programs billing rehabs that accept Humana appear among the rehab centers in Oklahoma shown here, from the two big metros to smaller towns.

Humana Healthy Horizons in Oklahoma is a product of Humana Wisconsin Health Organization Insurance Corporation and has been a SoonerSelect plan, alongside Aetna Better Health and Oklahoma Complete Health, since the program went live on April 1, 2024. No outside vendor manages its behavioral care. Humana’s employer medical plans ended after 2023, so its Oklahoma business is Medicaid and Medicare.

Does Humana Cover Inpatient Rehab in Oklahoma?

Yes. Humana Healthy Horizons in Oklahoma covers inpatient and residential rehab programs as SoonerCare benefits, and SoonerCare’s guide lists residential substance use treatment from age 13. For Oklahomans on Humana Medicare, Part A rules set the inpatient benefit, and the only printed limit is 190 lifetime days in a stand-alone psychiatric hospital.

Does Humana need prior authorization for inpatient rehab?

Yes. Before inpatient or residential care starts, Humana Healthy Horizons in Oklahoma requires prior authorization, as it does for partial hospitalization and opioid treatment programs; inpatient behavioral requests get a decision within 24 hours. A request missing information can take up to 14 more days, and Humana does not cut authorized care partway through without notice.

What if I was already in treatment when I joined Humana?

Humana Healthy Horizons in Oklahoma lets new members already in a treatment plan keep seeing their current behavioral health providers for up to 90 days, even out of network. After that, Humana helps the member move to a network provider, which matters for anyone switching SoonerSelect plans partway through a program.

Does Humana Cover Detox in Oklahoma?

Yes. Humana Healthy Horizons in Oklahoma members get inpatient medical detox programs as part of the SoonerCare benefit, which lists inpatient detoxification with prior authorization. Humana Medicare members get inpatient detox as hospital care under Medicare Part A rules, with the plan’s per-day copays applying to the first days of the stay.

Does Humana Cover Outpatient Rehab in Oklahoma?

Yes. Humana Healthy Horizons in Oklahoma covers outpatient behavioral health agency services, peer recovery support and Program of Assertive Community Treatment, and its 2026 handbook says outpatient behavioral health needs no prior authorization. That makes outpatient rehab services the quickest level of care for Humana Medicaid members to start in Oklahoma.

Humana Healthy Horizons covers partial hospitalization treatment with prior authorization, and audio or video visits with participating providers, including medication management. Humana Medicare plans cover outpatient therapy, family counseling within treatment and counseling by phone or video; some charge a copay for telehealth and some do not.

Does Humana Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Humana Healthy Horizons in Oklahoma’s 2026 handbook lists buprenorphine with naloxone, Vivitrol and methadone among covered medications for addiction treatment, and members use Humana’s Preferred Drug List. The usual monthly limit on prescriptions does not apply to Humana members, though brand-name drugs are limited to two a month unless a third is medically necessary.

Does Humana cover Suboxone?

Yes. Humana Healthy Horizons covers buprenorphine with naloxone, the medicine in Suboxone, and since October 1, 2020, SoonerCare has paid for methadone, naltrexone, buprenorphine and the other FDA-listed treatments in both clinics and offices. Buprenorphine treatment programs can prescribe it in an office setting, and members aged 21 and over may owe a small prescription copay.

Does Humana cover methadone?

Yes, for both Medicaid and Medicare members. Healthy Horizons members can start at methadone clinic programs once Humana authorizes the opioid treatment program in advance. On the Medicare side, Humana plan documents filed in Texas and elsewhere name opioid treatment program services as a benefit, and a Medicare-enrolled Oklahoma clinic bills Humana for the dose and counseling together.

Does Humana cover Vivitrol?

Yes, for Humana Healthy Horizons members, whose 2026 handbook names Vivitrol. Vivitrol injection programs can bill the plan, and the Preferred Drug List shows any limits. Humana Medicare members should look up naltrexone on their own plan’s Part D drug list before treatment begins, since coverage and tier vary from one Humana Medicare plan to another.

