Common questions

Answered from what is listed here.

What is Align Senior Care?

Align Senior Care is a family of Medicare Advantage plans, mostly Institutional Special Needs Plans for individuals living in nursing facilities or senior living communities. It operates in California, Florida, Michigan and Iowa, and each plan includes Part D drug coverage alongside its medical benefits.

The plans are fully owned and operated by Curana Health, formed in May 2022 when AllyAlign Health combined with Provider Health Services and Elite Patient Care.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Does Align Senior Care cover rehab?

Yes, within Medicare rules. The plans list inpatient psychiatric hospital care, partial hospitalization, intensive outpatient, opioid treatment program services and outpatient substance use services as covered benefits. Everything on that list except intensive outpatient needs prior authorization, and care must come from network providers.

No separate residential, non-hospital rehab benefit appears in the 2026 plan documents, so ask member services how a specific level of care would be paid.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Does Align Senior Care cover inpatient rehab?

Yes, inpatient mental health care in a psychiatric hospital is covered on every plan with prior authorization. Florida's Senior Care plan uses Original Medicare cost sharing: a $1,736 deductible per stay, $0 for days 1 to 60 and $434 a day for days 61 to 90.

Florida's Premier Care plan instead charges $100 a day for days 1 through 6 and $0 from day 7 to day 90 in 2026.

Align Senior Care is a small family of Medicare Advantage plans written for residents of nursing facilities and senior living communities in California, Florida, Michigan and Iowa, and it is fully owned and operated by Curana Health. Addiction treatment under these plans follows Medicare’s rules, inside an HMO network, for an older adult who already receives care where they live.

Almost every product is an Institutional Special Needs Plan, or I-SNP. Enrollment turns on Medicare Parts A and B, residence in the service area, and either more than 90 days in a participating nursing facility or an institutional level of care while living in assisted or independent living. The one exception found is Advantage Care (HMO) in California, a non-SNP plan with standard Medicare eligibility.

This guide sets out which levels of substance use care the 2026 plans list, where prior authorization applies and where it does not, what a member pays under each named plan, how medication is handled through the Part D benefit, and how to work out whether a program sits inside the network.
