Continuing Care Programs After Treatment

Treatment intensity steps down rather than stopping outright. Continuing care is the scheduled contact that follows an inpatient stay or an intensive outpatient block: weekly groups, individual sessions, check-in calls or medication reviews spread across months instead of weeks, at a lighter but steady pace.

What counts as continuing care differs widely between the providers below. Some run formal alumni programming with set session lengths; others offer a monthly call. Ask how often contact happens, for how many months, and whether it costs anything extra before deciding.

Why Trust RehabSeekers.com?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

What is continuing care in addiction treatment?

Continuing care is the lower-intensity treatment that follows a more intensive phase such as residential care or an intensive outpatient program. NIAAA-published research describes it as a step-down period meant to solidify gains from the first phase and to keep a return to use from escalating into another acute episode.

It can also mean longer-term management delivered through primary care or regular checkups over extended periods rather than a fixed block of sessions.

Aftercare and Continuing Carehttps://rehabseekers.com/programs/aftercare-and-continuing/
What is the difference between aftercare and continuing care?

In the original step-down model the two terms mean the same thing: lower-intensity treatment following residential care or an intensive outpatient program. Continuing care carries a second meaning as well, covering disease-management approaches delivered over long periods through primary care or scheduled checkups.

NIDA frames addiction treatment as managing a chronic condition rather than curing it, which is why ongoing contact matters after the intensive phase ends.

What happens after you leave rehab?

After intensive treatment, NIDA says the provider should ensure a transition to less intensive continuing care that supports and monitors ongoing recovery. In practice that means outpatient or IOP sessions, medication where it applies, a recovery residence, mutual help meetings and peer recovery support.

Ask the discharging program for a written plan naming the next appointment date, the prescriber and the housing arrangement before you leave.

Continuing care is the lower-intensity treatment and support that follows an intensive phase such as residential care or an intensive outpatient block. It usually means weekly groups, individual sessions, medication reviews, check-in calls and a link to recovery housing or a mutual help group, spread across months rather than weeks.

NIDA states that after a course of intensive treatment the provider should ensure a transition to less intensive continuing care that supports and monitors the person in ongoing recovery. The same guidance notes that addiction treatment is not a cure but a way of managing the condition, in the way treatment manages heart disease or asthma, and that patients typically need long-term or repeated episodes of care.

This guide covers what continuing care is, what a typical schedule looks like, how long it runs, what it costs and who pays, how recovery housing and mutual help fit in, and how to find a program that runs this phase rather than handing it off.
