Intensive Outpatient Programs (IOP) in Vermont

An intensive outpatient program, usually called IOP, gives a person with a substance use disorder structured treatment several days a week while they keep living at home. Vermont’s IOP listings are mostly outside the cities: 71 percent of mapped programs are in rural areas and 29 percent in towns, in places such as Burlington, Rutland, Bennington, Montpelier and Wilder.

RehabSeekers.com builds these listings from government treatment registers, state licensing records, accreditor directories and the information providers give about their own programs. The sections below cover weekly hours, how IOP compares with other levels of care, Vermont Medicaid and other ways to pay, evening and virtual options, and how to check a program’s state certification.

Common questions about IOP

Answered from what is listed here.

Where are IOP programs in Vermont?

Vermont listings for intensive outpatient treatment cluster in Burlington, Wilder, North Springfield, Rutland and Bennington, with Burlington named first among the places across the state where these programs are recorded.

Larger nearby cities also matter when planning a schedule around work or school. Burlington, Lebanon and Chester are the bigger cities near these listings, so a person living close to one of them may find a group schedule within reach.

Does Green Mountain Care cover IOP in Vermont?

Most of the intensive outpatient programs in Vermont record accepting Green Mountain Care, so state Medicaid members can often find a program that takes their coverage. Asking intake staff to confirm before starting is sensible.

Vermont has no private Medicaid managed care companies. The Department of Vermont Health Access, under an agreement with the Agency of Human Services, pays for behavioral health and substance use disorder services.

Are there IOP programs near a river in Vermont?

La Platte River, White River and Middle Branch Williams River are the most-named rivers near intensive outpatient programs in Vermont. Nearly all of the intensive outpatient programs in Vermont sit within 25 miles of a river, at a median of about 3.9 miles from the nearest one, measured from each listed facility.

What is an intensive outpatient program for substance use in Vermont?

An intensive outpatient program for substance use disorder in Vermont is scheduled treatment, mainly group and individual counseling, held on several days each week while you sleep at home. Adult programs at this level involve 9 or more hours of treatment a week.

Vermont treatment programs are certified by a division of the Vermont Department of Health, under the state’s Substance Use Disorder Treatment Certification Rule. Some Vermont IOPs treat mental health conditions only and follow a different model. Before you book an assessment, ask whether the program treats addiction.

If you need care for both, the dual diagnosis listings for Vermont are a better starting point. An assessment, not a preferred schedule, decides the level of care, and intensive outpatient generally fits people who are medically steady, living somewhere safe and able to get to sessions on several days.

Most programs plan a set number of weeks once the opening assessment is done, review that plan along the way and taper hours toward standard outpatient care. Anyone stopping alcohol, benzodiazepines or opioids may need withdrawal management first, and Vermont Medicaid, run by the Department of Vermont Health Access, pays for substance use disorder treatment.

How many hours a week does IOP take, and how long does it last?

Adult IOP means 9 or more hours of treatment a week under the ASAM Criteria, the placement standard published by the American Society of Addiction Medicine, which calls IOP level 2.1. Adolescent programs at the same level start at 6 hours a week. The table compares IOP with the levels on either side of it.

Level of careAdult weekly hoursSetting
Outpatient (ASAM 1)Fewer than 9Office or clinic visits
IOP (ASAM 2.1)9 or moreSessions several days a week, home at night
PHP (ASAM 2.5)20 or moreFull-day program, home at night
Residential (ASAM 3)Around the clockLive on site

IOP has no fixed length. Most programs set a planned number of weeks after the first assessment and review it as treatment goes on, then reduce hours toward standard outpatient care when the care team and the participant agree. Vermont’s certification rule sets the standards certified programs must meet. This page quotes only the ASAM hours, so ask the program or the Division of Substance Use Programs whether Vermont adds a weekly minimum of its own.

Is IOP the right level of care, or is PHP or residential needed?

An assessment decides the level of care, not a preference for a particular schedule. IOP generally suits someone who is medically stable, has a safe place to live and can attend several sessions a week.

Withdrawal can change the starting point. Stopping alcohol, benzodiazepines or opioids can bring on withdrawal, and intake staff will usually recommend withdrawal management first. Vermont options are in the detox listings. People who need daily structure may be placed in partial hospitalization in Vermont or residential treatment. People who need fewer hours may fit standard outpatient care.

Does Vermont Medicaid or private insurance cover IOP?

Vermont Medicaid pays for substance use disorder treatment through the Department of Vermont Health Access (DVHA), but ask each program whether it bills Medicaid for its IOP. Vermont has no private Medicaid plans. Under the Global Commitment to Health waiver, DVHA acts as the only public managed care entity, and substance use services are paid through DVHA. Vermont expanded Medicaid in January 2014 to adults with incomes up to 138 percent of the federal poverty level. Programs that accept Medicaid nationally are listed on the Medicaid page.

State funding, Medicare and private plans

The Vermont Department of Health, through its Division of Substance Use Programs, manages federal block grant money for treatment. Programs certified as preferred providers hold a Department contract or grant. See state-funded rehab in Vermont. Medicare covers some outpatient substance use care, so ask whether a program bills Medicare for IOP; options are on the Medicare listings. Private plans often require prior authorization, so call the number on your card before you start.

Can I attend IOP in the evenings or online in Vermont?

Evening and virtual IOP are offered by some Vermont programs, alongside daytime tracks. The format on each listing comes from the provider, so confirm the current schedule with the admissions team.

Vermont Medicaid pays for clinically appropriate telemedicine, including substance use treatment. It also pays for listed audio-only phone services at the telemedicine rate. Virtual options are in the Vermont telehealth listings.

Using the Vermont listings

Each listing shows accepted insurers, languages and the groups it serves, where the provider has supplied them. Vermont law lets a minor 12 or older consent to substance use treatment without a parent once a licensed physician verifies the condition (18 V.S.A. 4226); see the teen listings. Veterans can also contact the White River Junction VA Medical Center or browse the veterans listings.

Every mapped IOP listing in Vermont is within 25 miles of a river, at a median distance of 3.9 miles, most often the La Platte River. Each is also within 25 miles of a waterfall, at a median of 8.9 miles, most often Winooski Falls. The nearest airport is a median of 5.6 miles away, and Patrick Leahy Burlington International Airport is the one that comes up most often. For other levels of care, return to the Vermont treatment page.
