Health Alliance Medical Plans, Inc. was a provider-owned health insurer in Champaign, Illinois, owned by the Carle Health hospital system. It is no longer an insurer. All Health Alliance coverage ended on December 31, 2025, and the company ceased operating every line of business effective January 1, 2026, so a Health Alliance card cannot pay for a treatment admission that begins now.

The plan began in 1980 as CarleCare HMO, took the Health Alliance name in 1989, and sold individual, employer group, Medicare supplement and Medicare Advantage products across Illinois, Iowa and Indiana, with Ohio also named in 2025 reporting. Carle Health announced the first half of the exit in late February 2025 and the Medicare Advantage exit on May 22, 2025. No successor carrier took the book, and provider contracts terminated on January 1, 2026.

Does Health Alliance Cover Drug and Alcohol Rehab?

No. No Health Alliance plan pays for treatment with a date of service after December 31, 2025. Until that date, its Illinois individual exchange HMO and small group POS policies listed Substance Use Disorder Treatment and Substance Use Withdrawal Management among covered services, alongside mental health care.

Medical policy MP-263, Mental Health and Substance Use Treatment, was the document that governed placement. It recognized withdrawal management, residential treatment, partial hospitalization and intensive outpatient care for substance use, and inpatient, residential, partial hospitalization and intensive outpatient care for mental health conditions. That policy was first effective May 11, 2009 and had its last annual review on December 8, 2025, three weeks before coverage ended.

Which card is in hand? A Health Alliance card from Carle Health is out of force. Health Alliance Plan (HAP) in Michigan, Clear Health Alliance in Florida and Central California Alliance for Health are separate organizations with similar names and their own rules. The legal name printed on the Carle plan was Health Alliance Medical Plans, Inc., Champaign, Illinois.

What Did Health Alliance Pay For at Each Level of Care?

The full ladder, from withdrawal management through to outpatient counseling, where the placement met criteria. Substance use placement decisions ran on the ASAM Criteria Navigator powered by InterQual; mental health placements ran on InterQual Behavioral Health, Adult/Geriatric and Child/Adolescent Psychiatry.

Medical Detox

Substance Use Withdrawal Management was a named covered service in both the 2025 individual exchange HMO and the small group POS policy, and MP-263 treated withdrawal management as its own level of care. A detox center admission generally needed prior authorization, with two state exceptions described below.

Residential and Inpatient

Residential substance use treatment and inpatient psychiatric care were both recognized levels under MP-263, authorized up front and then reviewed concurrently. Concurrent review applied to acute behavioral health and substance use inpatient admissions in and out of network. Residential rehab stays were measured against ASAM placement rather than a fixed night count in the policy text.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient programs sat in the same policy, and concurrent review did not extend to outpatient services. The policy sections reviewed stated no visit limit for mental health or substance use care, and the Illinois individual HMO listed telehealth and virtual visits as covered options.

Did Health Alliance Need Prior Authorization for Rehab?

Yes. Prior authorization was generally required for behavioral health admissions under MP-263. In-house registered nurses handled initial review, medical directors who were board-certified psychiatrists issued any mental health or substance use denial, and Prest & Associates, Inc. provided independent psychiatrist review of cases that did not meet criteria on first pass.

Two state rules cut across that. In Illinois, no prior authorization review applied to the first 48 hours of an inpatient or detoxification admission for a pregnant or postpartum member. In Washington, where coverage ran under the Health Alliance Northwest joint venture with Confluence Health, the first two to three business days of substance use treatment were exempt from review. Published authorization lists applied to Illinois state-regulated plans and not to self-funded ERISA plans or state and local government plans.

Requests moved through Altruista Health GuidingCare, with eviCore handling parts of medical management. In the Illinois reporting year running October 1, 2022 to September 30, 2023, the plan logged 74,343 prior authorization requests across medical, surgical, mental health and substance use categories, with zero denials recorded for inpatient behavioral health admissions and for substance use disorder admissions.

