Does Anthem Cover Rehab in Indiana?

Yes. Anthem covers substance use treatment on every plan line it sells in Indiana, and its Healthy Indiana Plan page lists substance use disorder services as covered in both HIP Plus and HIP Basic. You can find rehabs that accept Anthem among the Indiana treatment programs listed here.

Anthem Insurance Companies, Inc. holds the Blue Cross and Blue Shield license for Indiana, and Elevance Health, renamed from Anthem, Inc. in June 2022, keeps its headquarters at 220 Virginia Avenue in Indianapolis, where the company began in 1946 as a pair of mutual hospital and medical insurers.

Anthem holds contracts in every one of Indiana Medicaid’s four managed care programs, and since January 1, 2026 it has also carried the former Indiana University Health Plans members. Anthem also sells individual and family plans in Indiana for 2026, along with Medicare Advantage HMO, PPO, HMO-POS and dual special needs plans.

Does Anthem Cover Inpatient Rehab in Indiana?

Yes. Anthem’s Indiana Medicaid provider manual describes residential treatment and withdrawal management among covered substance use services, and Indiana Medicaid covers residential treatment programs at Level 3.1 and 3.5 when Indiana’s Division of Mental Health and Addiction designates them. A 2024 Anthem individual certificate lists inpatient substance use care and residential treatment among mental health and substance use benefits.

How long will Anthem pay for residential rehab?

Anthem bases the length of a residential stay on medical criteria, not a fixed day count. For Indiana Medicaid members it applies MCG guidelines together with The ASAM Criteria to substance use requests, and Indiana Medicaid’s rule since February 2019 is that every plan authorizes at least 14 days of residential care taking insurance before reassessing.

Does Anthem need prior authorization for inpatient rehab?

Yes. Anthem’s Indiana Medicaid plans require prior authorization for inpatient care, partial hospitalization, intensive outpatient treatment and residential substance use services, filed on the state’s single form. Provider lines are 866-408-6132 for Hoosier Healthwise, 844-533-1995 for HIP and 844-284-1798 for Hoosier Care Connect and PathWays. Anthem individual certificates also require precertification for facility-based substance use care.

Does Anthem Cover Detox in Indiana?

Yes. Anthem’s Indiana Medicaid provider manual lists withdrawal management among its substance use services, and the state pays for inpatient detox once it is authorized. Commercial and marketplace members reach medical detox that takes insurance as an inpatient facility service, and alcohol detox programs taking insurance appear in the listings below.

Does Anthem Cover Outpatient Rehab in Indiana?

Yes. Anthem covers outpatient counseling on all its Indiana plans, and its Indiana Medicaid manual names intensive outpatient treatment and partial hospitalization as covered levels. Anthem individual certificates list intensive outpatient treatment, partial hospitalization, outpatient office visits and virtual visits, and LiveHealth Online offers virtual therapy and psychiatry through the Sydney Health app.

Anthem Indiana Medicaid counts substance use disorder as a qualifying condition for Condition Care, its program in which licensed nurses help members manage ongoing health needs. That support runs alongside aftercare and continuing care, which helps members stay connected to treatment after a residential or intensive outpatient stay ends.

Does Anthem Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Anthem’s Indiana Medicaid provider manual lists opioid treatment services including methadone, buprenorphine and naltrexone, and CarelonRx, a sister company within Elevance Health, manages Anthem’s pharmacy benefits and drug lists. Members can look up medication-assisted treatment on the drug list for their own Anthem plan.

Does Anthem cover Suboxone?

Yes. Anthem agreed in January 2017 to stop requiring prior authorization for buprenorphine treatment of opioid use disorder on most Anthem-affiliated plans. For Indiana Medicaid members, new state criteria for opioid use disorder drugs took effect in the plans on February 1, 2026, and buprenorphine maintenance programs are in the listings.

Does Anthem cover methadone?

Yes. Anthem’s Indiana Medicaid members get methadone programs at opioid treatment programs, and state rules let that care start without prior authorization. Indiana Medicaid also pays for naltrexone through those programs, and Vivitrol programs taking insurance appear below for members who prefer a monthly shot.

Anthem Rehab Centers Near the Indiana Dunes, Steuben County Lakes and the Ohio River

Anthem’s Indiana Medicaid plans are statewide with no regional limits, so the plan rules stay the same from the dunes to the Ohio River. Individual HMO members must stay in network outside emergencies, which matters most in rural settings with fewer programs nearby.

