Every listing has a source: an official register, a licensing record, or details the facility sent us.

Credentials shown honestly. Accreditation, licensing and registration listed separately.

Reviews shown exactly as written. Never edited, never removed because a facility asked.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

What is Avera Health Plans?

Avera Health Plans is a regional, provider-sponsored health insurer founded in 1999 and owned by Avera Health, the non-profit Catholic system based in Sioux Falls, South Dakota. It sells individual and family plans, employer group coverage, self-funded administration and Medicare Supplement insurance, and reports more than 88,000 members.

The same Avera name belongs to the hospital system, which runs its own addiction treatment programs, so a center naming Avera may mean either one.

Avera Health Planshttps://rehabseekers.com/locations/us/south-dakota/avera-health-plans/
Does Avera Health Plans cover rehab?

Yes. Avera Health Plans states that its plans cover mental health and substance use treatment under ACA rules, including emergency room visits, outpatient counseling and therapy, inpatient hospitalization, partial hospitalization, residential addiction treatment and psychiatric medication management. What you pay depends on the plan, the network and the level of care.

Inpatient and residential stays are reviewed before admission. Member customer care at 888-322-2115, Monday to Friday, 8 AM to 5 PM Central, can confirm your benefits.

Private Insurance Acceptedhttps://rehabseekers.com/insurance-accepted/private-insurance-accepted/
Does Avera Health Plans cover inpatient rehab?

Yes, inpatient mental health and substance use services are a covered benefit on the 2026 ConnectPlus PPO and DirectConnect HMO plans, with preauthorization required. On the ConnectPlus Standard $6000 individual plan the member pays 40% coinsurance in network; on the DirectConnect $7500 HSA Eligible HDHP there is no charge after the deductible.

Preauthorization applies to an inpatient admission of any length. Without it, the member pays the full billed charge.

Avera Health Plans is the insurance arm of Avera Health, a non-profit Catholic health system based in Sioux Falls, and it sells individual and family coverage, employer plans and Medicare Supplement policies in South Dakota, northwest Iowa and northeast Nebraska. For anyone looking at addiction treatment, the benefit works like other commercial coverage: the plan pays its share once the service is medically necessary, in network and, where the rules say so, authorized in advance.

The name is shared, and that causes trouble. Avera Health is also a treatment provider, running medical detox, residential care up to about four weeks, intensive outpatient and outpatient addiction services in Sioux Falls and Aberdeen on 605-322-5142. A program that names Avera may mean it bills Avera Health Plans, or it may mean it belongs to the hospital system. Listings outside the three-state footprint almost certainly mean something else again, because the insurer is not sold there.

This guide sets out which levels of care the plans pay for, which admissions need preauthorization and how long a decision takes, what the 2026 cost shares look like on the named plans, how medication is handled across two pharmacy benefit managers, and how to work out whether a program sits in the network printed on the member card. Where the plan documents are silent, the question to ask is given instead of a guess.
