Rehabs That Take UnitedHealthcare

UnitedHealthcare is the largest health insurer in the United States, and addiction treatment is covered on essentially every plan it writes. Behavioral health is administered by Optum, a sister company under UnitedHealth Group, so the network directory and the authorization decision for rehab both sit with Optum rather than UnitedHealthcare.

Plan type decides most of what follows. An employer PPO, a UnitedHealthcare Medicare Advantage plan and a UnitedHealthcare Community Plan Medicaid product have different networks and different rules at admission. Check which you hold, and use the behavioral health number on your card rather than the general one.

Why Trust RehabSeekers?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Licensing and accreditation: Shown where the state regulator, The Joint Commission or CARF lists the facility.

Corrections welcome: Facilities can tell us what is out of date, and we update the listing.

100% independent: We are independent and never accept any commissions.

Direct connections only: Listings show direct contacts to avoid third-party centers.

Does UnitedHealthcare Cover Drug and Alcohol Rehab?

Yes. UnitedHealthcare covers detox, residential treatment, partial hospitalization, intensive outpatient care and standard outpatient therapy. Federal parity law requires substance use benefits to carry no tighter restrictions than comparable medical benefits, and marketplace and most employer plans must include them as an essential health benefit.

UnitedHealthcare sits inside UnitedHealth Group alongside Optum, and behavioral health is Optum’s territory. When a facility says it has verified your benefits, it has usually spoken to Optum Behavioral Health. When an admission is authorized or declined, that decision is Optum’s. Knowing this shortens most phone calls considerably.

Which UnitedHealthcare Do You Have

UnitedHealthcare writes employer coverage, Medicare Advantage, and Medicaid managed care under the UnitedHealthcare Community Plan name in a number of states. These are separate networks with separate rules. A residential program in network for an employer PPO may be entirely unavailable on a Community Plan, and the reverse happens too.

Coverage by Level of Care

Medical Detox

Medically supervised withdrawal is covered where physical dependence is established, typically three to seven days for alcohol and five to ten for opioids. Inpatient detox requires prior authorization in almost all cases. If someone is in acute withdrawal from alcohol or benzodiazepines, emergency care is covered regardless of network status, and an emergency department is the right destination rather than a waiting room.

Residential Treatment

Residential care is covered against medical necessity criteria, with prior authorization and ongoing concurrent review. Optum reviews continued stay as treatment progresses rather than approving a full ninety days at admission. Ask any program what its approval rate looks like with Optum specifically, and what happens financially if an extension is refused mid-stay.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient are covered and commonly authorized, and are frequently offered as the alternative when residential is declined. Standard outpatient therapy needs no authorization on most plans. Medication for opioid use disorder is covered, with formulary placement depending on your pharmacy benefit.

Finding an In-Network Facility

Use the Optum behavioral health directory rather than the general UnitedHealthcare provider search, since the two are not always in step. Verify directly with the facility as well, because directories across the whole industry carry stale entries and a facility that left the network last quarter may still be listed.

Plan structure governs whether out-of-network is even worth considering. PPO plans pay something outside the network. HMO and EPO plans generally pay nothing except emergencies. Community Plan Medicaid coverage is effectively in-network only. This single question decides whether a well-known program in another state is realistic or not.

Verifying Coverage Before Admission

Ask before you admit

Is this specific facility in network, and what does my plan pay if it is not?

What are my deductible, coinsurance and out-of-pocket maximum this year, and how much have I already met?

Does detox, residential, PHP or IOP require prior authorization?

How is continued stay reviewed, and what is the appeal route if care is denied?

Ask specifically whether your employer plan is self-funded. Large employers frequently pay claims themselves while UnitedHealthcare administers them, which places the plan under federal ERISA rules and changes where an appeal goes. Nothing on the card indicates this.

What Treatment Costs With UnitedHealthcare

Typical US market rates before insurance
Level of careMonthlyDaily
Medical detox (7–14 days)$1,500 – $5,200 per week$215 – $745
Residential inpatient$6,200 – $26,000$205 – $865
PHP$7,200 – $14,000$240 – $465
IOP$2,700 – $9,200$90 – $305
Outpatient$1,300 – $5,200$45 – $175

Your actual cost depends on deductible, coinsurance and out-of-pocket maximum. That maximum is the meaningful number for residential care, because once it is met, in-network covered treatment is paid at 100 percent for the remainder of the plan year.

Medicare Advantage and Community Plan Coverage

UnitedHealthcare is the largest Medicare Advantage insurer in the country. Those plans cover addiction treatment including inpatient and outpatient care, with networks tighter than commercial PPOs and referral rules that vary by plan. Late-onset drinking and alcohol interacting with prescribed medication are common in this population and frequently missed, so a primary care conversation is often the quickest legitimate route in.

UnitedHealthcare Community Plan administers Medicaid in a number of states, including Arizona and Tennessee. Benefits follow each state’s Medicaid program, generally with little or no cost sharing and a narrow network. Give the plan name and your state when you call a facility rather than simply saying Medicaid.

Coverage for Specific Substances

Benefits do not vary by drug, but the treatment pathway does, and that shapes what Optum will authorize. Alcohol, benzodiazepine and barbiturate dependence carry medically dangerous withdrawal, which usually makes supervised detox straightforward to justify clinically.

Opioid use disorder is covered including medication-assisted treatment with buprenorphine, methadone through licensed opioid treatment programs, and naltrexone. Leading clinical bodies treat medication as the standard of care for opioid dependence, and coverage reflects that. Stimulant use disorder, whether methamphetamine or cocaine, has no approved medication, so treatment is behavioral and residential authorization typically requires a stronger case built on psychosocial factors rather than withdrawal risk.

Dual Diagnosis and Integrated Treatment

Co-occurring mental health conditions are common in addiction treatment and are covered under the same parity protections. Optum manages both halves of that picture, which in principle should make integrated authorization simpler than with carriers that split behavioral and medical administration.

In practice, the quality question sits with the program rather than the insurer. Ask whether psychiatric care is delivered on site by staff or arranged externally, whether medication can be started and adjusted during a residential stay, and how the program handles someone who arrives already on psychiatric medication.

Denials and How to Appeal

Most addiction treatment denials fall into a small number of categories: level of care judged unnecessary, continued stay declined during review, out-of-network facility on a plan that excludes it, or a missing referral on a plan that requires one. The first two are appealable on clinical grounds.

Federal parity law entitles you to the specific criteria applied to your case, in writing. Expedited appeals are available while someone remains in treatment, and are the right route when a residential stay is cut short. Self-funded employer plans appeal under ERISA and ultimately to independent external review; fully insured plans also have a state regulator route. Ask the facility to file, and keep your own record of dates and reference numbers.

Aftercare and Step-Down

Treatment that ends abruptly at discharge produces worse outcomes than treatment followed by structured step-down and ongoing support. Outpatient therapy, psychiatric review and medication management are all covered, and telehealth has widened access considerably for people returning to work or living far from a clinic.

Recovery housing is the usual exception. Sober living is generally not a medical benefit and is paid privately, so build that cost into planning during the stay rather than at the end of it.