Anthem rehab near the Indiana Dunes

Anthem members in Porter County live along the Indiana Dunes, the Lake Michigan shoreline between Portage, Chesterton and Michigan City. Anthem Medicaid, marketplace and employer members using treatment programs near beaches or rehab programs in Portage follow the same authorization rules used statewide.

Anthem rehab near the Steuben County lakes

Anthem members in Steuben County, Indiana’s lake corner near Angola, Lake James and Pokagon State Park, live close to the Michigan and Ohio lines. Anthem’s Indiana plan rules apply to lakeside rehab programs and treatment near Angola, while programs over the state line fall under each plan’s out-of-network rules.

Anthem rehab along the Ohio River

Anthem members in Jefferson and Dearborn counties live along the Ohio River at Madison, Aurora and Lawrenceburg, across from Kentucky. Members can compare rehab centers near rivers and programs around Madison, and Elevance also holds the Blue license in Kentucky and Ohio across the water.

How Do I Find a Rehab That Accepts Anthem in Indiana?

Browse the listings on this page. Each entry pairs a phone number with the program’s care levels and accreditation, and filters cover city, service and insurance. Programs that take Indiana Medicaid plans are marked in the filters, and intake staff can tell you which Anthem lines they accept.

Anthem admission to residential, inpatient, partial hospitalization or intensive outpatient care starts with authorization. Intake staff at the program you choose usually send it to Anthem, or members can call the member line for their plan, listed below, and say where they want to go. Anthem’s behavioral health crisis line for Indiana Medicaid members, 833-874-0016, answers 24 hours a day.

Anthem rehab in Indianapolis

Anthem’s home market is Indianapolis. Elevance Health is headquartered there, and all four of Anthem’s Indiana Medicaid programs, its marketplace plans and its Medicare Advantage plans are sold side by side. The same Anthem authorization rules cover drug rehab in Indianapolis and Castleton treatment centers on the north side.

Anthem rehab in Bloomington and south-central Indiana

Anthem took over Indiana University Health Plans, an Indiana-only Medicare Advantage and employer plan carrier, after a deal announced in September 2024 and closed at the end of that year. On January 1, 2026 those plans became Anthem Blue Cross and Blue Shield plans, so former members near Bloomington rehab centers now hold Anthem cards.

Anthem rehab in Merrillville and Lake County

Anthem’s HealthLink network gives State of Illinois employee plan members network access in Indiana, and its visitor search sorts members living in Indiana, Kentucky, Ohio and Wisconsin into one group. That helps Illinois state workers living in Lake County who want downtown Merrillville programs close to home.

Anthem rehab in Evansville and Fort Wayne

Anthem’s Hoosier Care Connect plan covers blind and disabled members under 60 in both Vanderburgh and Allen counties, with residential and intensive outpatient care authorized on the same state form. Members there can compare Evansville rehab programs and downtown Fort Wayne treatment centers.

Is Anthem the same as Elevance Health or Carelon?

Not quite. Anthem Blue Cross and Blue Shield is the Indiana health plan, Elevance Health is its Indianapolis parent, and Carelon Behavioral Health, formerly Beacon Health Options, is a sister company Elevance has owned since 2020. Anthem’s Indiana Medicaid behavioral health page names no outside vendor; authorization runs through Anthem’s own lines.

Can I use Anthem for rehab in a different state?

Usually only with the plan’s agreement. Anthem individual HMO certificates from Connecticut, Colorado and New Hampshire pay for out-of-network substance use care only in emergencies or urgent care. For Anthem’s Indiana Medicaid plans, a 2019 quick reference says out-of-network providers need prior authorization for every service, so out-of-state treatment needs the plan’s agreement first.

How much does rehab cost with Anthem?

Anthem costs depend on the line. Indiana Medicaid members pay little or nothing, with HIP Plus members paying a small monthly POWER account contribution and HIP Basic members small copays. Anthem marketplace plans in Indiana come in Bronze, Silver, Gold and Platinum tiers with a deductible and coinsurance, and employer members find their share in the plan’s benefit summary.

Anthem is one route among several for paying for care in Indiana. The Indiana Chamber’s ChamberCare plan moved from Anthem to SIHO on October 1, 2025, so some small business members changed carriers. Self-pay treatment programs and rehabs accepting Medicare are other routes listed here.
