I’ve been going to Clean Slate for outpatient addiction treatment and I couldn’t be happier with the care I receive. They prescribed Suboxone to help manage my opiate withdrawals, which has made a huge difference in how… I feel day-to-day. I’m also on Wellbutrin through them for depression, and the combination has really helped me stay on track. My doctor, Jen, is absolutely awesome. She’s knowledgeable, caring, and genuinely listens. Appointments are efficient—I’m usually in and out pretty quickly, which is perfect for my schedule. The location is super convenient too, and overall this place has given me life-changing support. If you’re looking for compassionate, effective outpatient addiction treatment, I highly recommend Clean Slate. They’ve been a real lifeline for me.

highly recommend CleanSlate Opioid Treatment Center to anyone seeking help with recovery. The entire team has been supportive and professional, but Kimberly and Joanna truly stand out. They are compassionate,… knowledgeable, and always make me feel comfortable and respected. I also want to recognize Joanne, who is honestly one of the best phlebotomists I’ve ever had. I almost always need ultrasound guidance to have blood drawn because my veins are so difficult, but Joanne was able to get it done in just two sticks. Her skill and confidence made a stressful situation much easier. If you’re looking for a treatment center that genuinely cares about its patients and provides excellent care, CleanSlate is the place to go.

The worst experience I have had so far in recovery. Jen changed my prescription and made me sick. Told me what I was saying “didn’t track” or in other words that I was lying. The most judgmental, condescending treatment… I have experienced so far. Jen clearly feels superior and in a position to judge which is unfortunate. There is nothing more difficult when trying to make this change in your life. I imagine she is either burnt out or in the wrong industry all together.

I went in there awhile back to inquire about getting treatment and the reception staff were very nice and encouraging and they set me up an appointment with the doctor on a future date !!! When I went back for the… appointment and met with the doctor it was like I was in a completely different place, everything I was told by the receptionist people was being disputed and or denied by the doctor and he was very discouraging and basically said we don’t think that you would get the assistance you want here !!! So I realized afterwards that he must have been watching from his office on his security cameras 🎥 and listening while I was there the first time and I guess there was something that he didn’t like about what I asked or what I said about previous clinics, basically he came across like a Facist type criminal minded individual !!! And that’s my honest opinion about the doctor, not anybody else that I spoke with at this place !!!

I have been here for a long time. What keeps me here is the great doctors and my favorite nurse of all, Ms.Kim! She is the best. Makes me feel like not just a number. I have had some bumps in the road and Everytime… they're there to help. If you're looking to get and stay clean, I am 7 years sober because of these guys. I would've lost my life. I truly believe they're my heros. I thought I could do it on my own, I found out it isn't so easy. They made it so much better for me. Love all you guys. And thank you for the years of support and love

I have been coming to the center for Suboxone treatment for over 6 years. I credit it with saving my life. The staff at the front and the Doctors are all amazing. It does help in the trying times of getting sober that… you can count on that team working with you to get you better. Non stop praises for this location of Clean Slate on 40th St

Brought my Grandson here to help get off fentanyl. It's been a God send. The girl's at the front desk are so nice and helpful, very considerate what we are going through. The providers he sees are great, they listen to… him. He has really turned his life around. We went to two other places before this one. This place has really worked out.

Been coming here 6 months and it's amazing what these place will do for you if you have the will to change .Kim and joanne at the front desk are great and always helpful. Been trying to get approved for my hep -c cure… for over 10 years and finally clean slate got it approved for me. Thank you guys so much.. over 3 years sober now

A shout out to Kim and doc. They have been essential with honestly My overall mental health along with some behavioral health also they've been here every time I've come in I can't tell you but it's been months so big… shout out to Kim and doc. Kim can prefer you to doc's full name he's the best. I met a couple new people Joanna and Amy they also been helpful and this has worked for me in my life being in my forties and never being in a situation like I was a much appreciate good customer service and good people. Those things go together and it's genuine from Kim I Joanna and Amy seem great I've just met them recently so I'm just being honest but they are very nice and informative. Amanda I know is that another office but she was at the start with Kim and give a shout out to her wherever she is in the Phoenix area

I have put together 19 months of sobriety. CleanSlate has been by my side the 19 months helping me cope with the problems that come with rebuilding a life that was destroyed by addiction. I can't thank CleanSlate enough… for the professional,kindness,and love they have provided me in my life and in my continued sobriety!

Everyone there was so kind and understanding and I genuinely felt like I could be honest with them and receive no judgement. (update ive been going here for a long time and as someome whos been to a few different… clinics, this one really is the perfect. Dr.heath and kim are so amazing and make the sobriety journey worth it💕)

I personally would not recommend this place for anyone who isn’t a hardcore addict. There were things that happen & I didn’t feel there was a sense of urgency & it was also contradictory. Thanks to myself I am opioid… free! I would give the receptionist Kim I think her name is, 5 stars she was great! Very personable & has great customer service. Same goes for the therapist Angela.
