Treatment Centers in Indiana That Accept Humana

Humana covers drug and alcohol rehab in Indiana mainly for older adults and military families: through Humana Healthy Horizons in Indiana, its plan for Indiana PathWays for Aging, through Medicare plans including a dual plan for people with both Medicare and Medicaid, and through TRICARE East, which Humana Military runs.

The listings below give each facility’s phone line, the care it offers (residential, intensive outpatient or methadone, for example) and any Joint Commission or CARF accreditation. The sources are government treatment registers and Indiana licensing records. Pick a city or a level of care, then ask whether the program bills your Humana plan.

Does Humana Cover Rehab in Indiana?

Yes. Humana covers substance use treatment in Indiana for PathWays, Medicare and TRICARE members, and its 2026 Indiana PathWays handbook lists residential substance use disorder services, inpatient behavioral health care, partial hospitalization and substance use disorder services among covered benefits. Programs that bill rehabs that accept Humana appear among the substance use programs in Indiana listed here.

Humana Healthy Horizons in Indiana is the brand of Arcadian Health Plan, Inc., one of three plans in Indiana PathWays for Aging, the Medicaid program for Hoosiers 60 and over, alongside Anthem and UnitedHealthcare. Humana does not serve HIP, Hoosier Healthwise or Hoosier Care Connect, and its behavioral health work is done in-house by Humana Behavioral Health, once called LifeSynch.

Humana’s Medicare business reaches Indiana too. Humana sold 2026 Medicare Advantage plans in 46 states and Washington, D.C., and lists two Indiana special needs plans: PathWays Dual Care, a fully integrated plan for people with both Medicare and Medicaid, and Humana Together in Health for residents of long-term care facilities.

Does Humana Cover Inpatient Rehab in Indiana?

Yes. Humana’s 2026 Indiana handbook lists residential substance use treatment and inpatient behavioral health services as covered for PathWays members, and Indiana Medicaid pays for Level 3.1 and 3.5 residential care in designated facilities. Humana Medicare plans follow Medicare, which covers hospital addiction treatment under Part A.

How long will Humana pay for residential rehab?

Humana PathWays members fall under Indiana Medicaid’s residential rule: since February 2019 every plan has authorized at least 14 days before reassessing a stay, with length then set by medical need. For Humana Medicare members, Medicare limits care in a freestanding psychiatric hospital to 190 days over a lifetime, a limit that does not apply in a general hospital.

Does Humana need prior authorization for inpatient rehab?

Yes. Humana’s Indiana handbook says some services need prior authorization, naming hospital stays outside emergencies, and Indiana Medicaid requires it for every residential or inpatient substance use admission on the state form. Providers reach Humana’s Indiana authorization line at 866-274-5888. Under TRICARE East, Humana Military authorizes nonemergency inpatient substance use admissions and residential treatment.

Does Humana Cover Detox in Indiana?

Yes. Humana covers detox for PathWays members under Indiana Medicaid, which pays for inpatient detoxification with prior authorization, and TRICARE covers withdrawal management for military families. Humana’s 2026 Indiana handbook also offers a Go365 reward for a follow-up visit within 30 days after leaving medical detox programs, residential care or inpatient treatment.

Does Humana Cover Outpatient Rehab in Indiana?

Yes. Humana’s Indiana handbook says behavioral health services are self-referral with an in-network provider, and a PathWays member can see any in-network doctor for outpatient treatment without a referral. The handbook also lists intensive behavioral health treatment and partial hospitalization programs, which Indiana Medicaid authorizes before they start.

Humana Military handles intensive outpatient programs, partial hospitalization and opioid treatment programs under TRICARE East by assessing them within 24 to 72 hours of starting, rather than authorizing them in advance. TRICARE Prime members who are not on active duty generally need no referral for outpatient mental health visits.

Does Humana cover rehab through TRICARE in Indiana?

Yes, in TRICARE East ZIP codes. TRICARE covers emergency and nonemergency inpatient substance use care, withdrawal management, residential rehabilitation, partial hospitalization, intensive outpatient care, medication-assisted treatment, office-based opioid treatment and opioid treatment programs when medically necessary. It does not cover aversion therapy or unproven treatments.

Does Humana Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Humana PathWays members get the Indiana Medicaid drug benefit through Humana Pharmacy Solutions, and the state benefit pays opioid treatment programs for methadone, buprenorphine and naltrexone. Humana’s Indiana handbook says a drug off the preferred list or filled out of network may cost the member the full price, so addiction medications belong at a network pharmacy.

