Anthem / Elevance Health

Rehabs That Take Anthem

Anthem is the largest Blue Cross Blue Shield licensee, operating Blue plans across fourteen states under a parent company renamed Elevance Health in 2022. Addiction treatment is covered across detox, residential care, partial hospitalization, intensive outpatient and outpatient therapy, subject to medical necessity and prior authorization.

Behavioral health is administered by Carelon Behavioral Health, an Elevance company formerly known as Beacon Health Options. Carelon reviews the rehab admission and holds the provider directory, so the behavioral health number on your card reaches the people deciding, while the general member services line usually does not.

Why Trust RehabSeekers.com?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

Does Anthem cover rehab?

Yes. Anthem covers substance use treatment across detox, inpatient and residential care, partial hospitalization, intensive outpatient and outpatient counseling, subject to medical necessity and plan rules. Individual certificates include inpatient facility, residential treatment, PHP, IOP, office visits and virtual visits as behavioral health benefits.

Behavioral health sits with Carelon Behavioral Health, part of the same parent company, Elevance Health, so the behavioral health number on your ID card reaches the team reviewing the admission.

Private Insurance Acceptedhttps://rehabseekers.com/insurance-accepted/private-insurance-accepted/
Does Anthem cover detox?

Yes, detox is a covered benefit. Anthem's Virginia Medicaid plan covers residential detox and inpatient withdrawal management at ASAM levels 3.7 and 4.0, and its New York Medicaid behavioral health grid includes inpatient detox and ambulatory detox. Commercial certificates group detox under inpatient substance use services.

In New York, inpatient detox needs notification within two business days rather than prior authorization, and ambulatory detox needs no authorization in network.

Detox Centershttps://rehabseekers.com/programs/detox-centers/
Does Anthem cover residential rehab?

Yes, with precertification. On Anthem Bronze Pathway Essentials X HMO 6000 in Colorado for 2023, inpatient facility care including residential treatment cost $500 per admission plus 40% coinsurance. On the 2026 California Silver 70 EPO, residential treatment facility fees are 30% coinsurance in network.

Those HMO and EPO plans do not cover out-of-network residential care except emergencies, so the center must be in the plan's network.

Residential Rehabhttps://rehabseekers.com/programs/residential/

Or claim or add your listing for free.

Does Anthem Cover Drug and Alcohol Rehab?

Yes. Anthem covers substance use disorder treatment across the continuum of care, as federal parity law requires. Anthem is the Blue Cross Blue Shield licensee in California, New York, Ohio, Indiana, Kentucky, Virginia, Georgia, Missouri, Wisconsin, Colorado, Connecticut, Maine, Nevada and New Hampshire, which makes it the largest single Blue operator in the country.

The corporate parent renamed itself Elevance Health in 2022, but the plans people hold still say Anthem in most markets, and Anthem Blue Cross in California. Nothing about coverage changed with the rebrand; it matters only because search results and older documents use both names.

Carelon Reviews Your Admission

Elevance administers behavioral health through Carelon Behavioral Health, formerly Beacon Health Options. For addiction treatment that means Carelon holds the network directory, applies the medical necessity criteria and issues the authorization. Use the behavioral health number on your card rather than general member services.

Coverage by Level of Care

Medical Detox

Medically supervised withdrawal is covered where dependence is physical. Inpatient detox requires prior authorization in almost all cases, normally initiated by the facility. Emergency care is covered regardless of network, and acute withdrawal from alcohol or benzodiazepines belongs in an emergency department rather than a waiting list.
