Does United Concordia Cover Drug and Alcohol Rehab?

No. United Concordia sells dental and stand-alone vision plans only, and its plan documents cover dental services. Detox, residential treatment, partial hospitalization, intensive outpatient care, counseling and addiction medication are paid, where they are paid at all, by the member’s separate medical plan rather than by the dental carrier.

Nothing in the 2026 product line changes that. The Smile for Health Wellness add-on gives extra gum disease and cavity benefits to members with diabetes, heart disease, stroke, lupus, rheumatoid arthritis, oral cancer, organ transplant, pregnancy, autism, GERD, Parkinson’s, Sjogren’s or vitamin deficiency, and Concordia Clarus, the prevention-focused copay plan effective January 1, 2027, adds cleanings rather than medical services. No therapy, psychiatry or telehealth mental health benefit appears in any United Concordia plan document, and the carrier operates no crisis or nurse line.

Medical Detox

Medical detox is billed as a medical service and goes to the medical plan, not to United Concordia. Where withdrawal from alcohol or benzodiazepines is already under way, an emergency department is the right destination and the coverage questions are settled afterwards.

Residential and Inpatient

A residential rehab stay is the largest bill in addiction treatment and the one a dental plan touches least. The admitting team bills the medical carrier, applies that plan’s deductible and coinsurance, and runs continued stay review with that carrier’s utilization team.

PHP, IOP and Outpatient

Partial hospitalization, intensive outpatient and weekly counseling follow the same route. Group and individual sessions, psychiatric follow-up and medication management are medical benefits, and a dental annual maximum has no bearing on any of them.

Which Card Pays for Treatment If United Concordia Does Not?

The medical card does. For a Highmark member in Pennsylvania, West Virginia, Delaware or western New York, the Highmark medical plan carries the behavioral health benefit. For a military family, TRICARE medical coverage is where substance use treatment sits, and the TRICARE Dental Program is voluntary and separate from it. A federal employee holds FEDVIP for dental and a Federal Employees Health Benefits plan for everything else.

United Concordia also administers only the dental slice of several government plans. Highmark Wholecare covers Medicaid and D-SNP members in Pennsylvania, where behavioral health runs through the HealthChoices arrangement, and Highmark Health Options Duals plans in Delaware and West Virginia carry their own medical benefits. In each case the dental administrator and the treatment payer are different organizations.

Does United Concordia Need Prior Authorization for Rehab?

No, because there is nothing to authorize. United Concordia has no substance use or mental health benefit, so it sets no prior authorization rules for detox, residential care or IOP and runs no concurrent review on treatment. Any authorization requirement comes from the medical plan named on the other card.

Those dental cost shares vary by product. The Concordia Flex Plan written for Lehigh Valley Dental effective September 1, 2026 carries a $50 individual and $150 family deductible that does not apply to Class I services, pays preventive care at 100% and basic care at 80% in and out of network, and caps the year at $1,000. FEDVIP has no deductible in 2026, with an unlimited annual maximum on the High Option and implants capped at $2,500, while the Standard Option annual maximum dropped from $1,500 to $1,000 and orthodontic lifetime maximums run $3,000 High and $1,500 Standard. The TRICARE Dental Program has no deductible, no copay for diagnostic and preventive services and 20% for basic restorative work, with premiums reduced on March 1, 2025 and held through February 28, 2026. Concordia Plus DHMO plans have no deductibles, waiting periods or calendar-year maximums.

Those are the sums a program would bill without any medical coverage behind it. Where no medical plan exists at all, the practical routes are self-pay arrangements, sliding scale fees and state-funded admissions rather than anything a dental policy can offset.

Does United Concordia Cover Suboxone, Methadone and Other Addiction Medication?

No. United Concordia runs no pharmacy or medical benefit of any kind, so buprenorphine, Suboxone, naltrexone and methadone are paid for by the member’s medical plan and its pharmacy coverage. A prescription filled at a retail counter is billed to that plan’s drug list, never to the dental carrier.

Medication assisted treatment also splits by setting. Medication given during an inpatient or residential stay is part of the program’s medical charge, while buprenorphine prescribed in an office and methadone dispensed through a licensed opioid treatment program follow two different billing paths on the medical side. Search by the medical insurer instead. Pull out the medical card, use that carrier’s behavioral health provider search, and give the admissions team the plan name and member ID from it. Is United Concordia the Same as Highmark or Delta Dental?

United Concordia is owned by Highmark Inc., a Blue Cross Blue Shield licensee in Pennsylvania, West Virginia, Delaware and western and northeastern New York, but it sells nationally under its own name and is not a Blue plan. Since March 18, 2024 all dental claims for Highmark BCBS Western New York members go to United Concordia. Delta Dental is a separate competitor with no connection to it, and neither dental carrier pays a psychiatrist.

The network also appears under other brands. Centene rents it for Ambetter marketplace dental across 23 states and for Wellcare Medicare dental in Illinois, Maine, Missouri, North Carolina, Nebraska and Ohio, so a card carrying another logo can still lead to United Concordia dentists while the medical benefit stays with the issuing plan.
