Common questions

Answered from what is listed here.

What is ArchCare?

ArchCare is the continuing care community of the Archdiocese of New York, run by the nonprofit Catholic Health Care System. It operates nursing homes, home care and Calvary Hospital, and its active health plan is ArchCare Senior Life, a PACE program for adults 55 and over in New York City and Westchester.

It is not a commercial insurer. ArchCare sells no employer or marketplace coverage, and its plans serve adults certified as needing nursing-home-level care.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Does ArchCare cover rehab for drugs or alcohol?

Yes. The ArchCare Senior Life participant handbook lists Alcohol and Substance Abuse Services, described as treatment to end the excessive use of a substance such as alcohol or drugs, among the Medicare-covered services the PACE program covers. The interdisciplinary care team authorizes the care and arranges it in network.

Individual levels of care are not named in the handbook, so ask the care team at 1-866-263-9083 what treatment it will arrange.

Alcohol Rehabhttps://rehabseekers.com/addictions/alcohol/
Does ArchCare cover detox?

Yes, but only inside the general PACE substance use benefit. ArchCare Senior Life materials do not name detox, residential care, partial hospitalization or IOP separately. The interdisciplinary care team decides and authorizes the setting with a network provider, and emergency care never needs prior authorization.

Someone who is an inpatient or resident of a New York OASAS facility may apply to PACE, but enrollment can begin only after discharge.

ArchCare is the continuing care community of the Archdiocese of New York, a nonprofit system of nursing homes, home care and hospice run by Catholic Health Care System, Inc. that also operates health plans. Its active plan is ArchCare Senior Life, a PACE program combining Medicare and Medicaid for adults 55 and older who are certified as needing nursing-home-level care but who live at home.

That shape decides how addiction treatment is arranged. ArchCare sells no employer or marketplace coverage, so there is no commercial network to shop. Under PACE an interdisciplinary care team arranges and authorizes every non-emergency service, and the participant handbook lists “Alcohol and Substance Abuse Services” among the Medicare-covered services, defined as treatment to end the excessive use of a substance such as alcohol or drugs. Two other ArchCare plans complicate the picture: ArchCare Community Life, the Medicaid managed long term care plan, has closed, and ArchCare Advantage, the Medicare Advantage institutional special needs plan, has no published documents newer than 2023.

This guide covers which ArchCare plan pays for what, where prior authorization applies, what treatment costs under PACE and under the last published Advantage figures, how medication for opioid and alcohol use is handled on the formulary, and how to reach a New York program that ArchCare will pay for.

Does ArchCare Cover Drug and Alcohol Rehab?

Yes, within the rules of the plan. ArchCare Senior Life names alcohol and substance use services among its covered Medicare services, with no copayment attached, provided the PACE care team authorizes the care and a network provider delivers it. The 2023 ArchCare Advantage plan covered outpatient sessions, partial hospitalization, inpatient psychiatric care and opioid treatment program services.

PACE is a combined Medicare and Medicaid benefit, so the usual split between medical coverage and long-term care does not apply. Enrollment requires New York State certification of nursing-home eligibility, residence in the service area and a need for more than 120 days of community-based long-term care. Anyone who is an inpatient or resident of an OASAS addiction facility may apply, but enrollment can begin only after discharge.

What Does ArchCare Senior Life Pay For at Each Level of Care?

The handbook and the plan brochure do not break the benefit into named levels of care. Detox, residential treatment, partial hospitalization, intensive outpatient and medication treatment are not listed separately, so the level is settled case by case by the interdisciplinary team on the basis of medical, physical, emotional and social needs rather than by a published benefit grid.

Medical Detox

Withdrawal management is not named in the PACE documents, so a participant or caregiver should ask the care team how medical detox is authorized before anything is scheduled. Emergencies are the exception to every authorization rule: alcohol and benzodiazepine withdrawal can cause seizures, and an emergency department is the right destination in acute withdrawal, with the hospital calling 866-263-9083 afterwards.

