Its coverage can relate to drug and alcohol treatment, varying by plan, network and level of care, and some services need approval first.New York centers listed here name Centers Plan for Healthy Living on their own websites. Common questions

Answered from what is listed here.

What is Centers Plan for Healthy Living?

Centers Plan for Healthy Living was a New York Medicaid managed long-term care plan based in Staten Island, serving Medicaid and dual Medicaid/Medicare adults who needed ongoing care at home. It ran an MLTC partial-capitation plan and Centers Plan for Medicaid Advantage Plus, a combined Medicare and Medicaid D-SNP.

Elevance Health bought the company on December 31, 2024, and its roughly 53,000 members were moved into Anthem and Wellpoint plans during 2026.

Medicaid Rehabhttps://rehabseekers.com/insurance-accepted/medicaid/
Does Centers Plan for Healthy Living still exist?

No, not as a plan name. Medicaid Advantage Plus members moved to Anthem HealthPlus Full Dual Advantage LTSS 2 (HMO D-SNP) on January 1, 2026, and MLTC members moved on March 1, 2026, to Anthem Blue Cross and Blue Shield HP downstate or Wellpoint in Erie and Niagara.

The legacy member and provider portals stay open through February 28, 2027, and claims for service dates before the transition follow the old procedures.

Medicaid Rehabhttps://rehabseekers.com/insurance-accepted/medicaid/
Who owns Centers Plan for Healthy Living?

Elevance Health owns it. The company completed its acquisition of Centers Plan for Healthy Living LLC on December 31, 2024, through the subsidiaries Anthem HP, LLC (doing business as Anthem Blue Cross and Blue Shield HP) and Wellpoint of NY, LLC. Earlier it sat in the Centers Health Care family.

Wellpoint says the Centers Plan legal entity was renamed Wellpoint of NY, LLC effective March 1, 2026, with the EDI payer ID WLPNT.

Centers Plan for Healthy Living LLC, usually shortened to CPHL, was a New York managed long-term care plan rather than a general health plan, and that one distinction settles most questions about addiction treatment. Run from 75 Vanderbilt Ave in Staten Island, it served Medicaid and dual Medicaid/Medicare adults who needed ongoing care at home across eleven counties, with roughly 53,000 members as of an April 2024 Empire Center report.

The name no longer sells coverage. Elevance Health completed its acquisition of Centers Plan for Healthy Living LLC on December 31, 2024. Medicaid Advantage Plus members moved to Anthem HealthPlus Full Dual Advantage LTSS 2 (HMO D-SNP) on January 1, 2026, and managed long-term care members moved on March 1, 2026: the downstate counties to Anthem Blue Cross and Blue Shield HP, and Erie and Niagara to Wellpoint of NY, LLC. Wellpoint states that the Centers Plan entity itself was renamed Wellpoint of NY, LLC on that date.

That leaves two live questions for anyone holding an old card or an old authorization: what the plan paid for while it existed, and who pays now. This guide covers both, across levels of care, authorization rules, cost sharing, addiction medication, and how to find a program in New York that bills the right payer.
