Rehabs That Take HealthEOS

HealthEOS is a preferred provider network named by treatment centers in Utah and Wisconsin. Plans that use the HealthEOS network can include coverage for drug and alcohol treatment, with benefits varying by plan, network and level of care. A network sets which providers are in network, while your plan sets the actual benefits. Some sites list it as HealthEOS Plus.

Centers listed here name HealthEOS on their own websites. Networks can change over time, so check the current provider list with your plan rather than relying on older materials. Before admission, ask the facility and your plan about network status, prior authorization and costs.

Why Trust RehabSeekers.com?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

Is HealthEOS insurance?

No, HealthEOS is a regional PPO provider network in Wisconsin, operated by Claritev, formerly MultiPlan. The network owner states it does not offer insurance, administer benefit plans or pay claims. The insurer, employer plan or third-party administrator named on your ID card sets and pays benefits.

A HealthEOS logo on the card shows that the plan rents that network for provider access and discounts; benefits questions go to the member number printed on the card.

Private Insurance Acceptedhttps://rehabseekers.com/insurance-accepted/private-insurance-accepted/
Does HealthEOS cover rehab?

No, the network does not decide coverage. Whether drug and alcohol treatment is paid for comes from the health plan behind your card: an insurer, an employer plan or a third-party administrator. HealthEOS only sets which providers are in network and the discount they accept.

Services from contracted providers are paid at the plan's in-network benefit level, which is how plans such as GHC-SCW's Connections PPO use the network in Wisconsin.

Private Insurance Acceptedhttps://rehabseekers.com/insurance-accepted/private-insurance-accepted/
Does HealthEOS cover detox?

No, detox coverage comes from your plan rather than from the network. HealthEOS is rented by health plans and has no benefits of its own, so your insurer or employer plan decides whether medical detox is covered, in which setting and with what cost sharing.

Ask the member services number on your ID card which detox settings are covered and whether an authorization has to be obtained first.

HealthEOS is a regional PPO provider network in Wisconsin, operated by Claritev, the company known as MultiPlan until it rebranded on February 17, 2025. It is not an insurance company. The network owner states plainly that it does not offer insurance, does not administer benefit plans and does not pay claims, so the HealthEOS logo on a card answers one question only: which doctors, hospitals and treatment programs are contracted at in-network rates.

Everything else is decided by the plan named elsewhere on that same card. Group Health Cooperative of South Central Wisconsin (GHC-SCW) uses HealthEOS as the primary network for its Connections PPO for Wisconsin residents, while Quartz lists its PPO network as PHCS/MultiPlan/Health EOS. Two separate carriers, one shared network, two different sets of benefit rules, deductibles and authorization desks.

The guide below sets out what a network can and cannot do for addiction treatment, which desk handles prior authorization, what a stay costs and who sets the share of it, how medication coverage works when the network has no formulary, and how to tell whether a program is contracted with HealthEOS before anyone discusses admission dates.