Residential and Inpatient

PACE is built around care at home with day health center support, so an extended residential rehab stay is a question for the team rather than an assumed benefit. Under the 2023 ArchCare Advantage plan, inpatient mental health care carried a $1,200 copay per Medicare-covered admission or stay.

PHP, IOP and Outpatient

The 2023 Advantage plan priced partial hospitalization at 20% coinsurance and outpatient substance use sessions, individual or group, at the same 20%. Outpatient mental health therapy was $0 with prior authorization. Senior Life commits to behavioral health access standards instead of copays: an initial clinic or professional visit within 10 business days, urgent care within 48 hours, follow-up within 7 days of a hospital or emergency discharge, and non-urgent follow-up within 10 business days.

Which ArchCare plan is on the card? ArchCare Senior Life is the PACE program, active in the Bronx, Manhattan, Staten Island, Westchester and part of Rockland. ArchCare Advantage was a Medicare Advantage institutional special needs plan under contract H1777, with no plan documents published since 2023. ArchCare Community Life, the Medicaid managed long term care plan, has closed and no longer appears in the New York State Department of Health plan directory.

Does ArchCare Need Prior Authorization for Rehab?

Yes for almost everything. Under ArchCare Senior Life a service has to be deemed necessary by the primary care physician and the care management team, come from a network provider and carry prior authorization before it is delivered. Emergency care is the exception and needs no authorization; the treating provider calls 866-263-9083. The 2023 Advantage plan required authorization for outpatient mental health therapy and for opioid treatment program services.

No named medical necessity standard such as ASAM or InterQual appears in the published PACE material, which is worth knowing before an admissions team asks which criteria set applies. The decision belongs to the interdisciplinary team, and a social worker sits on every participant’s team, so a request for mental health treatment or addiction care can start there.

What Happens If Treatment Is Out of Network?

A PACE participant can be held fully liable for unauthorized or out-of-PACE services, and a provider who does not contact ArchCare before delivering non-emergency care forfeits the right to bill. New arrivals may continue with a non-network provider for up to 60 days, and anyone whose provider leaves the network may continue for up to 90 days.

<div style="background: #fff3ec; border-left: 4px solid #fe632d; border-radius: 0 8px 8px 0; padding: 16px; margin: 12px 0;
“>

If an appointment cannot be found in time. When the behavioral health access standards above cannot be met, ArchCare Senior Life takes an Access Complaint, locates an in-network or out-of-network provider and sends the contact details within 3 business days. Directory errors go to ProviderRelations@ArchCare.org or 1-866-263-9083, Option 1, and are corrected within 15 calendar days.

How Much Does Rehab Cost With ArchCare?

Under ArchCare Senior Life, nothing for authorized care. Covered services carry no copayments and no out-of-pocket expenses, and most participants pay no monthly premium or cost share, depending on income. A Medicaid participant with a spend-down pays that amount monthly to ArchCare, and a private-pay participant pays a private premium instead.

The Advantage figures are older and come from the 2023 plan year: a $38.90 monthly premium on top of the Part B premium, no in-network medical deductible, a maximum out-of-pocket of $8,300, a $505 Part D deductible followed by 25% coinsurance in the initial and coverage gap stages, and the behavioral health cost shares listed above. Nothing newer than 2023 has been published for that plan, so a current Advantage member should ask what applies this year.

Typical US market rates before insurance
Level of careMonthlyDaily
Medical detox (7–14 days)$1,500 – $5,200 per week$215 – $745
Residential inpatient$6,200 – $26,000$205 – $865
PHP$7,200 – $14,000$240 – $465
IOP$2,700 – $9,200$90 – $305
Outpatient$1,300 – $5,200$45 – $175

Those are the sums a New York program would bill without coverage, and they are the exposure a PACE participant takes on by arranging an intensive outpatient program or a residential stay without authorization. The authorization step, not the sticker price, is what decides the bill.
