About Into Action Recovery – Laveen

Into Action Recovery is a privately owned for-profit men’s residential program on West Pedro Lane in Laveen Village, in the southwest corner of Phoenix. It admits men aged 18 and over only, with no women and no adolescents, and combines twelve-step work with evidence-based therapy. The operator has been serving men in recovery since 2012 and also runs a sister program in British Columbia, Canada.

The program treats alcohol, opioids including heroin and fentanyl, stimulants such as methamphetamine, cocaine and crack, benzodiazepines and prescription drug use, and takes co-occurring conditions seriously rather than referring them out. Depression, anxiety, bipolar disorder, schizophrenia and psychosis, ADHD, OCD, and PTSD and trauma are all on record as treated alongside the substance use work, which is what dual diagnosis means in practice here.

One admission condition shapes who can come straight in. Withdrawal care is not provided on site, so anyone who needs detox completes it at a hospital or detox center first and arrives afterwards. Chris Burwash is chief executive and founder; Paul Flatley, DO, is medical director in addiction medicine, with Tara Harvell, LPC, as clinical director and David Cummings as program director.

Treatment Services at Into Action Recovery

Residential care runs at both short-term and long-term lengths, with stays of 30 to 90 days and most men staying 30 to 60, long enough for clinical work to move past stabilization. Step-down is available through the operator’s men’s intensive outpatient program in Scottsdale, so someone finishing residential treatment in Laveen does not have to find a new provider for the next stage of care.

Therapy is CBT, DBT and motivational interviewing, delivered in individual and group sessions and layered over twelve-step work. No medication for addiction treatment is named: residents may continue medication prescribed by their own physician, and the medical director and nursing staff help manage anything newly prescribed. That is a different thing from a buprenorphine or naltrexone program, so clarify it on the phone if medication is what you need.

Admissions run on a 24/7 line with a callback option, and same-day admission is possible, with the team checking insurance benefits and arranging intake before arrival. Around the residential stay sit case management, a family program, aftercare and an alumni network. The clinical roster includes a registered nurse and residential technicians, so staff are on site through the night rather than only during program hours.

Into Action Recovery Address, Location and Contact Information

The house is at 5213 West Pedro Lane, Laveen, Arizona 85339, in Maricopa County, on a suburban residential street in southwest Phoenix rather than on a clinical campus. Phoenix proper lies 8 miles north-east and Maricopa 22 miles south, so the home is close enough to the city for appointments and family visits while still sitting in a quiet neighborhood setting.

Inside, men have private bedrooms with desks, and the property has a pool, hot tub, gym and an outdoor lounge with a fire pit. The admissions line, (623) 745-0719, is answered 24 hours a day, and email reaches the program at info@intoactionrecovery.us. The operator’s admissions office and its men’s intensive outpatient program are at separate addresses on East San Alberto in Scottsdale.

One note on numbers. The program’s federal provider record carries a British Columbia phone number from the operator’s Canadian side, so use the Arizona line above rather than anything attached to the provider identifier. If you are travelling in, confirm the bed before you set off, since same-day admission depends on what is free that day rather than being guaranteed to anyone who calls.

How Much Does Treatment at Into Action Recovery Cost?

Into Action Recovery does not publish a nightly or monthly rate for the Laveen home, so cost has to be established on the phone. The program accepts private insurance and self-pay, and payment plans are available. Aetna, Blue Cross Blue Shield, Cigna and Humana are named as carriers; Medicaid is not accepted, and whether Medicare is billed is not stated on the pages the program publishes.

Benefits checks are free, by phone or online, and the admissions team does the legwork: calling the insurer, handling prior authorization and pursuing appeals, and seeking single case agreements where a plan is out of network. That last point is worth pressing if your carrier is not one of the four named, because a single case agreement can be the difference between a covered stay and a private-pay one.

For context, statewide private-pay ranges in Arizona sit at around $6,000 to $25,000 a month for residential treatment, $7,500 to $24,000 for medical detox, $8,000 to $13,500 for partial hospitalization, $3,000 to $10,000 for intensive outpatient and $1,500 to $5,000 for outpatient care. Those are Arizona-wide figures across all providers rather than this program’s prices, and detox would be billed by whichever facility provides it.

Into Action Recovery Reviews

Into Action Recovery Inc holds NPI 1760354443, enumerated in September 2025, with Christopher Burwash listed as owner and authorized official. A state license number for the Pedro Lane address is not published: the program shows an Arizona Department of Health Services badge without a number, and no license appears against this street in the state’s open licensing data, which may be because the record postdates that extract.

Accreditation needs the same care. Joint Commission and LegitScript badges appear on the program’s own pages, but no accreditation or registration number is given for either, so ask for both along with the state license number before you commit to a placement. The bed count for this home is not published either: the figure the operator gives covers more than 200 beds across two countries, not this address.

No reviews have been left for Into Action Recovery on RehabSeekers yet. This listing is built from state licensing and federal provider records alongside the program’s own published pages. If you or someone in your family has stayed at the Laveen home, a review covering the structure of the day, the staffing at night and how the step-down to Scottsdale worked would help the next caller a great deal.
