What is BrightPath insurance?

BrightPath is not an insurance company. It is an Idaho provider network, the name St. Luke's Health Network uses in two of its regions and the name Select Health gives its network for Eastern and Northern Idaho. The plan on the card, usually Select Health or PacificSource, pays the claims.

BrightPath decides which doctors and facilities are in network. Your Select Health or PacificSource plan document sets what detox, residential or outpatient care costs and whether it needs authorization.

Is BrightPath the same as Select Health?

No. Select Health is the insurer; BrightPath is one of four provider networks its Idaho plans are sold on, alongside Select Health SLHP, SAHA and Med. A BrightPath card means a Select Health plan built on the Eastern and Northern Idaho network.

Select Health is the nonprofit health plan of Intermountain Health, with more than one million members and plans sold in Utah, Idaho, Nevada and Colorado.

Is BrightPath the same as St. Luke's Health Partners?

Yes, in effect. St. Luke's Health Partners, now renamed St. Luke's Health Network, operates under the BrightPath name in its regions 1 and 3. It is a value-based care network owned by St. Luke's Health System in Boise and covering 20 Idaho counties.

St. Luke's lists SelectHealth SLHP/BrightPath and PacificSource SLHP/BrightPath/Navigator as in-network plan labels, and the network counted more than 3,500 providers in 2026.

Is BrightPath the same as Bright Health?

No. Bright Health Group was a Minneapolis insurer that exited individual markets in more than a dozen states during 2022 and sold its California Medicare Advantage plans in 2023. BrightPath is an Idaho provider network used by Select Health and PacificSource plans.

If a card reads Bright Health rather than BrightPath, call the member number printed on the back; the two organisations have no corporate connection.

What states does BrightPath operate in?

Idaho only. The BrightPath network covers Eastern and Northern Idaho counties: Bear Lake, Bingham, Bonneville, Franklin, Fremont, Jefferson, Kootenai, Madison, Oneida and Teton. In Southwest Idaho the same St. Luke's network is sold under the Select Health SLHP label instead.

Select Health itself sells plans in Utah, Idaho, Nevada and Colorado, but the BrightPath network option is offered only to Idaho members.

Does BrightPath need prior authorization for rehab?

For commercial plans in Idaho the stated decision time is 2 business days. Urgent requests need an immediate contact number and a written explanation from the treating provider.

Does BrightPath cover out-of-network rehab?

Only in limited situations. BrightPath cards read with MED or CARE2 Access, which adds those Select Health networks, and Idaho small employer plans use the UnitedHealthcare Options PPO network outside Colorado, Idaho, Nevada and Utah. Plan summaries do not publish out-of-network cost share.

Is BrightPath a Medicaid plan?

No, BrightPath is not a Medicaid product. Select Health states that the network is used for all of its lines except Medicaid, so a BrightPath card means commercial, individual marketplace or Medicare coverage. Select Health does describe BrightPath as serving Idaho's growing Medicare population.

If you carry Idaho Medicaid instead, treatment is billed through that program, and the rules for detox and residential care come from the state rather than Select Health.

Bright Path, written BrightPath by the organizations that use the name, is an Idaho provider network rather than an insurance company. A card showing it belongs to a health plan built on that network, almost always Select Health and in some listings PacificSource. That split decides how a treatment search works: the network sets which clinics and hospitals are contracted, and the plan sets what is covered and what it costs.

The network is run by St. Luke’s Health Network, formerly St. Luke’s Health Partners and part of St. Luke’s Health System in Boise, which says it operates as BrightPath in regions 1 and 3 of its service area. Select Health lists Select Health BrightPath among its Idaho provider networks, offered for all lines except Medicaid, and sells it across Eastern and Northern Idaho: Bear Lake, Bingham, Bonneville, Franklin, Fremont, Jefferson, Kootenai, Madison, Oneida and Teton counties. Does Bright Path Cover Drug and Alcohol Rehab?

The network itself pays nothing. The Select Health or PacificSource plan on the card decides coverage. Select Health reviews inpatient, residential, partial hospitalization and intensive outpatient behavioral health requests on its preauthorization form, which shows those levels are considered for payment when they are medically necessary, authorized and delivered in network.

The published Idaho plan summaries do not list inpatient mental health or substance use treatment as a separate line, so the benefit wording for detox and residential care sits in the plan contract rather than the brochure. What Does a BrightPath Plan Pay For at Each Level of Care?

The level is matched to clinical need at assessment, and cost share then follows the plan tier. Office and virtual behavioral health visits sit at a flat copay on every Idaho individual plan; the residential and hospital end of the ladder runs through preauthorization and the plan’s inpatient cost share.

Medical Detox

Medical detox is billed as an inpatient or residential admission, which is the category the behavioral health preauthorization form opens with, so the request goes in before the date wherever the admission is planned. Where withdrawal has already turned acute, an emergency department is the right destination; a St. Luke’s Health Partners plan description states that out-of-network emergency coverage includes a mental or nervous condition, substance use disorder or addiction.

Residential and Inpatient

Residential rehab and inpatient admissions are reviewed through the same initial request form. On 2027 Idaho individual plans general inpatient hospitalization is shown either as a daily copay, for example $850 per day with a five-day maximum, or as coinsurance such as 50% after the deductible, depending on the plan. The medical necessity criteria Select Health applies are not published, so the admissions team is worth asking which standard the reviewer will use and how continued stay is handled.

