Highmark Blue Cross Blue Shield of Western New York is the Blue plan for eight counties at the western end of New York State, centered on Buffalo and Erie County. It is a trade name rather than a company: the insurer behind it is Highmark Western and Northeastern New York Inc., which was HealthNow New York Inc. and, before that, BlueCross BlueShield of Western New York. The affiliation with Highmark completed on March 1, 2021, and the brand changed with it.

The distinction that decides this page is geographic and regulatory at once. New York carries four separate Blue brands that share no network and no rulebook, and the Highmark enterprise itself applies different substance use criteria in New York from the ones it uses in Pennsylvania, Delaware and West Virginia. A New York request is judged against the state’s own level-of-care tool, and New York members get a long window at the start of an inpatient stay in which no advance approval is asked for at all.

What Is Highmark BCBS of Western New York and Who Runs It?

It is the trade name used by Highmark Western and Northeastern New York Inc. across eight western New York counties. The company belongs to Highmark Inc. of Pittsburgh, an independent licensee of the Blue Cross Blue Shield Association, whose own parent is the nonprofit enterprise Highmark Health. Behavioral health review is handled in house.

Highmark Health also owns Allegheny Health Network, enGen, United Concordia Dental and HM Insurance Group, so the insurer is one arm of a larger group rather than a standalone carrier. Highmark Inc. holds Blue licenses as Highmark Blue Cross Blue Shield in western and northeastern Pennsylvania, Delaware, West Virginia and western New York, and as Highmark Blue Shield in central and southeastern Pennsylvania and northeastern New York. After the Blue Cross and Blue Shield of Kansas City affiliation was approved in March 2026, Highmark and its Blue affiliates report nearly 8 million members across Pennsylvania, Delaware, New York, West Virginia, Missouri and Kansas. Highmark’s own Behavioral Health Services department runs utilization review, with the New York line on 844-946-6264.

Which Counties and Plan Lines Does It Cover?

Eight counties in western New York under this brand, and 13 more in the northeast under the sister Blue Shield name, for a 21-county region in total. Nothing outside the Highmark footprint is sold directly, so a member living elsewhere in New York will hold a different Blue plan.

Across the company’s New York lines the state plan directory names Albany, Allegany, Cattaraugus, Chautauqua, Clinton, Columbia, Erie, Essex, Fulton, Genesee, Greene, Montgomery, Niagara, Orleans, Rensselaer, Saratoga, Schenectady, Warren, Washington and Wyoming. Employer coverage is the line confirmed for the western New York brand, and the plan is also one of the health maintenance options offered to public employees and retirees through the New York State Health Insurance Program, where it is listed for 2026 alongside the statewide Empire Plan and the other participating plans.

The Medicaid and Essential Plan Lines

The same legal entity holds mainstream New York Medicaid managed care, a Health and Recovery Plan line and Child Health Plus, reached on (866) 231-0847. The Medicaid counties named are Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans and Wyoming. Mainstream Medicaid enrollment stood at 44,927 in February 2026 and 45,990 in May 2026, the smallest book among the New York mainstream plans. The same number serves Essential Plan members.

What the Member ID Prefix Does

The three characters at the start of a member ID identify the Blue company and route the claim, which is how a program in another state knows where to send a request. Under the inter-plan arrangement the local Blue company prices the claim at its contracted rates while the home plan applies the benefits and pays, so cost sharing stays the home plan’s: a $25 copay in Buffalo is a $25 copay elsewhere. Does Highmark BCBS of Western New York Cover Drug and Alcohol Rehab?

Yes. Highmark’s provider manual names inpatient mental health, inpatient withdrawal management, inpatient substance use rehabilitation, residential treatment, partial hospitalization and intensive outpatient as the behavioral health levels it reviews. In New York those requests are judged against the state’s LOCADTR 3.0 level-of-care tool rather than the criteria Highmark uses in its other states.

That choice of tool is not cosmetic. Pennsylvania, Delaware and West Virginia requests are measured against the current ASAM Criteria, while other behavioral health care across the enterprise is measured against a separate set of care guidelines. New York is the one Highmark state where the state addiction services agency’s own instrument decides whether inpatient treatment is the right setting, so the documentation an admissions team prepares in Buffalo is not the documentation it would prepare in Pittsburgh.

Emergency care needs no authorization unless it turns into an inpatient admission, which is the usual route by which a medical detox admission reaches the plan’s attention after the fact rather than before it.

Does Highmark BCBS of Western New York Need Prior Authorization for Rehab?

Yes, for inpatient behavioral health, including withdrawal management and inpatient substance use rehabilitation, on most products. Partial hospitalization was added to Highmark’s prior authorization list on March 1, 2026, together with applied behavior analysis and transcranial magnetic stimulation codes, and the change does not reach the federal employee plan or Medicare products.

The same March 2026 addition brought intensive outpatient treatment onto the list, so on commercial coverage that step now needs a request where previously it did not. A member working from older guidance can settle the point on the New York behavioral health line.

The First 28 Days of Inpatient Care

In New York, Highmark asks for no preauthorization for the first 28 days of inpatient substance use or opioid use disorder treatment, or the first 14 days of inpatient psychiatric care for a member under 18, where the care is given at an in-network site licensed by the state addiction services agency. The admitting program has to notify the plan and send the initial treatment plan within two business days.

How Long a Decision Takes

A routine behavioral health authorization request gets a decision within two business days across the Highmark states, and within three business days in New York. A treating clinician who disagrees with a determination can ask for a peer-to-peer discussion on the line Highmark publishes for that purpose, which is usually faster than a written appeal while a member is still admitted.

Two New York rules pull in opposite directions. The 28-day window removes the advance request at the top of the care ladder, where delay does the most damage. The March 2026 change added one lower down, at partial hospitalization and intensive outpatient. A member stepping down from an inpatient stay in western New York may therefore need an authorization for the next, less intensive stage that was not needed for the first.

How Much Does Rehab Cost With Highmark BCBS of Western New York?

$10 per visit for outpatient mental health and drug and alcohol rehabilitation on the plan’s public employee health maintenance option, with no visit limit and no copayment for an inpatient stay, according to the 2026 plan comparison for that program. Employer designs outside it set their own deductibles and coinsurance.

One published example shows the shape a non-public-employee design can take, though it belongs to the sister brand rather than this one. The Highmark Blue Shield of Northeastern New York point-of-service summary dated July 1, 2025 sets inpatient withdrawal management and rehabilitation at a $250 copay per admission in network, with 20% coinsurance after the deductible out of network, and outpatient substance use visits at $25.
