Opioid, Fentanyl and Heroin Rehab Centers in Vermont

Opioid treatment in Vermont runs largely through the state’s hub-and-spoke system. Hubs are opioid treatment programs that dispense methadone and buprenorphine. Spokes are office-based practices that prescribe buprenorphine closer to home. The listings here cover those options along with withdrawal management, residential care, outpatient counseling and telehealth for opioid, fentanyl and heroin use.

Listed opioid programs in Vermont are mostly rural: four in five are in rural settings and the rest are in towns. Every one sits within 25 miles of a river, a median of 3.2 miles away, most often the La Platte River. Listing details come from government treatment registers, state licensing records, accreditor listings and information the providers supply themselves.

Common questions about Opioid Rehab

Answered from what is listed here.

Where are opioid rehabs in Vermont?

Opioid rehab listings in Vermont cluster in Rutland, Burlington, Bennington, Saint Albans and Hartford, so checking the town a program sits in is a sensible first step. Those towns hold the largest share of opioid treatment listings across the whole state.

Families farther away often weigh Burlington, Rutland or Lebanon, the larger population centers nearby, when planning travel time for regular appointments or visits during treatment.

Rehab Centers in Rutlandhttps://rehabseekers.com/locations/us/vermont/rutland/
Do opioid rehabs in Vermont take Green Mountain Care?

Ask the admissions team whether your Green Mountain Care enrollment is active, because nearly all of the opioid rehabs in Vermont record accepting it. The Department of Vermont Health Access is the only public managed care entity, so behavioral health and substance addiction services are paid through that department rather than a private health plan.

Rehab Centers in Vermont That Accept Green Mountain Care (Vermont Medicaid)https://rehabseekers.com/locations/us/vermont/green-mountain-care-vermont-medicaid/
Are there opioid rehabs near a river in Vermont?

A river lies within 25 miles of nearly all of the opioid rehabs in Vermont, and the median distance to the nearest one is about 3.2 miles from listed facilities. The most-named rivers are the La Platte River, the Cold River and the Little Hoosic River, each measured from individual program locations.

How do I start methadone or buprenorphine treatment in Vermont?

In Vermont, methadone is available only at an opioid treatment program, which means one of the hubs. Buprenorphine can come from a hub or from an office-based spoke prescriber. Federal rules in 42 CFR Part 8 limit methadone for opioid use disorder to certified opioid treatment programs. The Vermont Department of Health adds its own requirements through the Rules Governing Medications for Opioid Use Disorder, in effect since May 2024. The state’s five treatment regions are Northwest, Northeast, Central, Southeast and Southwest. Each region has had at least one hub since the Bennington hub opened in July 2025. Listings for methadone clinics in Vermont and Suboxone clinics in Vermont show both routes.

Primary care can also be a starting point. The federal waiver requirement ended in 2023, so any clinician whose standard DEA registration covers controlled substances can now prescribe buprenorphine. For help finding care, VT Helplink is free, confidential and open around the clock. You can call or text 802-565-LINK (5465) or 833-565-LINK (5465), and staff can point you to a nearby hub or spoke.

Extended-release naltrexone is a third medication. A clinician gives it only after withdrawal is complete, because it blocks opioids rather than easing withdrawal.

Starting by telehealth

Vermont’s 2024 rule allows several steps to happen by telehealth:

  • Prescribing medication for opioid use disorder.
  • The evaluations the rule requires.
  • The office-based intake exam, when this matches federal guidelines.

Vermont Medicaid pays for telemedicine for substance use treatment. It also pays for some audio-only phone visits at the telemedicine rate. See telehealth rehab in Vermont for listings.

If you are pregnant

Vermont’s rule protects quick access for pregnant people. If you are pregnant and ask an office-based provider for medication, that provider must admit you to or refer you to an opioid treatment program within 48 hours of first contact (Section 6.8). Federal block grant rules also give pregnant women preference in admission, with interim services within 48 hours. Programs that report serving pregnant people appear under rehab for pregnant women in Vermont.

What kinds of opioid treatment programs are available in Vermont?

Vermont listings for opioid use disorder include:

  • Opioid treatment programs
  • Withdrawal management
  • Residential care
  • Partial hospitalization
  • Intensive outpatient
  • Standard outpatient
  • Telehealth

Fentanyl is common in the street drug supply, and withdrawal from it can follow a different pattern than withdrawal from heroin. That is a good reason to look for medically managed withdrawal or a supervised medication start. Clinicians may also change how they begin buprenorphine for someone who has been using fentanyl. Withdrawal care is a first step, not a full course of treatment. Listings for detox in Vermont show where it is offered.

MedicationWhere you get itWhen it starts
MethadoneOpioid treatment program (hub)At intake, as the program directs
BuprenorphineHub, spoke practice or telehealth prescriberUsually once withdrawal has begun
Extended-release naltrexonePrescriber or program offering injectionsAfter withdrawal is complete

Rutland, Burlington, Bennington, Saint Albans and Hartford are the towns that appear most often in Vermont opioid listings. Patrick Leahy Burlington International is the nearest airport for more than a quarter of listed programs, at a median distance of 21.8 miles. Each listing shows accreditation, languages, populations served and payment types where the provider reports them.

Does Vermont Medicaid pay for opioid treatment medications?

Vermont Medicaid pays for substance use treatment. The Department of Vermont Health Access (DVHA) administers it and pays for behavioral health and substance use services under the state’s Global Commitment to Health waiver. Vermont expanded Medicaid in January 2014 to adults with incomes up to 138 percent of the poverty level. Before you start, ask the hub or spoke office to confirm how your medication will be billed. More payment details are on the Medicaid rehab page.

Other ways to pay include:

  • Medicare: Part B includes an opioid treatment program benefit. Listings for Medicare rehab in Vermont show programs that accept it.
  • State funding: For people without coverage, the Vermont Department of Health’s Division of Substance Use Programs handles state and federal treatment funds. Preferred providers hold a Department contract or grant. See state-funded rehab in Vermont.

Where can I get naloxone in Vermont?

Anyone in Vermont can get naloxone without a prescription under a statewide standing order. VT Helplink sends it free by mail. You can also pick it up at:

  • Pharmacies
  • Clinics
  • Syringe services programs
  • Treatment programs
  • Recovery centers

Overdose risk is higher after a period of not using. Withdrawal, a residential stay, jail or a hospital stay all lower tolerance, so an amount you used before can be dangerous. Keep naloxone nearby after any of these.

If someone is not responding, call 911. Vermont’s Good Samaritan law, 18 V.S.A. 4254, bars citing, arresting or prosecuting a person who seeks help in good faith for an overdose. The same protection covers the person who is overdosing.

Do residential programs in Vermont accept people taking methadone or buprenorphine?

Whether a residential program accepts people taking methadone or buprenorphine depends on the program. Check the listing and ask the admissions team before you go. Useful questions include:

  • How is daily methadone dosing arranged with a hub?
  • Will the program continue a buprenorphine prescription during the stay?
  • What happens with medication and naloxone at discharge?

Listings for residential rehab in Vermont show which programs report medication support. All Vermont treatment types are on the Vermont rehab page, and opioid options in other states are under opioid rehab nationwide.
