Rehabs That Take Provider Partners Health Plan

Provider Partners Health Plan is a health plan named by treatment centers in Kentucky and Maryland. Its plans may include drug and alcohol treatment, varying by plan, network and level of care.Facilities listed here name Provider Partners Health Plan on their own websites. Members or family members should contact the plan and the center before admission to check benefits and network status. Ask whether prior authorization or a referral from a care team is needed, and what the member may owe out of pocket. Plans of this kind often coordinate care closely, so the plan’s care coordinator can be a useful first call.

Common questions

Answered from what is listed here.

What is Provider Partners Health Plan?

Provider Partners Health Plans is a Medicare Advantage HMO Institutional Special Needs Plan sponsor based in Columbia, Maryland. Its plans serve individuals with Medicare Parts A and B who live in a nursing home or skilled nursing facility, or who live at home and need nursing-home-level care.

Every plan is an HMO with Part D drug coverage included. It is not a commercial, marketplace or Medicaid insurer, and it operates through separate state HMO entities.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Does Provider Partners Health Plan cover rehab?

Yes, substance use programs and treatment are covered benefits that require prior authorization. As a Medicare Advantage plan it must also cover at least the Part A and Part B benefits of Original Medicare, which include inpatient hospital care, outpatient treatment, partial hospitalization and intensive outpatient care.

Coverage runs through the HMO network, so the treating center normally needs a contract with the plan before an admission is authorized.

Medicare Rehabhttps://rehabseekers.com/insurance-accepted/medicare/
Does Provider Partners Health Plan need prior authorization for rehab?

Yes, prior authorization is required. Inpatient admissions, partial hospitalization, substance use programs and treatment, outpatient hospital services, therapies and most non-participating provider services all need authorization before care begins, and the plan's own utilization management team reviews the request using InterQual criteria.

Planned admissions should be notified ten days ahead and unplanned ones within 24 hours. Members can check on a request at 800-405-9681, TTY 711.

Provider Partners Health Plans is a Medicare Advantage sponsor that writes only Institutional Special Needs Plans, HMO coverage built for Medicare beneficiaries who live in a nursing home, skilled nursing facility, intermediate care facility or assisted living community, or who live at home with a state certification that they need nursing-home-level care. For addiction treatment that shapes everything: the benefit floor is Medicare Part A, Part B and Part D, and the network rules are an HMO’s.

The company is based in Columbia, Maryland, founded by Scott Rifkin, MD, and led by CEO Rick Grindrod, and it operates through separate state HMO entities such as Provider Partners Health Plan of Missouri Inc. For 2026 it covers 268 counties across Illinois, Indiana, Kentucky, Maryland, Missouri, North Carolina, Pennsylvania and Texas, through three plan families: Advantage for residents of a contracted facility, and Community and Essential for enrollees who live at home. Care is delivered with registered nurses and nurse practitioners placed inside partner facilities.

This guide covers which levels of substance use treatment the plan pays for, where prior authorization applies and who makes the decision, what the 2026 premiums, deductibles and cost shares come to, how addiction medication sits between the Part D formulary and the Part B benefit, and how to work out which programs hold a contract.
