Amazing place with amazing people, i have worked with John the NP and several counselors there and they have brought such a different perspective to my life. I am working the steps to being a better mother, wife and… person - This is the place to go if you want a better life!!!

I had severe spine surgery in 2017, debilitating pain that had stolen my life. Multiple car wrecks. Nothing helped. I was in pain management for 8 years with no relief. A friend talked me into trying them, but far the… best choice i have ever made. I love these guys and they truly care

I went here for a few months. Its expensive, doesn't take most insurances. Basically just paying for valium as I had an alcohol problem. I showed up one day under the influence and discharged me. I was not being loud… and angry. I had a problem and instead of helping me through it, they discharged me even though my account balance had money for another appointment. It saddens me to say all of this, but it's the truth to what happened and how I feel. They called me a week later to (check on me). I had apologized for showing up that way and asked if I could come back and never heard from them again. I never spoke with a psychiatrist. The only person I ever saw was John who I believe is a nurse practitioner. I felt abandoned and still do. I thought paying out of pocket for help would provide better care and unfortunately that wasn't the case.

Dr. D, rip, was a professional gentleman willing to help you with anything. When nobody listened, he did. Even fixed his wifes 3000gt!! I followed him to C5R where I was introduced to the 3 Mr. John's and Dan. 3… outstanding men always extending a hand when needed. 2 months ago when life could've been bad, I was given a break. Very thankful to these gentleman. Thank you!!

Dr. John has been so kind and understanding during this transition in my life. I really really recommend this person and this place. Couldn’t have done it without you guys ♥️

Love this place you have freedom don't have to worry about anything. Dr and staff are awesome !!

Nowhere else is better

I could go on for hours about my experience here, not only is the staff absolutely amazing and care whole heartedly about each persons care but the compassion, patience, hope, positivity and acceptance they all give is… beyond measure. I was so nervous when I took the leap to get help for my alcoholism. Nurse Practitioner John has truly changed my life. He is more than just my Wednesday appointment I look forward to it every single week! He has given me the tools to get my life back. I truly can’t ever thank him enough and beyond GRATEFUL for him and his guidance. Every clinic and health care provider should take a page out his book because he is the kindest soul I have ever met! I thank god I found this place and will continue going every week for the joy and hope it brings To my life.

This is what ALL other treatment and addiction centers, not only in Ohio but the U.S, should be looking up towards and trying to model the same level of care and adequacy. John the NP is brilliant, and an absolute Earth… Angel. Go, it will change your life forever, if you’re ready to be honest with yourself and seek help.

I have been coming to this office for 5+ years and I like the fact the "stigma" of our issues are left at the door, they treat everyone like they truly matter and it's not to look good I feel it's genuine, I like the… fact the doctors let us be judge of our recovery for the most part, sometimes us ex addicts get treated like there's a magic pill, or time that fixes us, and when "according to them" that tome gets here us ex addicts are left in the dark, with our only source of help treating me like I'm cured. I have never been treated like this here. Thanks for always being here for me.

There isn’t enough space on here to say all the wonderful things I could say about Clinic 5. It’s the HUMAN factor that really leaves you feeling like you’re special and not just a number. They really listen and help… and show you that a good life and success IS attainable. The staff here are efficient, smart, knowledgeable and they truly care about each person who comes through their door. My life has reached heights that I had long ago dismissed as being too far gone but now I know that the sky is truly the limit with every new goal made and every former goal achieved I’m not just “clean” or “sober” I’m actually growing and learning how to be successful in society. I truly feel like this is the type of formula that should be utilized in A more widespread way across our country.

This clinic is great! The staff and personnel here really care and take their time to help each patient’s specific needs. John who I’ve worked with throughout my journey here at this clinic, has helped me tremendously… and I’m so grateful to have help from people who genuinely care about their patient’s needs and go the extra mile to help. This is the best place I have found over 35 years! I have been with them for over 12 years now and they understand addiction and how it works better than all the other places I have been to. They care more than anywhere… else and help me remember that if I get knocked down to pick myself up and try again….thanks to them I'm sober with 5/10 beautiful kids, a handsome husband that I couldn't be more proud to have and a very successful professional career that I can actually be part of without having to let everyone know bc they don’t require the weekly check ins and meetings and pill counts like other places. Which would keep me from being able to work freely and having to inform everyone. They call and check on me when I get in my own head and start to slip off. I believe they are the foundation that jump started my sobriety and the support to keep going everyday. Thanks Sub 5 Clinic ❤️ you have became my family also!!

I'll start off by saying that this place has totally went down hill in the past 2 years. I've been here for 10 years total it's never been like it is now it's all about money that's all they care about. They tell you… one thing to your face and then totally turn around and lie. PLEASE look elsewhere I wouldn't recommend anyone here and I wouldn't even give it a1star if I didn't have to!!

Clinic 5 is like family everyone there gets along with each other and we talk are problems out if we have one if it wasn't for clinic 5 I don't know what or how I would deal with any of my stress this place I would… gladly take any advice from and I would gladly bring anyone here if they need to get help it's a great place for recovery

My experience at clinic 5 is one of the upmost ! I have nothing but respect for the doctors and counselors and nurses all of them are amazing the front desk girl is so welcoming and polite and you can tell she cares ,… John is absolutely amazing he is a true friend for life for sure and Dan oh man he is the most inspirational man I’ve ever met me and my family owe nothing but thanks to all of clinic 5 they saved my life and my family🙏🏻🙏🏻💪🏻💪🏻💪🏻