Does Humana cover methadone?

Yes. Humana Medicare Advantage plan documents and TRICARE list methadone clinic programs as covered through opioid treatment programs, and Indiana Medicaid pays those programs for PathWays members without prior authorization. Medicare covers methadone, buprenorphine and naltrexone with counseling at Medicare-enrolled opioid treatment programs.

Does Humana cover Suboxone?

Yes. Indiana Medicaid covers buprenorphine, including Suboxone, for Humana PathWays members, with new prior authorization criteria in the plans from February 1, 2026, and TRICARE covers office-based opioid treatment. Buprenorphine treatment programs and naltrexone programs appear in the listings below.

Humana Rehab Centers Near Patoka Lake, the White River and the Deam Wilderness

Humana PathWays covers every Indiana county, and Indiana Medicaid sets no regional limit, so an older Hoosier can choose a quieter setting without changing plans. Humana Medicare and TRICARE members can do the same within their networks, using the authorization rules above.

Humana rehab near Patoka Lake

Humana members in Orange, Crawford and Dubois counties live around Patoka Lake, a large reservoir between Jasper and French Lick. Treatment near lakes and programs around Jasper serve PathWays members there, whose care and services coordinator can arrange transport to residential care.

Humana rehab along the White River

Humana members along the White River, which runs from Muncie and Anderson through Indianapolis to Martinsville and Washington, have the widest choice of programs in the state. River-area rehab programs and Muncie treatment options bill PathWays like any other Indiana program.

Humana rehab near the Deam Wilderness

Humana members in Monroe and Brown counties live near the Charles C. Deam Wilderness, the only federal wilderness in Indiana, inside the Hoosier National Forest. Rehabs near wilderness areas and programs near Nashville fall under the same PathWays and Medicare rules as programs in any city.

How Do I Find a Rehab That Accepts Humana in Indiana?

Browse the listings on this page. For each facility you get a phone number, levels of care and accreditation, and the filters work by city, service and insurance. Programs that accept Medicare include many that bill Humana Medicare plans; intake staff can say which Humana plans the program takes.

Humana authorizes residential or inpatient admissions through the plan named on the member’s card. A program found here can file the request, or the member can call the number below and name the program. Humana rehab in Indianapolis and Marion County

Humana PathWays members in Marion County pick Humana over Anthem or UnitedHealthcare by calling the PathWays enrollment line, 877-284-9294, which also handles plan changes. Their care and services coordinator can arrange treatment at Indianapolis addiction treatment centers or Southside Indianapolis programs.

Humana rehab in Evansville and southwest Indiana

Humana members around Evansville who have both Medicare and Medicaid can ask whether PathWays Dual Care, Humana’s fully integrated dual plan, is sold in their county; Indiana is one of three states where Humana lists that design. One plan then handles Medicare and Medicaid authorization for treatment centers in Evansville, with no Medicaid copayments.

Humana rehab in South Bend and Mishawaka

Humana Together in Health, Humana’s institutional special needs plan for Medicare members in long-term care facilities, has been offered in Indiana since 2025, with Longevity Health as clinical partner. Where the plan is sold, residents of St. Joseph County facilities use the same authorization route to reach South Bend rehab programs and Mishawaka treatment options.

Humana rehab in Terre Haute and western Indiana

Humana Military runs TRICARE East, while TriWest runs TRICARE West in some Indiana ZIP codes, so military families in western Indiana should look up their region with TRICARE’s ZIP code tool before admission. East Region families call 800-444-5445 for authorization at downtown Terre Haute programs.

Can I use Humana for rehab in a different state?

Sometimes. Humana’s Indiana handbook allows out-of-network care when no network provider can give the service and lists out-of-state medical services among Indiana State Plan services, but moving out of Indiana ends PathWays enrollment. Humana Medicare members can use network programs in other states where their plan has a network, and TRICARE covers care nationwide under its regional rules.

How much does rehab cost with Humana?

Humana charges PathWays members no copayments for Medicaid-covered services, its 2026 Indiana handbook says, though some members owe Medicare cost sharing or a Medicaid waiver liability. Humana Medicare plans set copays per plan in the Evidence of Coverage, and TRICARE costs depend on the plan and beneficiary group.

Humana coverage is one of several payment routes shown on Indiana listings. Programs also take Medicaid through other plans, self-pay and state funding, and facility pages list them. Rehabs that accept TRICARE and Indiana Medicaid coverage options are listed on separate pages.
