Idaho Medicaid covers outpatient, inpatient, residential and crisis treatment through the Idaho Behavioral Health Plan, a statewide plan run by Magellan Healthcare. 231 of the 238 facilities listed in Idaho accept it, and anyone with a substance use disorder is exempt from the expansion work requirement.

Are rehabs in Idaho licensed?

No, not for adults. Idaho issues no license for an adult substance use program. Instead, residential programs must hold accreditation from the Joint Commission, the Council on Accreditation or CARF plus an ASAM level of care certification, outpatient care runs through the plan’s enrolled providers, and only adolescent programs are licensed (Idaho Department of Health and Welfare, IDAPA 16.07.17).

Accreditation is voluntary in most states, and in Idaho it carries the weight a license carries elsewhere. Joint Commission accredited rehabs have passed an outside survey of charts, staffing and safety, and CARF accredited programs a comparable review. Setting and surroundings: where Idaho rehabs are

Substance use in Idaho: what the data shows

Idaho’s overdose deaths have fallen two years running. Federal data put the state at 326 drug overdose deaths for the 12 months ending December 2024, 11.2% below the 367 a year earlier, and at 265 for the 12 months ending May 2026 (CDC provisional counts, 12 months ending May 2026).

The state’s own measure agrees. Idaho’s overdose death rate dropped 14% in 2024, to 16.8 per 100,000, from a high of 19.6 in 2023, and its fentanyl rate fell 36% that year, though the rate remains 46% above 2012 (Idaho Statistical Analysis Center, June 2026). The 2022 drug-induced mortality rate of 19.6 per 100,000 ran well under the national 32.6 (Idaho Office of Drug Policy, 2024 needs assessment).

About 273,000 Idahoans aged 12 or older, 16.43%, had a substance use disorder in 2023 to 2024 (SAMHSA NSDUH 2023–2024 state tables). Of those classified as needing treatment in 2024, 81.56%, about 228,000 people, went without. About 116,000 adults had a co-occurring disorder, and 138 listed facilities in Idaho treat both.

Is overdose still rising in Idaho?
