The same split applies elsewhere, including the Texas Medicaid and CHIP children’s dental programs and the Medi-Cal dental managed care plan run in Sacramento and Los Angeles counties by California Dental Network, which does business as DentaQuest.

Two cards, two benefits. The DentaQuest card opens the dental network. The medical card, whether that is a Medicaid health plan, a Medicare Advantage plan, a marketplace policy or an employer plan, is the one that authorizes and pays for addiction treatment. In Medicare Advantage, DentaQuest administers the supplemental dental benefit for the health plan rather than selling a plan of its own.

Which Plan Pays for Each Level of Care?

The medical plan on the other card pays for all of it, and the questions below belong to that plan’s benefits team rather than to DentaQuest. What follows is what to ask about at each level, since the dental plan has no authorization role, no cost share and no network for any of it.

Medical Detox

Ask the medical plan whether inpatient medical detox requires authorization before admission and what the daily or per-admission cost share is. For a Medicaid dental member in Florida, hospital care is assigned to the Medicaid health plan, so withdrawal management is that plan’s decision from the first phone call.

Residential and Inpatient

A residential admission is reviewed against the medical plan’s own criteria. DentaQuest publishes no level-of-care criteria for substance use treatment, because it has no such benefit, so a program quoting dental authorization rules at an admission is quoting the wrong document.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient sit with the medical plan too, as does weekly counseling. DentaQuest’s only virtual service is teledentistry, offered as a supplement to its government dental network, so behavioral telehealth is a question for the medical plan.

Does DentaQuest Need Prior Authorization for Rehab?

No. Prior authorization at DentaQuest applies to dental procedures only. In Florida that list covers orthodontics, some debridement, periodontal scaling, crowns and root canals for seniors and certain other specialized dental work, and the Tennessee handbook says some dental services need approval before treatment. Rehab authorization is issued by the medical plan.

DentaQuest holds NCQA certification for credentialing, recredentialing and utilization management and is URAC accredited, all of it on the dental side. There is no DentaQuest medical necessity policy for a residential stay to appeal, and no behavioral health vendor behind the dental plan, because there is no behavioral benefit to vend.

Who Authorizes Treatment If the Dental Plan Does Not?

Whichever health plan or state program carries the medical benefit. In New York, DentaQuest administers dental for EmblemHealth, Fidelis Care, Healthfirst, MetroPlus, MVP and Hamaspik Choice MLTC, and each of those plans runs its own behavioral health authorization. In Texas it administers dental for Medicaid products from Aetna, Blue Cross Blue Shield, Driscoll, Molina, Superior, Community First and Wellpoint. In Arizona the dental sits behind Banner University Family Care and Molina Complete Care.

Have this ready before calling a program. The medical plan name and ID from the health card, the state Medicaid or CHIP ID where that applies, and the plan’s behavioral health phone number. The DentaQuest number on the dental card, general member services 888-278-7310, answers dental questions and cannot authorize a treatment admission.

How Much Does Rehab Cost With DentaQuest?

The dental plan contributes nothing, so the cost of treatment is whatever the medical plan’s deductible, copays and coinsurance come to, or the full billed rate where there is no medical coverage at all. DentaQuest’s published cost shares are dental figures and apply only to dental care.

On the personal dental plans, the Basic plan and both Comprehensive plans carry a $50 per member deductible with a $150 family maximum, the Plus plan carries $100 per member and $300 per family, and the Preventive plan has none. Waiting periods run six months for basic restorative work on the Basic plan and eighteen months for major work on Plus. Florida Medicaid dental charges $3.00 per day for non-emergency visits at federally qualified health centers, with most covered dental services at no charge. None of those figures moves the price of a treatment admission.
