Does TRICARE Cover Drug and Alcohol Rehab?

Yes. TRICARE covers substance use disorder treatment as a medical benefit, spanning emergency and inpatient detoxification, residential treatment, partial hospitalization, intensive outpatient programs, outpatient therapy and medication-assisted treatment. Care must be medically necessary and delivered by a TRICARE-authorized provider.

TRICARE is a Defense Health Agency program rather than a commercial insurer, which changes the texture of the whole process. There is no employer plan document to interpret and no network negotiation in the commercial sense. There is a uniform benefit, a set of authorized providers, and regional contractors who administer it.

East, West and Which Contractor Holds You

That transition was rough. Referral processing problems in the West prompted the Defense Health Agency to temporarily waive the requirement for contractor approval of outpatient specialty referrals during 2025, and to extend point-of-service fee waivers while the backlog cleared. Operations have since stabilised, but the episode is worth knowing about if you are working through a claim or authorization from that period.

Plan Types and What They Require

TRICARE Prime

Prime works like an HMO. You have a primary care manager, and specialty care generally requires a referral from them. For addiction treatment this is the step people most often skip, and seeking care without a referral can mean point-of-service charges rather than the ordinary cost share. Active duty service members are enrolled in Prime and generally receive care at no cost, with treatment usually beginning at a military hospital or clinic.

TRICARE Select

Select works like a PPO. You may see any TRICARE-authorized provider without a referral, and cost shares are higher than Prime. Some higher levels of care still require prior authorization, so authorization and referral are separate questions worth asking separately.

Other Programs

TRICARE For Life coordinates with Medicare for those eligible for both. TRICARE Reserve Select and Retired Reserve cover National Guard and Reserve members under their own terms. Family members of veterans may instead be covered by CHAMPVA, which is a separate program administered by the Department of Veterans Affairs, not TRICARE.

Coverage by Level of Care

Detox and Withdrawal Management

Emergency and inpatient detoxification are covered. Where withdrawal is medically dangerous, as with alcohol, benzodiazepines and barbiturates, emergency care takes precedence over paperwork and a military treatment facility or emergency department is the right destination.

Residential Treatment

TRICARE covers residential substance use disorder treatment at authorized facilities. Prior authorization is required, and the facility must hold TRICARE authorization specifically, which is a narrower category than simply being licensed and accredited. Ask a program directly whether it is TRICARE-authorized rather than whether it accepts TRICARE.

PHP, IOP and Outpatient

Partial hospitalization, intensive outpatient and standard outpatient care are all covered, as is medication-assisted treatment for opioid use disorder. Telehealth options have widened access for those stationed away from major medical centers.

Confidentiality and Career Concerns

For service members this is frequently the real obstacle rather than cost. Command notification rules differ by service and by circumstance, and self-referral is treated differently from a command-directed evaluation or an incident-driven one. It is a legitimate question to ask a military treatment facility or a chaplain before starting, and asking it does not itself trigger anything. Avoiding treatment altogether carries the larger risk to a career.

Verifying TRICARE Coverage

Add two questions specific to TRICARE. Is this facility TRICARE-authorized, and is it in my regional contractor’s network? And does my plan require a primary care manager referral before admission? Those two settle most of what goes wrong.

Costs Under TRICARE

Costs are set by the program rather than negotiated plan by plan, and they are considerably lower than commercial market rates. Active duty members generally pay nothing. Other beneficiaries pay set cost shares that depend on plan, sponsor status and whether care is in network. The market rates below are for context, showing what the same treatment costs without a TRICARE benefit behind it.

Coverage for Specific Substances

TRICARE covers treatment for alcohol and drug dependence without distinction by substance, though the pathway differs. Alcohol remains the most common presentation among service members and veterans, and its withdrawal carries real medical risk, which makes supervised detox the appropriate starting point where dependence is physical.

Opioid use disorder is covered including medication-assisted treatment. Prescription opioid dependence that began with a service-connected injury is a familiar route into this, and it carries no more stigma clinically than any other. Stimulant and cannabis use disorders are covered through behavioral treatment, since no approved medication exists for either.

PTSD, Trauma and Co-Occurring Conditions

Substance use alongside post-traumatic stress is common enough in this population that any program treating service members should be equipped for both. TRICARE covers integrated treatment, and trauma-focused therapies including EMDR and cognitive processing therapy are well supported by evidence and available through authorized providers.

Treating the drinking while leaving the trauma untouched tends to produce a short remission. Ask a program directly what trauma-specific therapy it delivers, who delivers it, and whether clinicians have experience with combat trauma or military sexual trauma specifically, which are not interchangeable with civilian trauma experience.

Family Members and Dependents

Spouses and dependent children are covered under the sponsor’s TRICARE benefit, and family involvement in treatment is itself a covered service where clinically indicated. Deployment cycles, relocation and the strain of military life affect the whole household, and family therapy is frequently the part of treatment that determines whether gains survive the return home.

Family members of veterans who are not TRICARE-eligible may be covered by CHAMPVA instead, which is administered by the Department of Veterans Affairs under separate rules. Veterans themselves are generally served through VA healthcare or VA Community Care rather than TRICARE, unless they are retirees with TRICARE eligibility.

Appeals and Getting a Decision Reviewed

Where authorization is denied, TRICARE provides a formal appeal process through the regional contractor, with defined timelines and an expedited route where delay would harm health. Requests for reconsideration should go to Humana Military in the East Region or TriWest in the West, depending on where you live.

Keep a written record of every referral, authorization number and call. This proved its worth during the 2025 West Region transition, when referral processing problems left beneficiaries needing to evidence what had been approved and when. Beneficiaries who could produce their paperwork resolved issues considerably faster than those relying on the contractor’s records.

Aftercare and Ongoing Support

Continuing care after a residential stay is covered, including outpatient therapy, medication management and intensive outpatient programs. For someone still serving, the practical question is how treatment fits around duty, and evening or virtual intensive outpatient is frequently the workable answer.

Peer support outside the clinical system matters here too. Veteran-specific meetings and groups exist in most areas, and for many people the value of talking to someone who understands the context is not something a covered benefit provides.
