What Optum Actually Is

Optum is a managed behavioral health organization inside UnitedHealth Group. It does not sell you insurance. It administers the mental health and substance use portion of someone else’s plan: most often UnitedHealthcare, but also employer plans and other health plans that carve out behavioral health to a specialist administrator.

For anyone seeking addiction treatment, this is a practical fact rather than a corporate one. Optum holds the behavioral provider network, publishes the directory you should be searching, applies the medical necessity criteria, and issues or refuses the authorization that determines whether a residential admission is paid for.

Which Card Do You Hold

Does Optum Cover Rehab?

The benefit belongs to your plan, and Optum administers it. In practice that means addiction treatment is covered across detox, residential, partial hospitalization, intensive outpatient and outpatient care, because federal parity law requires it and because the underlying plans are subject to that law.

What Optum determines is whether a particular level of care is medically necessary for you, at a particular facility, for a particular length of time. Those determinations are made against published clinical criteria, and you are entitled to see them.

Authorization and Concurrent Review

Inpatient detox and residential treatment require prior authorization. A facility normally initiates it, submitting clinical information for review before admission. If approved, authorization typically covers an initial block of days rather than a whole stay.

Continued care is then reviewed as treatment progresses, which is where families are most often caught out. Someone admitted for what was described as a thirty-day program can be told at day twelve that criteria for continued residential care are no longer met. This is concurrent review functioning as designed, not an error, and it is appealable.

Finding an In-Network Facility

Search the Optum behavioral health directory rather than the general medical directory of whichever carrier your card names, because the two are maintained separately and are not always consistent. Then verify with the facility, and ideally with Optum, because directory accuracy across the industry is poor and a facility can leave a network without the listing changing.

Whether out-of-network is viable depends on the underlying plan rather than on Optum. PPO plans pay something outside the network. HMO and EPO plans generally pay nothing except in emergencies.

Coverage by Level of Care

Detox

Medically supervised withdrawal is covered where physical dependence is established. In acute withdrawal from alcohol or benzodiazepines, emergency care applies and an emergency department is the right destination regardless of authorization.

Residential

Covered where criteria are met, with the authorization and review process described above. Criteria generally follow recognized standards such as those published by the American Society of Addiction Medicine, and an assessment should place you at a defined level rather than at whatever a program offers.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient are covered and commonly authorized, and are the usual alternative offered when residential is declined. Outpatient therapy rarely requires authorization. Medication for opioid use disorder is covered, with the drug itself falling under your pharmacy benefit rather than Optum’s behavioral one.

Appeals and Your Rights

Federal parity law entitles you to the specific criteria used in any denial, in writing, and to appeal. Expedited appeals are available while someone remains in treatment, which is the route to use when a stay is cut short rather than accepting discharge and disputing later.

Where the underlying plan is a self-funded employer plan, appeals run under federal ERISA rules and ultimately to independent external review. Where it is fully insured, your state insurance regulator provides an external review route as well. Ask which applies to you, because the timelines differ.

What Treatment Costs

Optum does not set your cost sharing; your plan does. Deductible, coinsurance and out-of-pocket maximum come from the plan document rather than from the behavioral administrator. The market rates below show the underlying cost of treatment before any coverage applies.

Dual Diagnosis and Aftercare

Optum administers mental health and substance use benefits together, which should make authorization for integrated dual diagnosis treatment more straightforward than where the two are split across administrators. Continuing outpatient care, psychiatric review and medication management after a residential stay are covered, and telehealth is widely available. Recovery housing is not a covered medical benefit.
