Assertive Community Treatment (ACT) Teams

Assertive community treatment puts a whole team around one person: psychiatrist, nurse, case manager, employment and substance use specialists, all sharing the same caseload. Contact happens at home and in the community, frequently, and continues for as long as it is needed.

ACT exists for people whose needs have not been met by regular outpatient appointments, so entry criteria are strict and capacity is limited. Ask about caseload size, after-hours contact and what a referral requires. Listings show which providers record an ACT team on staff.