Humana Rehab Centers Near Fort Gibson Lake, the Cimarron River and the Wichita Mountains

Humana Healthy Horizons in Oklahoma is statewide, so its network, benefits and authorization rules stay the same whether a member chooses a program on a reservoir, a river town or near the southwestern mountains. Humana Medicare members should look at plan type: HMO plans limit out-of-network care, while PPO plans allow it at higher cost.

Humana rehab near Fort Gibson Lake

Humana members in Wagoner and Cherokee counties, within the Cherokee Nation reservation, live around Fort Gibson Lake, the reservoir on the Grand River east of Tulsa. Rehab programs near lakes and treatment options in Wagoner serve the area, and Healthy Horizons covers rides to appointments through ModivCare at 877-718-4213.

Humana rehab along the Cimarron River

Humana members in Logan County live along the Cimarron River as it bends past Guthrie, north of Oklahoma City. Rehab centers near rivers and Guthrie recovery services are close by; Humana answers a residential request from Guthrie on the same 24-hour timeline as one from Oklahoma City.

Humana rehab near the Wichita Mountains wilderness

Humana members in Comanche County live near the Wichita Mountains Wildlife Refuge and its Charons Garden Wilderness, northwest of Lawton. Programs near wilderness areas serve this corner of southwestern Oklahoma, where Healthy Horizons members can add audio or video visits with participating providers between in-person sessions, including medication management appointments.

How Do I Find a Rehab That Accepts Humana in Oklahoma?

Browse the listings on this page. Humana members see a direct phone line for each program, a summary of its care and any accreditation, and filters by city, care level or insurer trim the list. Programs listed under Medicaid rehab programs include those billing SoonerCare plans; ask admissions whether Humana Healthy Horizons in Oklahoma is one of them.

Prior authorization comes first when a Humana Healthy Horizons member enters residential care, inpatient detox, a day program or a methadone clinic. Usually the program sends Humana the request; a member may instead phone Member Services with the program’s name. Below are the Humana lines Oklahomans may hold in 2026 and what each one covers.

Humana rehab in Oklahoma City

Humana Healthy Horizons members in Oklahoma County can reach Member Services on weekdays from 8 a.m. to 5 p.m. Central, and the nurse advice line at any hour. Humana’s statewide Medicaid network covers Bricktown treatment programs and rehab services near the Paseo, with behavioral care open to members without a referral.

Humana rehab in Tulsa

Humana Healthy Horizons members in Tulsa County, the state’s second-largest county, can search Humana’s online Find Care tool with the Oklahoma Medicaid network preselected, then compare results with East Tulsa rehab centers or Midtown Tulsa treatment options listed here. Members can manage benefits, see authorizations and chat with Humana through the MyHumana portal.

Humana rehab in Lawton and Fort Sill

Humana no longer administers TRICARE for military families at Fort Sill, because Oklahoma moved from the East Region to the West Region on January 1, 2025, where TriWest Healthcare Alliance took over. TRICARE members heading to Lawton addiction treatment now call TriWest at 888-874-9378, weekdays 8 a.m. to 6 p.m.

Humana rehab in Ada and Durant

Humana Healthy Horizons members in Pontotoc and Bryan counties, home to the Chickasaw and Choctaw nations’ headquarters, have one SoonerSelect rule to know. Humana must let American Indian and Alaska Native enrollees see any Indian Health Care Provider, in network or not, with no referral. Members can also use rehab programs in Ada and downtown Durant recovery services.

Can I use Humana for rehab in a different state?

Rarely, for Humana Healthy Horizons members. If Humana’s Oklahoma network has no provider who can give the needed care, Humana arranges care outside the network, but out-of-network referrals need prior approval from the plan. A Humana Medicare PPO pays an out-of-network program willing to treat the member, though the member pays a larger share.

How much does rehab cost with Humana?

Humana Healthy Horizons in Oklahoma’s 2026 handbook lists no copays for behavioral health or medical services, only a small prescription copay for members 21 and over where it applies. On Humana Medicare, expect a set amount per office visit and a daily charge in the first part of a hospital stay, with a yearly out-of-pocket cap.

Peer recovery support is another Humana Healthy Horizons benefit in Oklahoma, useful for follow-up after treatment once a residential stay ends. Oklahoma’s plans point members to 988, the Oklahoma Mental Health Lifeline, by call or text at any hour, and tell them to call 911 in an emergency.
