Optum is not an insurance company. It is the health services arm of UnitedHealth Group that administers behavioral health benefits, and it manages addiction treatment authorizations both for UnitedHealthcare members and for a number of employers and other health plans that contract with it separately to handle this part of their coverage.

That distinction matters when you call. If your card says UnitedHealthcare, Optum is who reviews your rehab admission. If your employer contracted Optum directly, your medical benefits may sit with another carrier entirely. Either way, Optum holds the behavioral network and makes the authorization decision.

Does Optum cover rehab?

Yes, when the member's health plan includes substance use treatment. Optum does not sell insurance; it manages the behavioral health benefit and decides which level of care is authorized using The ASAM Criteria, 4th Edition. Is Optum the same as UnitedHealthcare?

No. UnitedHealth Group runs two main divisions: UnitedHealthcare, which sells insurance and managed care, and Optum, the health services business covering care delivery, pharmacy benefits and data analytics. Does Optum cover IOP?

Yes, intensive outpatient is one of the levels Optum manages, and it is treated as a non-routine service that needs authorization under the national manual. Texas, Kentucky, New Jersey and Louisiana Medicaid plans all require prior authorization for intensive outpatient programs. Cost and visit limits are set by the member's own plan.

Yes, generic buprenorphine-naloxone is covered on both of the main drug lists, though brand Suboxone is not. UnitedHealthcare's 2026 Advantage 3-Tier list puts the generic film and tablet on Tier 2 with a quantity limit and no prior authorization; the Optum Rx Premium Standard list puts it on Tier 1.

Yes, methadone is one of the three opioid use disorder medications Optum recognizes alongside buprenorphine and naltrexone, but payment follows the member's plan. What Optum Actually Is

Optum is a managed behavioral health organization inside UnitedHealth Group. It does not sell you insurance. It administers the mental health and substance use portion of someone else’s plan: most often UnitedHealthcare, but also employer plans and other health plans that carve out behavioral health to a specialist administrator.

Which Card Do You Hold

Look at the back of your insurance card for a separate behavioral health or mental health number. If one is listed, that is usually the administrator, and on many plans it is Optum. Calling it directly reaches the people who make the decision, rather than a general member services line that will transfer you anyway.

Does Optum Cover Rehab?

The benefit belongs to your plan, and Optum administers it. In practice that means addiction treatment is covered across detox, residential, partial hospitalization, intensive outpatient and outpatient care, because federal parity law requires it and because the underlying plans are subject to that law.

Ask any program two questions before admission. How does it handle concurrent review with Optum, and what happens financially if an extension is refused mid-stay. Programs that answer clearly have thought about it; programs that deflect have usually left the risk with you.

Finding an In-Network Facility

Search the Optum behavioral health directory rather than the general medical directory of whichever carrier your card names, because the two are maintained separately and are not always consistent. Then verify with the facility, and ideally with Optum, because directory accuracy across the industry is poor and a facility can leave a network without the listing changing.

Whether out-of-network is viable depends on the underlying plan rather than on Optum. PPO plans pay something outside the network. HMO and EPO plans generally pay nothing except in emergencies.

Detox
