MultiPlan and PHCS are not insurance companies and they do not pay claims. They are provider networks that other plans rent, which is why a PHCS or MultiPlan logo turns up on cards issued by employers and smaller insurers who wanted national network access without building one of their own.

For addiction treatment the practical effect is that MultiPlan tells you which facilities have agreed discounted rates, while the plan named alongside it decides what is actually covered, what needs authorizing and what share of the bill falls to you. You will usually need to speak to both.

No, MultiPlan does not cover rehab, because MultiPlan and PHCS are provider networks rather than insurers. What MultiPlan and PHCS Actually Are

MultiPlan operates provider networks that other organizations rent. PHCS is one of its network brands. If your insurance card carries a PHCS or MultiPlan logo alongside another name, the second name is the entity paying your claims and the MultiPlan mark indicates which network of contracted providers you can use at negotiated rates.

This arrangement is common among self-funded employer plans and smaller regional insurers. Building a national network is expensive; renting one is not. It works well until someone needs to establish what is covered, at which point the split between network and payer becomes confusing.

Two Different Questions

Is this facility in the MultiPlan or PHCS network is a question about rates. Is this treatment covered, authorized and at what cost share is a question for the plan administrator named on your card. A facility that says it takes PHCS has answered only the first, and people regularly admit on the strength of it.

Does It Cover Rehab?

The coverage belongs to the underlying plan. Most plans using these networks are employer plans subject to federal parity law, which requires substance use benefits to sit on equal terms with medical ones. In practice detox, residential treatment, partial hospitalization, intensive outpatient and outpatient care are covered, subject to the plan’s own terms.
