Treatment Centers in Indiana That Accept Indiana Medicaid

Indiana Medicaid, which the Family and Social Services Administration runs as the Indiana Health Coverage Programs, pays for drug and alcohol rehab through the Healthy Indiana Plan, Hoosier Healthwise, Hoosier Care Connect, Indiana PathWays for Aging and Traditional Medicaid. Residential treatment, detox, intensive outpatient care and opioid treatment programs are all covered benefits.

Every listing below gives a direct phone number, the levels of care offered, such as residential, intensive outpatient or methadone, and whether The Joint Commission or CARF accredits the program. Details come from government treatment registers and Indiana licensing records. Narrow the list by city or service, then ask the program which Indiana Medicaid plan it bills.

Does Indiana Medicaid Cover Rehab in Indiana?

Yes. Indiana Medicaid covers drug and alcohol rehab in all 92 counties, and state member pages list mental health care and substance use services under both Package A, the standard benefit, and Package C, the children’s CHIP benefit. A member looking for Indiana drug and alcohol rehabs has the same benefit whichever plan is on the card.

Indiana Medicaid is the state’s Medicaid program, run by the Family and Social Services Administration, whose Division of Mental Health and Addiction certifies addiction providers and designates residential levels of care. Four programs run through contracted health plans, and Traditional Medicaid pays enrolled providers directly. Behavioral health sits inside each plan; there is no separate statewide behavioral health vendor.

Which health plans run Indiana Medicaid in 2026?

Five health plans run Indiana Medicaid in 2026: Anthem, CareSource, Managed Health Services (MHS), UnitedHealthcare and Humana. Anthem is the only one in all four managed care programs. MDwise left HIP and Hoosier Healthwise on January 1, 2026; its members moved to Anthem, CareSource or MHS, and authorizations already granted were honored during the move.

Does Indiana Medicaid Cover Inpatient Rehab in Indiana?

Yes. Indiana Medicaid covers residential treatment programs at ASAM Level 3.1, low intensity, and Level 3.5, high intensity, in facilities the Division of Mental Health and Addiction has designated and Medicaid has enrolled as specialty 836. The state’s Section 1115 waiver also pays for short-term inpatient rehab stays for adults 21 to 64 in facilities with more than 16 beds.

How long will Indiana Medicaid pay for residential rehab?

Indiana Medicaid ties the length of a residential stay to medical need under ASAM criteria. Since February 11, 2019, every Indiana Medicaid plan has authorized at least 14 days of residential treatment before the plan reassesses the stay. The waiver that funds short-term Medicaid residential care runs on temporary extensions through December 31, 2026, pending a December 2024 renewal.

Does Indiana Medicaid need prior authorization for inpatient rehab?

Yes. Indiana Medicaid requires prior authorization before any residential or inpatient substance use admission, and every plan accepts one state form, the Residential/Inpatient Substance Use Disorder Treatment Prior Authorization Request Form, version 6.2 from August 2024. Traditional Medicaid requests go to Acentra Health at 866-725-9991; plan members’ requests go to the plan named on the card.

Does Indiana Medicaid Cover Detox in Indiana?

Yes. Indiana Medicaid covers inpatient detoxification, with admission criteria set in the Indiana Health Coverage Programs behavioral health services module and prior authorization required for the stay. The Division of Mental Health and Addiction certifies detox providers under its regular addiction certification, which requires accreditation, and medical detox that takes Medicaid appears in the listings below.

Does Indiana Medicaid Cover Outpatient Rehab in Indiana?

Yes. Indiana Medicaid covers outpatient counseling, Medicaid intensive outpatient programs and Medicaid partial hospitalization programs. Intensive outpatient treatment runs at least three hours a day, three days a week, with at least 120 minutes of therapy per session, at any age. Both intensive outpatient and partial hospitalization need prior authorization from the plan.

Indiana Medicaid also pays for peer recovery services from a Certified Peer Support Professional, up to 365 hours in a rolling 12 months without prior authorization, and crisis intervention never needs authorization. Members with chronic substance use disorder can qualify as medically frail for HIP State Plan benefits, and telehealth addiction treatment can stand in for many face-to-face visits.

Does Indiana Medicaid Cover Suboxone, Methadone and Other Addiction Medications?

Yes. Indiana Medicaid covers methadone, buprenorphine and naltrexone through opioid treatment programs, and buprenorphine is also paid when a DEA-registered physician, nurse practitioner, physician assistant or community mental health center gives it. Residential programs at Levels 3.1 and 3.5 must keep members on their medications for addiction treatment, billed outside the daily rate.

Does Indiana Medicaid cover methadone?

Yes. Indiana Medicaid covers methadone clinic treatment for members 18 and older who meet ASAM opioid treatment criteria, with no prior authorization. Since July 1, 2023 opioid treatment programs have been paid in weekly bundles, and take-home add-ons can stretch a supply to about a month. The Division of Mental Health and Addiction oversees every opioid treatment program in Indiana.

Does Indiana Medicaid cover Suboxone?

Yes. Indiana Medicaid covers Suboxone and buprenorphine with Medicaid on its preferred drug list. A November 2025 bulletin moved Brixadi, an injectable buprenorphine, to preferred status and set new prior authorization criteria for opioid use disorder drugs from January 1, 2026 in Traditional Medicaid and February 1, 2026 in the plans. Vivitrol shots through Medicaid are covered too.

Indiana Medicaid Rehab Centers Near Lake Michigan, the Hoosier National Forest and the Ohio River

Indiana Medicaid pays for treatment the same way on the Lake Michigan shore, in the hills of the Hoosier National Forest and along the Ohio River, because the state’s plan pages set no regional limit on any program. The setting changes the view, not the benefit: the same plan and authorization rules apply in every county.

Indiana Medicaid rehab near the Lake Michigan beaches

Indiana Medicaid members in Lake, Porter and LaPorte counties live along the state’s only stretch of Lake Michigan shoreline, where the Indiana Dunes run between Gary and Michigan City. Rehab centers near beaches and treatment programs in Michigan City use the same residential prior authorization form as any program downstate.

Indiana Medicaid rehab near the Hoosier National Forest

Indiana Medicaid covers care in the south-central hill counties that hold the Hoosier National Forest, including Lawrence, Orange, Perry and Crawford. Bedford, Paoli and Tell City sit near its edges. Programs near national forests and rehab options in Bedford bill the member’s plan, and PathWays members 60 and over have a care coordinator to arrange services.

Indiana Medicaid rehab along the Ohio River

Indiana Medicaid members along the Ohio River, from Evansville in the southwest to Jeffersonville, Madison and Lawrenceburg in the southeast, often live minutes from Kentucky or Ohio. Rehab centers near rivers on the Indiana bank bill Indiana Medicaid directly, while a program across the water must be enrolled with the Indiana Health Coverage Programs before Traditional Medicaid will pay it.

How Do I Find a Rehab That Accepts Indiana Medicaid in Indiana?

Browse the listings on this page. Phone numbers, levels of care and accreditation appear on each entry, and you can filter by city, service or payment type. One call to the program answers which Indiana Medicaid plans it bills, and rehabs that accept Medicaid in other states are on a separate page.

Indiana Medicaid admission for residential, inpatient, intensive outpatient and partial hospitalization care runs through the member’s health plan, because the plan holds the authorization.