PHP, IOP and Outpatient

Partial hospitalization and intensive outpatient both appear on the preauthorization form. Behavioral health office and virtual visits are a listed benefit on every Idaho individual plan at the plan’s copay, and Select Health promotes Mental Health Integration, collaborative mental health care built into primary care. Transcranial magnetic stimulation is covered on all Select Health plans for treatment-resistant depression from September 1, 2024 with preauthorization; Spravato is not covered on commercial plans.

Which card are you holding? The 2024 individual card sample for this network reads “Select Health Network POS with MED or CARE2 Access (70/71) – Eastern and Northern Idaho Counties”, and the small employer version reads “Living in Eastern and Northern Idaho Counties”. The payor ID on Select Health Idaho cards is 39026. The other Idaho network labels are Select Health SLHP, SAHA and Med, and a program contracted for one is not automatically contracted for another.

Does Bright Path Need Prior Authorization for Rehab?

Yes. Select Health uses a Behavioral Health-Related Preauthorization Initial Request form for inpatient, residential, partial hospitalization and intensive outpatient care, and the stated decision time for commercial plans in Idaho is two business days. Plan summaries add that preauthorization is required for certain services and that visit limits may apply.

Requests are submitted by the treating provider through the Select Health Preauth & Care Plan Tool, where some requests can auto-approve, or by fax: commercial 801-442-0825, Medicaid and CHIP 801-442-0625, and Medicare 801-442-0302. An urgent request needs an immediate contact phone number and a written explanation from a medical provider. The provider line for preauthorization questions is 800-222-6358.

Does a BrightPath Plan Use an Outside Behavioral Health Vendor?

No outside behavioral health manager is named for these plans. Requests go to Select Health’s own fax lines and online tool, and the code lists that drive them are updated quarterly by state and plan type. NovumHealth, the behavioral health advocacy service Select Health offers, is available in Utah only and excludes self-funded and level-funded plans, so an Idaho BrightPath household works through Member Services and Care Management instead.

How Much Does Rehab Cost With Bright Path?

It depends on the plan tier rather than the network. On 2027 Select Health Idaho individual plans, a behavioral health office or virtual visit copay runs about $15 to $50; Bronze 7750 is $30 after the deductible and Gold plans sit at $15 to $25. On 2027 Idaho small employer plans the mental health office visit copay is $25, $30, $40 or $45 after the deductible. For 2026, individual behavioral health visits range from $0 to $50 by plan and cost-share reduction level.

Deductibles on 2027 Idaho individual plans run from $0/$0 on Silver Copay and some Gold and CSR plans up to $12,000/$24,000 on Expanded Bronze 12000, single and family, with out-of-pocket maximums from $2,000/$4,000 on the 87% CSR Silver Copay plan to $12,000/$24,000. The 2026 line included Bronze 8000 at $8,000/$16,000, Silver plans at $5,000, $4,000 and $3,000, Silver Copay at $0 and Gold 1500 at $1,500/$3,000, with the Bronze out-of-pocket maximum rising to $10,150/$20,300 and Silver standardized to $9,900/$19,800. The published summaries do not show out-of-network cost share, so that figure has to come from Member Services.

Those are the amounts a program bills without coverage, and they are the reason the authorization step matters more than the sticker price. With a plan in place the figure that counts is the deductible and out-of-pocket maximum above, which is why a stay starting late in the plan year is worth planning around the January reset.

Does Bright Path Cover Suboxone, Methadone and Other Addiction Medication?

The drug list on the plan settles it, and formulary placement for buprenorphine, naltrexone and methadone is not published on the pages that describe these Idaho plans. Select Health runs a Drugs with Special Requirements look-up by drug list, including RxSelect and RxCore on the commercial side, and Pharmacy Services answers on 800-442-3129.

Where the Prescription Is Filled

A prescription picked up at a pharmacy runs through the drug list; medication dispensed inside an opioid treatment program is part of the program service and is billed under the medical benefit instead, which is why it never shows as a pharmacy fill. What to Ask About Medication Treatment

Three questions settle it: is the specific formulation on the plan’s drug list, does it carry a step or preauthorization requirement, and is the prescribing clinic contracted for the BrightPath network. Those apply equally to Suboxone, to naltrexone including the extended-release injection, and to a methadone clinic, where dosing is authorized as a program service rather than a fill.

Is Bright Path the Same as Select Health, St. Luke’s or Bright Health?

Not quite, in three different ways. Select Health is the health plan, part of Intermountain Health, and BrightPath is one of four Idaho networks its plans are sold on alongside SLHP, SAHA and Med. St. Luke’s Health Network, owned by St. Luke’s Health System and formerly called St. Luke’s Health Partners, runs the network and operates under the BrightPath name in two of its regions; St. Luke’s Health Plan, an insurer launched for coverage from January 2023, is a separate organization again.

Two similar names are unrelated. Bright Health Group, the Minneapolis insurer, exited individual markets across more than a dozen states in 2022 and sold its California Medicare Advantage plans to Molina in 2023. Clear Spring Health BrightPath Advantage (PPO) was a 2026 Medicare Advantage plan in Georgia, and Clear Spring Health states it is leaving the Medicare Advantage market as of June 1, 2026. Neither has anything to do with the Idaho network.

Before you call a program: read the network name and payor ID off the card, note the plan tier and deductible, and ask the admissions team whether it is contracted for that exact network and whether the preauthorization request has been filed. Those three answers settle the cost before anyone talks about dates.
