Does Healthy Louisiana pay for addiction treatment?
Yes. Louisiana Medicaid serves most members through Healthy Louisiana managed care plans, and since December 1, 2015, mental health and substance use services have been part of those plans. Children enrolled in the Coordinated System of Care are the exception.
| Type of care | Healthy Louisiana | Louisiana self-pay price |
|---|---|---|
| Withdrawal management (detox) | Ambulatory, residential and hospital settings | $220–$700 a day |
| Residential treatment | Covered; the federal waiver allows stays past 15 days in larger facilities | $6,000–$25,000 a month |
| Intensive outpatient | Covered | $2,500–$9,000 a month |
| Outpatient therapy | Covered | $1,200–$5,000 a month |

- Six plans, one benefit. The Healthy Louisiana plans are Aetna Better Health, AmeriHealth Caritas Louisiana, Healthy Blue, Humana Healthy Horizons, Louisiana Healthcare Connections and UnitedHealthcare. About 1.5 million people are enrolled in Louisiana Medicaid.
- Who qualifies. Coverage for low-income adults under Medicaid expansion started on July 1, 2016. Adults can qualify with income at or below 138% of the federal poverty level. Apply through the Louisiana Medicaid Self-Service Portal at MyMedicaid.la.gov.
- Updated clinical rules. The addiction services chapter of the Medicaid provider manual now follows the ASAM Criteria, 4th Edition, and providers must fully comply as of July 1, 2026. Levels of care may be subject to prior approval and ongoing review by your plan.
- Longer residential stays. Since February 1, 2018, a federal waiver has let Medicaid pay for addiction treatment and withdrawal management in larger residential facilities beyond the usual 15-day limit; the current approval runs through December 31, 2027. The state aims for a statewide average stay of 30 days or less, a target rather than a cap on any member’s stay.
- A renewal is underway. LDH took public comment from October 7 to November 6, 2026, on renewing the waiver for 2028 through 2032; the renewal depends on CMS approval.
- Small copays, with limits. Louisiana Medicaid’s copay rule covers prescriptions, at up to $3 per drug, and exempts people under 21 and pregnant women, among others. Providers may not refuse care because you cannot pay the copay, and total cost-sharing is capped at 5% of family income.
Methadone has been a Medicaid benefit only since 2020
Louisiana Medicaid began paying for methadone for opioid use disorder in early 2020, through opioid treatment programs. A Louisiana Department of Health report to the Legislature notes that before then, methadone treatment was paid in cash or covered by Office of Behavioral Health grants.
From January 1, 2027, some members aged 19 to 64 face work or community engagement rules; being in substance use disorder or alcohol treatment is an exemption. Search programs that accept Medicaid to find Healthy Louisiana providers.

- What public services include. The Office of Behavioral Health lists intensive inpatient and outpatient substance use care, residential treatment for pregnant and parenting women with their children, and opioid treatment programs among its services.
- What you pay. State law, R.S. 28:771, tells the Office of Behavioral Health to set criteria for judging whether each patient can pay all or part of the cost of care. One example in the statute: a urine drug screen copay of no more than $12, charged only when a sliding fee scale shows you can afford it.
- Medicaid-eligible patients are exempt from that drug-screen copay.
Our directory also lists state-funded and sliding-scale programs across Louisiana, alongside private centers.

What does Louisiana insurance law require for addiction care?
Louisiana’s statute R.S. 22:1066.2 applies to health coverage plans issued in Louisiana that include mental health and substance abuse benefits. Any medical-necessity decision those plans make must comply with the federal Mental Health Parity and Addiction Equity Act and with Louisiana’s internal appeals and external review law.
In plain terms, if your state-regulated plan denies a residential stay or cuts days short, it must judge necessity under federal parity rules, and you can file an internal appeal and then seek an independent external review. The statute also requires coverage of care delivered through integrated models such as the Psychiatric Collaborative Care Model.
| Route | What you pay | Keep in mind |
|---|---|---|
| Healthy Louisiana (Medicaid) | Pharmacy copays of up to $3 per prescription for some members; total cost-sharing capped at 5% of family income | Care levels may need prior approval from your plan |
| Local Governing Entity | Based on what the Office of Behavioral Health criteria say you can pay | Serves the parishes in its region |
| Insurance plan issued in Louisiana | Your deductible, copays and coinsurance | Medical-necessity decisions must follow federal parity rules; you can appeal. The law applies to state-regulated (insured) plans. |
| Self-pay | $6,000–$25,000 a month residential; $2,500–$9,000 a month IOP | Ask what the quote leaves out, such as detox, labs or medication |

How we collected Louisiana rehab prices
A team of researchers commissioned by RehabSeekers.com called treatment centers in every state, at least five per state and in more than one city, and asked for self-pay prices at each level of care. The Louisiana ranges span the lowest to the highest quote from Louisiana centers.
- Prices: quotes for admission in early 2027, gathered from programs directly rather than copied from other websites.
- Timing: calls began in October 2026, with final checks continuing through January 2027.
- Totals: detox multiplies the daily price; every other level prorates the monthly price by day.
- Coverage and health data come from the Louisiana Department of Health, the Louisiana Revised Statutes, CMS, SAMHSA, the CDC and NIDA, all listed under Sources.
