Does Medi-Cal Cover Drug and Alcohol Rehab?

Yes, and more completely than most people expect. Medi-Cal is California’s Medicaid program, covering roughly a third of the state’s population, and its substance use benefit spans screening and assessment, withdrawal management, residential treatment, partial hospitalization, intensive outpatient care, outpatient counseling, medication for opioid use disorder, case management, recovery services and peer support.

For most members there is no copay for any of it. The barrier in California is rarely cost and frequently availability: knowing which door to knock on, and whether a bed exists when you need one.

Start With the County

How the Pieces Fit

Californians on Medi-Cal typically hold a managed care plan: Anthem Blue Cross, Health Net, Molina, Kaiser Permanente, L.A. Care, IEHP, CalOptima, Partnership HealthPlan or another county-organized plan depending on where they live. That plan handles general medical care and mild to moderate mental health treatment.

Specialty substance use treatment sits elsewhere, with the county. Specialty mental health for serious conditions also sits with the county. Someone with both an addiction and a serious mental illness may therefore deal with county services for both, and it is reasonable to ask the access line to coordinate rather than run two separate processes.

Coverage by Level of Care

Withdrawal Management

Medically supervised withdrawal is covered at several intensities, from ambulatory through residential settings. Alcohol and benzodiazepine withdrawal carry seizure risk, and in an emergency the emergency department is the right destination rather than the access line.

Residential Treatment

Residential care is covered under Drug Medi-Cal with authorization from the county, assessed using American Society of Addiction Medicine criteria. California obtained a federal waiver permitting Medicaid payment for residential treatment in larger facilities, which is why residential access is broader here than in states without one.

Intensive Outpatient and Outpatient

Both are covered, and both are usually available sooner than a residential bed. If residential care involves a wait, starting intensive outpatient immediately is generally better than waiting, and the assessment can be revisited.

Medication for Opioid Use Disorder

Buprenorphine, methadone and naltrexone are all covered. Methadone comes through licensed narcotic treatment programs. Medi-Cal Rx administers the pharmacy benefit statewide, so a prescription follows the state formulary rather than your health plan’s, which removes one common source of confusion.

Recovery Services and What Comes After

Drug Medi-Cal covers recovery services after primary treatment ends, including continuing counseling, relapse prevention work and peer support, and members can access them without a fresh assessment. This is a genuine strength of the California benefit and one that many people never use because nobody mentions it.

Recovery housing is not a medical benefit, though the state funds some recovery residences through separate channels. Ask the county rather than assuming, since availability varies enormously between counties.

CalAIM and Enhanced Support

California’s CalAIM reforms added Enhanced Care Management and Community Supports to Medi-Cal, aimed at people with complex needs including those experiencing homelessness or cycling through emergency departments. For someone whose addiction sits alongside unstable housing, these services can matter more than the treatment episode itself. Ask your plan whether you qualify for Enhanced Care Management, because it is the plan rather than the county that administers it.

Eligibility

California expanded Medicaid, and eligibility extends to adults under a set income threshold regardless of dependent children. The state also extended full-scope Medi-Cal to income-eligible residents regardless of immigration status, which is unusual nationally and worth knowing, because fear of immigration consequences keeps people out of treatment who are in fact entitled to it.

Federal work requirements arrive for the expansion population from January 2027. People in substance use disorder treatment are exempt, though the exemption must be identified rather than assumed, so keep a record of your treatment enrollment.

If You Are Refused

Medi-Cal members can file a grievance with the plan or county and then request a state fair hearing. The Department of Managed Health Care also handles complaints against Medi-Cal managed care plans. Denials of residential treatment are appealable, and the ASAM criteria used in the assessment are the ground to argue on.

What Treatment Costs

Coverage for Specific Substances

Fentanyl now dominates the opioid picture across California, and it changes the practical advice. Withdrawal is less predictable than with heroin or prescribed opioids, buprenorphine induction can be harder, and a supervised setting is worth insisting on rather than attempting alone.

Methamphetamine is the other defining presentation in much of the state, and it has no approved medication. California has invested in contingency management, which has the strongest evidence base for stimulant use disorder and is available through county programs in a growing number of areas. Ask the access line whether it is offered locally, because few people know to.

Counties Differ, Sometimes Sharply

Availability of residential beds, waiting times and the range of programs vary a great deal between counties, and a benefit that exists on paper statewide is not evenly reachable. Larger urban counties generally have more capacity and more competition for it; rural counties may have very little locally and rely on placements elsewhere.

If your county cannot place you within a reasonable time, say so explicitly and ask what the alternatives are. Counties have obligations around timely access, and a documented request carries more weight than a call that ends with being told to try again next week.
