Rehabs That Take Lifeprint

Lifeprint is a name listed by treatment centers in Arizona as a source of coverage. Drug and alcohol treatment may be covered, with benefits that vary by plan, network and level of care.Arizona centers on this page mention Lifeprint on their own websites, and the listing reflects that wording only. Start with your member card to see which organization manages your coverage. Then call the number on the card and ask whether addiction treatment is covered, whether the facility is in network, and whether prior authorization or a referral is needed. The center can explain its admission steps and whether it still accepts Lifeprint.

No commission, no referral fees. Nothing a facility pays for changes where it appears.

Contact details always go direct. Calls reach the facility itself, never a call centre.

Common questions

Answered from what is listed here.

What is Lifeprint insurance?

Lifeprint is not an insurance company. LifePrint Health, Inc. was a UnitedHealth Group company that arranged medical services, claims and authorizations for UnitedHealthcare and AARP-branded Medicare Advantage members, mainly in Arizona. The coverage behind a Lifeprint card is a UnitedHealthcare Medicare Advantage plan.

Member ID cards for these plans carry claims payer ID LIFE1, and member services for delegated plans is 1-877-370-2843 (TTY 711).

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Is Lifeprint the same as Optum?

Yes. On June 15, 2022, LifePrint Health, Inc. changed its name to Optum Health Networks, Inc., and the former Lifeprint Network is now listed as Optum Medical Network in Arizona and Utah. The Arizona accountable care entity does business as Optum Accountable Care, Arizona.

Optum, Inc. is UnitedHealth Group's health services arm, so a Lifeprint card and an Optum card can sit behind the same UnitedHealthcare Medicare Advantage plan.

Is Lifeprint the same as UnitedHealthcare?

No. UnitedHealthcare is the insurer, while Lifeprint was the delegated provider organization that managed care, referrals and prior authorization for Medicare Advantage members in Arizona. Payer listings for ID LIFE1 read "AARP MedicareComplete thru UnitedHealthcare formerly Lifeprint Arizona", showing the plan itself is UnitedHealthcare.

Both sit under UnitedHealth Group Incorporated of Eden Prairie, Minnesota. Benefits, copays and network rules come from the UnitedHealthcare or AARP plan named on the card.

Lifeprint is not an insurance company. LifePrint Health, Inc. was a UnitedHealth Group company that arranged medical services and ran care management, claims and authorizations for UnitedHealthcare Medicare Advantage members, mainly in Arizona. When Lifeprint appears on a member card, the coverage that pays for addiction treatment is a UnitedHealthcare plan, usually an AARP-branded Medicare Advantage plan.

The name is also retired. On June 15, 2022 LifePrint Health, Inc. became Optum Health Networks, Inc., and Lifeprint Network is now listed as Optum Medical Network / AZ, UT under claims payer ID LIFE1. The Arizona accountable care entity keeps the legal name Lifeprint Accountable Care Organization, LLC, trading as Optum Accountable Care, Arizona. Older cards, payer directories and treatment center insurance lists still carry the Lifeprint wording, which is why the search keeps turning up.

This guide explains what the plan behind the Lifeprint name pays for at each level of care, which admissions need prior authorization and who issues it, what treatment costs before coverage, how medication for opioid use disorder is handled, and how to work out whether a program is in network before admission.
