Rehabs That Take Special Needs Basic Care

Special Needs Basic Care (SNBC) is a Minnesota Medical Assistance program for adults with disabilities, delivered through managed care health plans. Members can have coverage for drug and alcohol treatment, with services depending on the health plan, provider network and the level of care that a comprehensive assessment supports. SNBC plans also coordinate care with Medicare where members have it.

Listed Minnesota centers name Special Needs Basic Care on their own websites. Contact the center and your SNBC health plan before admission to check network status, prior authorization and any cost. Ask as well whether a referral from your care coordinator is required.

Why Trust RehabSeekers.com?

RehabSeekers is an independent behavioral health directory covering drug and alcohol addiction and mental health treatment providers.

Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

What is Special Needs BasicCare?

Special Needs BasicCare (SNBC) is a voluntary Minnesota Department of Human Services managed care program inside Medical Assistance, the state's Medicaid program. It serves adults ages 18 through 64 who have a certified disability, and coverage is delivered through contracted health plans rather than by the state directly.

Enrollment is optional: an eligible adult can decline SNBC and keep Medical Assistance fee-for-service, often called straight MA, with the same underlying benefits.

State Insurance Rehabhttps://rehabseekers.com/insurance-accepted/state-insurance/
Who is eligible for Special Needs BasicCare?

Eligibility is limited to adults ages 18 through 64 who are enrolled in Minnesota Medical Assistance and certified disabled by the Social Security Administration or the State Medical Review Team. Anyone who also has Medicare must be enrolled in both Part A and Part B to join SNBC.

Minnesota DHS runs related programs for other groups, including Minnesota Senior Health Options for seniors and Medical Assistance for Employed Persons with Disabilities.

Medicaid Rehabhttps://rehabseekers.com/insurance-accepted/medicaid/
Does Special Needs BasicCare cover rehab?

Yes. SNBC members keep every Medical Assistance benefit, so substance use disorder treatment covered by Minnesota Medicaid is available through the SNBC health plan. The DHS program page does not spell out levels of care, so covered services follow the health plan's rules and a comprehensive assessment.

Call the member line for your plan before admission to check network status, authorization and whether your care coordinator must make the referral.

Special Needs BasicCare (SNBC) is a Minnesota Medical Assistance managed care program for adults ages 18 through 64 who have a certified disability, delivered through health plans under contract to the Minnesota Department of Human Services. It is voluntary, it costs nothing extra, and members keep every Minnesota Medical Assistance benefit they already hold, including treatment for substance use.

SNBC is not an insurance company, and that matters when calling an admissions team. The benefit set belongs to Medical Assistance; the network, the authorization rules and the care coordinator all belong to the health plan the member enrolled in. In 2025 those plans were Medica, UCare, PrimeWest Health, South Country Health Alliance and Hennepin Health, with HealthPartners until April 1 of that year. The name on the member card is the name an admissions office needs to hear.

This guide covers what SNBC pays for across the levels of addiction care, who issues prior authorization, what a member pays, how medication is handled under integrated and non-integrated enrollment, and how to work out which Minnesota treatment programs hold a contract with the right plan.