What Happens to a Claim From Before January 1, 2026?

Services started or completed before January 1, 2026 fall under claims run-out, which Health Alliance and sister plan FirstCarolinaCare said they would staff after the close. How Much Does Rehab Cost With Health Alliance?

Nothing is payable now, and the 2025 figures are useful mainly as a benchmark for the replacement plan. Under the State of Illinois Health Alliance HMO for the plan year July 1, 2024 to June 30, 2025, behavioral health and substance use care cost $30 per outpatient visit and $425 per inpatient stay, with preauthorization required for inpatient admissions.

The 2025 Simplete Memorial HMO Limited Network 3500 Silver, an individual two-tier plan, charged $10 per outpatient mental health or substance use visit in Tier 1 and 40% coinsurance in Tier 2, with inpatient care at 30% Tier 1 and 40% Tier 2, against a $3,500 individual and $7,000 family deductible in Tier 1 and $7,000 / $14,000 in Tier 2. Its out-of-pocket maximum was $7,000 / $14,000 in Tier 1 and $9,200 / $18,400 in Tier 2.

On the Health Alliance Medicare Advantage POS Rx plan for 2025, inpatient psychiatric care cost $300 per day for days 1 to 4 and $0 for days 5 to 90 in network, outpatient mental health was $25 per visit, opioid treatment services $45 per visit, partial hospitalization $70 per day and outpatient substance use services 20% coinsurance, with out-of-network behavioral services paid at 30% coinsurance.

Those are the sums a program bills without any coverage behind it, and they are what applies to anyone who fell into a gap between the Health Alliance end date and a new plan taking effect. Where that gap exists, ask an admissions team about self-pay rates and sliding scale terms before dates are set.

Did Health Alliance Cover Suboxone, Methadone and Other Addiction Medication?

Partly documented. The Medicare Advantage POS Rx plan paid opioid treatment services at $45 per visit in network during 2025, and the Medicare list of medications requiring preauthorization effective December 1, 2025 did not name buprenorphine, Suboxone, Sublocade, Brixadi, naltrexone, Vivitrol, methadone or lofexidine. Formulary tier placement on commercial plans was not stated in available documents.

Because the pharmacy benefit closed with the rest of the plan, the live question is how the replacement policy handles medication assisted treatment. Ask the new plan whether buprenorphine and long-acting injectable formulations sit on the drug list, which tier they fall into, whether prior authorization applies, and whether methadone dispensed inside an opioid treatment program is billed as a program service rather than a pharmacy fill.

If a Dose Was Mid-Course on January 1

A prescription written under Health Alliance did not transfer automatically. The clinic that prescribes it reissues the authorization under the new plan, and raising that with the prescriber before a refill runs out avoids an interruption at the pharmacy counter.

Which Rehab Centers Are In-Network With Health Alliance?

None, as of January 1, 2026. Provider contracts terminated that day, new provider applications closed on November 1, 2025, and no provider directory remains on the Health Alliance site. Network status now belongs to whichever carrier issued the replacement card, and it is settled in that plan’s provider search for that specific product.

Each listing shows the payments a program says it accepts, its licensing and accreditation, and how far it sits from the nearest airport, which narrows a shortlist before the call that settles benefits under the current plan.

How Do I Find a Rehab That Took Health Alliance?

Start from the replacement coverage rather than the old card. Carle Health pointed former Medicare Advantage members to ChooseCarleHealth.org to see other Medicare plans the system works with, and State of Illinois employees moved during Benefit Choice open enrollment from May 1 to June 2, 2025, defaulting to the state PPO if no choice was made.

University of Iowa retirees on the Health Alliance Medicare Advantage HMO and PPO could enroll in another plan from October 15, 2025, with a Medigap guaranteed issue right running until February 28, 2026. A program that still advertises “Health Alliance accepted” is describing a contract that has expired, so the admissions team should be asked which carrier it bills today.
