Community Health Alliance is not the name of one insurer. It is a phrase that several unrelated health plans share part of, and the payer whose published record sits behind this page is Health Alliance — legal name Health Alliance Medical Plans, Inc. — a Champaign, Illinois insurer owned by the Carle Health hospital system. That insurer ended all coverage on December 31, 2025 and ceased operating every line of business on January 1, 2026.

The distinction that decides this page is that there is no live benefit to check. Anyone holding a card with these words on it is almost certainly a member of something else: a Colorado regional accountable entity, a state or county Medicaid plan, or a Michigan insurer with a near-identical name. The full name and the program on the card identify the payer, not the two words a search engine matched.

This guide covers what the Illinois insurer paid for before it closed and what still happens to a 2025 claim, how its behavioral health reviews worked, who owned it and which states and plan lines it reached, what treatment costs without a plan behind it, and which similarly named organizations are separate.
