Starting is a short, carefully timed process. federal health agencies says a patient should stop using opioids for at least 12 to 24 hours and be in early withdrawal before the first dose, because buprenorphine taken with opioids still in the bloodstream can cause acute withdrawal. Who Can Prescribe Buprenorphine Maintenance, and Can It Be Done by Telehealth?

Any practitioner with a DEA registration that includes Schedule III authority may prescribe buprenorphine where state law permits. federal health agencies says section 1262 of the Consolidated Appropriations Act of 2023 removed the special waiver and that there are no longer limits on how many patients a practitioner may treat. The same Act added a one-time 8-hour training requirement for most DEA registrants.

NIDA says buprenorphine can be prescribed by many doctors, nurse practitioners and physician assistants, with no need to visit a special treatment clinic. That is the structural difference from a methadone clinic, where dosing is dispensed on site. NIDA also notes that methadone and buprenorphine can be equally effective in helping individuals reduce opioid use, and that methadone may help some individuals stay in treatment longer.
