Crystal Run Healthcare is a multispecialty physician group in New York’s Hudson Valley, headquartered at 155 Crystal Run Road in Middletown and owned by Optum since February 22, 2023. It delivers care rather than paying for it, so no insurance card carries its name and no benefit department there decides whether a detox admission or a residential stay is paid.

The name still turns up on intake paperwork because Crystal Run did sell coverage. Crystal Run Health Plans launched in 2015 for anyone who lived or worked in Orange and Sullivan counties, and the two insurance entities behind that brand left the New York market in stages, finishing on November 30, 2020. Anyone handed a treatment questionnaire that lists Crystal Run Healthcare as a payer is working from an old record.

This guide sets out what the practice does and does not pay for, which insurer actually decides each level of addiction care, where prior authorization comes from, what treatment costs without coverage, how medication is handled, and how to find a New York program that takes the plan on the card.

Does Crystal Run Healthcare Cover Drug and Alcohol Rehab?

No. Crystal Run Healthcare is a medical group, not a payer, so it issues no benefits and covers no rehab. Payment for detox, residential treatment, partial hospitalization, intensive outpatient care or outpatient counseling comes from the insurer named on the card, which for Crystal Run patients is commonly UnitedHealthcare or Oxford, Aetna, Anthem BCBS, EmblemHealth, MVP, Medicare or Medicaid.

The practice does treat addiction clinically. Its Behavioral Health and Psychiatry department staffs psychiatrists, psychologists and licensed clinical social workers, sees patients from age 3 through older adulthood, and lists substance use disorder among the conditions treated alongside depression, anxiety, bipolar disorder, ADHD and psychotic disorders. Psychiatry there offers medication management and psychotherapy. Nothing published by the group describes a detox unit, a residential program or an opioid treatment program, so a higher level of care is arranged elsewhere.

Read the card, not the letterhead. Crystal Run’s published list of accepted plans includes Aetna, Anthem BCBS, CDPHP commercial, Cigna, EmblemHealth, GHI and HIP, the Empire Plan, Geisinger, Humana, MagnaCare, MultiPlan and PHCS, MVP, Northwell Direct, Oxford, UnitedHealthcare, UMR, Medicare, TRICARE, CHAMPVA and the VA Community Care Network, each with exclusions. The insurer in that list, not the practice, answers rehab questions.

What Happened to Crystal Run Health Plans?

They closed. Crystal Run announced the wind-down of its health plan operations on March 29, 2019. Medicaid Managed Care and the Essential Plan ended September 1, 2019, the individual HMO ended December 31, 2019, and the small group HMO ended November 30, 2020, the same date Crystal Run Health Insurance Company withdrew from the New York market.

At year-end 2018 the HMO held 3,584 members, 2,499 in Orange County and 1,085 in Sullivan, split across Medicaid, the Essential Plan and small group lines, while the insurance company counted 2,517 EPO and PPO enrollees. The state examination report attributes the exit to limited enrollment growth at a 3.0% market share, limited capital against operating losses driven by the federal risk adjustment program, and expected Medicaid losses. No source records where members moved, so a former member should look at the current card and work from there.

Who Decides Coverage at Each Level of Care?

The plan on the card, applying its own medical necessity criteria. The questions below are the ones to put to that plan’s behavioral health line rather than to a Crystal Run office, because the practice bills its own visits and has no say in what a treatment program is paid.

Medical Detox

Withdrawal management sits outside anything Crystal Run runs, so medical detox is arranged with a licensed program and authorized by the insurer. Residential and Inpatient

Residential admissions almost always need authorization before the date and concurrent review afterwards, and the length of an authorized stay comes from the plan. Ask the admissions team to open that request and to state in writing what has been authorized before anyone travels.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient programs are the usual step down and the usual starting point for anyone medically stable. Outpatient counseling and psychiatric medication management are the levels Crystal Run itself can deliver, billed to the plan as ordinary behavioral health visits.

Does Crystal Run Healthcare Need Prior Authorization for Rehab?

No. Crystal Run sets no prior authorization rules because it pays no claims. Authorization for detox, residential care, PHP or IOP is requested from the insurer on the card, and the rules differ by product: a commercial HMO, an employer PPO, Medicare and a Medicaid managed care plan each run their own review.

No published material from the practice describes a referral route into addiction treatment, and no record survives of how the former Crystal Run Health Plans handled authorization, so there is nothing historical to fall back on. How Much Does Rehab Cost With Crystal Run Healthcare?

Crystal Run charges nothing toward a rehab stay because it neither bills for one nor pays for one. The share owed comes from the insurer on the card. No cost-sharing figures were ever published for the defunct Crystal Run Health Plans, and those products ended by November 30, 2020, so there is no legacy copay schedule to work from.

For New York Medicaid members there is a further wrinkle: the practice has accepted no new Medicaid patients since May 27, 2021, and several Medicaid, HARP and Essential Plan products are excluded from its accepted list.

Does Crystal Run Healthcare Cover Suboxone, Methadone and Other Addiction Medication?

No, because a physician group has no formulary. Coverage for buprenorphine, naltrexone or methadone comes from the pharmacy benefit attached to the plan. No published Crystal Run page names those medications; psychiatry there lists substance use disorder among conditions treated and offers medication management, so whether prescribing is available is a question for the department.

Where the Prescription Is Filled

A prescription written in an office is filled under the plan’s drug list, with its own tiers and authorization steps, while medication given inside a licensed opioid treatment program is billed as a program service. Patients are also pointed to Optum Perks, a free prescription discount program, which is not insurance and does not count toward a deductible. Programs listed for medication assisted treatment and buprenorphine can describe which products they dispense on site.

Behavioral Health Offices and Video Visits

Behavioral health is reached in Middletown on 845.703.6999, Monroe on 845.615.6999, Newburgh on 845.725.0100, Rock Hill on 845.794.6999 and West Nyack on 845.348.1100. Video visits run for primary care and more than 20 specialty services, availability varies by specialty and reason for the visit, and the patient has to be physically in the provider’s state during the appointment. The practice does not state whether behavioral health is among the specialties offered by video.

None, strictly speaking: network contracts belong to insurers, and Crystal Run holds none to offer. How Do I Find a Rehab That Takes Crystal Run Healthcare?

Start with the insurer on the card instead of the Crystal Run name. Call the behavioral health number printed on the back, ask for addiction treatment programs in the Hudson Valley at the level of care being considered, and give that list to the admissions team. A program that says it takes Crystal Run is usually describing a plan that ended in 2020 or the medical group itself.

Network details shift, which is a reason to ask rather than assume. CDPHP added the practice to its network in Orange, Dutchess and Ulster counties on February 5, 2024; the practice stated on May 14, 2026 that MVP remains accepted, excluding HQNet, Vermont and New Hampshire products; and Cigna Medicare Advantage and Dual Special Needs plans fall out of network from January 31, 2026. Is Crystal Run Healthcare the Same as Optum or UnitedHealthcare?

Not quite. Crystal Run is part of Optum, the care delivery arm of UnitedHealth Group, inside the Optum Tri-State Region, and the practice describes itself as physician-led with Optum handling non-clinical administration. UnitedHealthcare is the separate insurance arm, and the group accepts many rival insurers’ plans. In June 2026 Optum said it was weighing the sale of some orthopedic, general surgery and urology services tied to Optum Medical Care and Crystal Run to large New York health systems, while keeping primary care, pediatrics and medical specialties; no deal had been announced.

One more name to keep separate: Crystal Run Village, Inc. is an unrelated charitable organization that happens to share the Middletown address book. It is not part of Crystal Run Healthcare and is not a payer for treatment.
