Does Gravie cover rehab?

Yes. Gravie-administered employer plans list inpatient hospital and residential treatment facility services for mental and substance use related disorders as covered, along with outpatient evaluation, therapy and psychiatric care. Cost sharing and rules come from the employer's own schedule of benefits, so two Gravie members can pay different amounts.

Members on Gravie CHOICE, the ICHRA platform, get treatment benefits from the individual carrier plan they bought, not from Gravie itself.

Is Gravie health insurance?

No, not in the usual sense. Gravie is a Minneapolis benefits company and licensed third-party administrator founded in 2013. Its level-funded plans are employer self-funded plans, with Gravie Administrative Services as claims administrator and excess loss coverage from Risk Solutions Insurance Company, a Vermont insurer.

Under Gravie CHOICE the employer gives a fixed allowance and the employee buys an individual policy from a carrier, which then sets and pays the benefits.

Is Gravie the same as Aetna or Cigna?

No. Gravie has no provider network of its own, so it leases the Aetna Signature Administrators and Cigna PPO networks. An Aetna or Cigna logo on a Gravie ID card shows which network a member uses; Gravie is independent and owned by neither company.

The network on the card also decides the authorization route and the claims address, so check the logo before asking a treatment program whether it is in network.

Does Gravie cover inpatient rehab or residential treatment?

Yes, inpatient and residential stays are covered benefits. On one employer's 2024 Gravie HSA $3,000 plan, inpatient hospital and residential treatment facility services for mental and substance use related disorders are paid at 80% of eligible charges after the deductible in network and 50% out of network.

Each admission needs precertification first, and plan wording allows a claim to be denied when precertification was not obtained.

Does Gravie need prior authorization for rehab?

Yes for inpatient and residential care. Aetna-network Gravie plans require precertification for all inpatient admissions, expressly including mental health, substance use disorder and residential treatment facility stays. American Health Holdings performs these reviews and takes requests at (833) 462-0102, by fax or online.

Send non-urgent requests at least seven business days ahead; acute requests are handled within 72 hours. Cigna-network cards follow the Cigna process printed on the card.

A Gravie card is a door into one employer’s plan and into a leased national network.

That structure decides almost everything about getting into treatment. On a level-funded plan the employer is the plan sponsor, Gravie Administrative Services is the claims administrator, and excess loss cover sits with Risk Solutions Insurance Company, a Vermont insurer whose voting stock Gravie owns. Under Gravie CHOICE, the ICHRA side of the business, the employer sets a fixed allowance and the employee buys an individual policy from a carrier, so that carrier, not Gravie, sets and pays the rehab benefit.

Does Gravie Cover Drug and Alcohol Rehab?

Yes on the employer plans Gravie administers. Gravie-administered schedules of benefits list inpatient hospital and residential treatment facility services for mental and substance use related disorders as covered, along with outpatient hospital, partial hospital and day hospital program services and outpatient office treatment. Cost sharing and the detailed rules sit in the employer’s own plan document.

Every employer group has its own Summary Plan Description and Schedule of Benefits, which is why two Gravie cards can carry very different numbers. For a Gravie CHOICE member the question is different again: the allowance buys an individual market policy from one of more than 340 carriers and 20,000 plans offered across the 50 states and DC, and that policy is what an admissions team needs to look at.

What Does a Gravie Plan Pay For at Each Level of Care?

The whole ladder appears in the plan documents, with the cost share set by the design the employer bought. On Comfort there is no in-network deductible and mental health care in a doctor’s office is listed at no cost to the member. No separate withdrawal management line appeared in the Gravie plan documents read, so medical detox is normally claimed as inpatient hospital care for a substance use related disorder, which is covered and needs precertification. Residential and Inpatient

Residential rehab and inpatient hospital treatment for mental and substance use related disorders are named as covered benefits. On the 2024 Gravie HSA $3,000 Ded/$5,000 OOPM plan these are paid at 80% of eligible charges after the deductible in network and 50% out of network. On the 2024 Gravie HSA 6350 plan they are paid at 100% in network once the $6,350 per person deductible is met.

PHP, IOP and Outpatient

A 2024 Gravie plan lists outpatient hospital, partial hospital and rehabilitation services in a day hospital program among covered services, and the schedule summary groups inpatient, residential, partial hospitalization, intensive outpatient and outpatient office care at the same coinsurance. Outpatient individual and group therapy, medication management and family therapy recommended by a designated provider are covered on that plan.

Does Gravie Need Prior Authorization for Rehab?

Yes for inpatient and residential admissions. The precertification list for Aetna-network plans, effective January 1, 2026, covers all inpatient admissions and expressly names mental health and substance use disorder care and residential treatment facility stays. Partial hospitalization and intensive outpatient are not named as separate items on that list, so the admissions team should ask rather than assume.

Gravie partners with American Health Holdings for prior authorizations, medical necessity review and related appeals. How Much Does Rehab Cost With Gravie?

It turns on the plan design the employer chose. Comfort carries no in-network deductible and charges hospital surgery and procedures until a chosen out-of-pocket maximum is reached; a 2023 Comfort plan at an Indiana school district set that maximum at $4,500 individual and $9,000 family. A 2024 HSA design ran a $3,000 individual and $6,000 family in-network deductible with a $5,000 and $10,000 out-of-pocket maximum.

Out of network the arithmetic changes sharply. That same 2024 plan applied a $10,000 individual and $20,000 family deductible to non-participating providers, paid 50% of eligible charges and carried no out-of-pocket maximum at all, which means an extended residential stay outside the leased network has no ceiling on the member share. Does Gravie Cover Suboxone, Methadone and Other Addiction Medication?

A prescription filled at a pharmacy afterwards runs on the formulary and its tiers: the 2024 HSA design paid generic and preferred brand at 80% after the deductible, non-preferred brand at 50%, and did not cover non-formulary drugs, while the 2022 Comfort sheet listed generics at $0, brand at $75, non-preferred brand at $100 and specialty at $125. Prior authorization applies to specialty drugs and to retail drugs over $1,500, with specialty fills through Accredo on 844.516.3319.
