Does Medicare cover drug and alcohol rehab?

Yes, in part. Part A covers inpatient hospital treatment for substance use disorders, Part B covers outpatient care, intensive outpatient programs, partial hospitalization and Opioid Treatment Program services, and Part D covers many addiction medications at the pharmacy. Non-hospital residential programs are generally not covered.

How many days of rehab does Medicare cover?

Part A covers up to 90 days of inpatient hospital care per benefit period, plus 60 lifetime reserve days that can be used once. A benefit period ends after 60 days in a row with no inpatient or skilled care, and there is no limit on the number of benefit periods.

Care in a freestanding psychiatric hospital also carries a 190-day lifetime limit, which does not apply to a certified psychiatric unit inside a general hospital.

How much does Medicare pay for rehab?

Inpatient hospital care costs $1,736 per benefit period in 2026, with $0 a day for days 1 through 60, $434 a day for days 61 through 90, and $868 per lifetime reserve day. Outpatient and intensive outpatient services cost 20% of the Medicare-approved amount after the $283 Part B deductible.

Original Medicare has no yearly cap on out-of-pocket costs unless you carry Medigap or join a Medicare Advantage plan.

Medicare rehab means substance use and mental health treatment paid for by Medicare, the federal health insurance program for anyone age 65 and older and some younger adults with certain disabilities or conditions. The coverage is split across the program’s parts: Part A for an inpatient hospital stay, Part B for the outpatient levels of care, and Part D for prescriptions picked up at a pharmacy. Medicare is a government program run by the Centers for Medicare & Medicaid Services, not an insurance company, and it was created under the Social Security Act in 1965.

What decides whether Medicare pays is the setting a program bills from, not the word on its sign. The settings Medicare names are an inpatient general or psychiatric hospital under Part A, and under Part B a clinician’s office, a hospital outpatient department, a community mental health center, a Federally Qualified Health Center, a Rural Health Clinic and an Opioid Treatment Program. Non-hospital residential treatment is not on that list, and that single gap decides more admissions than any dollar figure on this page.
