Align Senior Care Florida, Inc. is the Florida plan sponsor behind Align Senior Care, a small family of Medicare Advantage plans built for residents of nursing facilities and senior living communities. In Florida it holds contract H9917 and sells two options: Senior Care, an institutional special needs plan, and Premier Care, a second institutional special needs plan with a different cost structure. Both include Part D drug coverage, and both are health maintenance organization plans, so the network matters.

The distinction that decides this page is who may hold the card. An institutional special needs plan is not sold to the general Medicare market: an enrollee needs Parts A and B, has to live in the service area, and must either have lived in a participating nursing facility for more than ninety days or live in assisted or independent living while meeting an institutional level of care. That eligibility rule shapes where treatment happens as much as the benefit schedule does.

This guide covers which substance use levels of care appear on the plan’s own lists, what needs authorization before it starts and what does not, who owns and administers the plan, which Florida counties it serves and who can join, what each of the two 2026 options charges, which similarly named companies are unrelated, and how to locate a program that takes it.
