I think it's a great place they take their time not just rush you through and if you need to talk they listen only good things to say

7/1/26 - The nurse was very rude!

Thanks for the kindness.. The entire staff at my local Cleanslate, as well as all employees of Cleanslate centers across the nation. I have had nothing but genuine care and support over the years. Thanks Lafayette 😵‍💫… as of 3/30=my report has changed in the worst way I had surgery and was assured I would be able to return to clean slate in Lafayette. But that was not the case due to having pain meds filled and meeting with a new in house provider that completely erased my whole 1st report . Other than the girls up front. This new woman is the exact opposite of what clean slate stands for. Was rude told me to get out of her office because 10 weeks after surgery and to my surprise the pain management program I was in was far from communicable. And I ran out of my post surgery pain meds quickly . While in withdrawal and seeking to come back to c.s. I was treated like a prisionernand new in house prescriber was the judge and jury. She would not help me back on to Suboxone and told me to get out . I asked her , to clarify that they are about harm reduction and if anything bad were to happen because I have been on this med for 7 years she would take full blame she rudely rushed me away telling me get lost basically. Mthis is not the clean slate it was for many years . I will be filling a formal complaint . I was giving no explanation other than I had pain meds filled this month .. duh!!! I had a major surgery and found out that that was not the route I was going to continue. This person needs to be held responsible and will be for any lapse in my recovery. I had care coordination and other providers on telemed agree that it was no problem . She is the worst thing that could have happened to clean slate causing major disruption to my recovery and the facts I was givin prior to surgery. Iam frantically searching for relief and they don't care the outcome . Horrible abuse of power. When it comes to harm reduction.

Cleanslate in Lafayette is a great place the staff is amazing, they truly made me feel at home. They get me in and out and offer so many opportunities for help with other things besides your medicine. I recommend this… place to anyone this place helped save my life!

Scheduling is difficult but they are really out to help people. People I know swear by them.

I have used clean slate for the past 4 months now. Every time I go in everyone is very friendly and helpful. They genuinely care about your recovery. But do not reschedule your appointment because it is almost… impossible to reschedule an appointment. I got called in for work and had to miss my appointment.I stayed on the phone with the lady trying to reschedule my appointment for over an hour.Trying to get a new appointment. But really?That's the only complaint that I have about pain slate.

Oh how I wish I could give zero stars! Do yourself a favor & only read the recent reviews. This place has gone from one that treated their patients with care & dignity to one that some providers forget to call in… meds...sometimes for days after your appointment!

If I could give ZERO stars, I absolutely would!!! Im a new patient here and im not impressed!!!! They let their patients go without meds and don't send in enough of the medication to get them through until their next… appointment! Even if you call and say you're out, they will not send anything in until the appointment! This is highly unprofessional and quite scary considering they work with addicts!

DO NOT I REPEAT DO NOT go to clean slate for your suboxone. Greg does NOT know how to manage this center their Dr's and employees change every month. Not to mention their extremely rude will cancel your appointment less… than an hour before your supposed to be there just do yourself a favor go to Meridian or another reputable Dr because this definitely does not operate as a Dr's office should.

This place has truly saved me and countless others. I'm always in and out within 15 minutes tops which is insane. The workers are all super friendly and kind and I've gotten to know them quite well. My trip today was… with a lovely new doctor and she was very kind and professional as well. If your struggling with addiction to anything. I highly recommend clean slate. After struggling with trying to find some place that was more than just trying to make money from you or trying to shove different types of meds at you to make more money. This place actually cares about your health and overall well being!

I absolutely love the people there they actually care and not trying to just push you to take meds

The first visit and the few weekly visits get old if you live far but after clean dts you get monthly so now it’s like 15 minutes for the month.

The staff are polite and knowledgeable. Clean Slate is saving my life.

I love this place Courtney is the best. I'm in and out within 15mins. They are always very nice and professional

They allow you to test positive for marijuana but if you have a slight smell of it on you while going to your appointment you will be denied. They do not care if you go without your medication. They do not offer any… type of therapy or actual treatment. They just push the medications on you with no talk of getting off of them. You don’t see an actual doctor. There is no type of treatment or recovery happening here. But hey I’m getting those suboxone they have me addicted to.

Youve given me hope and i needed it. Thank you. I believe i can lick this habit and thats my plan. Thank you Clean Slate! If you succeed in helping me succeed, i will be your biggest spokesman.

Process was very fast, and great customer service. Workers are very nice

went for my 1st appointment today the whole staff was supportive & more than willing to help. Overall great atmosphere conducive to aiding my recovery. I am eager to continue my journey towards sobriety with them… despite my anxiety of switching from a video Dr. Through a app previously.
